Application form Who this form is for 0817 When we refer to Standard Life we mean Standard Life Assurance Limited. This form is for people who want to become members of the Standard Life Stakeholder Pension Scheme. Please read the notes that are contained in this form. If you need any more information please speak to your financial adviser. Filling in this form Before completing this form, please read the Key Features Document (SPP17). The information in this form is mainly needed for tax purposes. It is a serious offence to give false statements. The penalties are severe and could lead to prosecution. The final contract is always between the member and Standard Life. However, when the member is under 16 their legal guardian will act on their behalf. If the prospective member is under 16 years old their legal guardian must complete this form. A copy of the standard terms and conditions of the Plan(s) and the completed application form(s) will be sent to you on request. You must tell us if any information changes before your Plan starts. If you don t, or if you fail to tell us any other relevant information, it may make your Plan void. If you are receiving advice from a financial adviser, you should remember that the financial adviser is acting on your behalf not only by giving you advice, but also regarding completing this form. Part 3a Eligibility To be eligible to make payments to this plan, you must answer Yes to one of the following questions: Are you resident in the UK for tax purposes (see Note A in Part 10)? Money Laundering Regulations To comply with the Money Laundering Regulations, we are required to verify the identity of our customers. We do this by carrying out an online identity check with a reference agency. This is not a credit check and will not affect your credit rating. The agency will verify your identity against public records and confirm whether you have a credit history (but will not disclose any information to us about your actual borrowings). The agency will add a note to your credit file to show that an identity check was requested by Standard Life. If successful, a copy of the results will be held by Standard Life to evidence that your identity has been verified. We regret that we cannot offer an alternative unless the online check does not confirm your identity, in which case we will carry out a manual check. Contribution controls Contribution controls legislation will apply to all employer payments. This means that Standard Life may be required to make a report to The Pensions Regulator if payments are paid late. This means that any electronic payments paid in respect of one month must be received by the 22 nd of the following month; all non electronic payments must be received by the 19 th of the following month. The legal requirements on employers who are subject to contribution control legislation mean that Direct Debit is only a suitable payment method where payments are paid to us at the same frequency as you are paid. Recycling a tax free lump sum If you recycle a tax free lump sum you may face a tax charge. Leaflet GEN449 explains what recycling means and what the tax consequences are. You can choose to invest in more than 1 fund, but you must ensure that the total percentage adds up to 100%. Please note that if you choose a Lifestyle profile and wish to combine this with another investment fund, you can only combine this with the with profits fund available to you. It is not possible to select more than one Lifestyle profile at a time. If you do not make an investment choice, please see your Key Features Document for details of how your payment(s) will be invested. Part 9 Declaration by legal guardian To comply with the Money Laundering Regulations, we are required to verify the identity of our customers. We do this by carrying out an online identity check with a reference agency. This is not a credit check and will not affect your credit rating. Part 10 Notes The descriptions and definitions given in these Notes are to help you complete this form and are not binding in law. The descriptions which apply are those set out in the legislation. If you are in any doubt you should contact a pensions practitioner, the Scheme Administrator, your accountant or, where appropriate, your Inspector of Taxes. A. If you are in any doubt as to whether or not you are resident in the UK (this can also apply to a period of residency within the current tax year), you should check with your UK tax inspector. If you cease to be resident in the UK, you can still make payments in a year of assessment as long as you were so resident at any time in the previous 5 tax years. B. A civil partnership is a legal contract between two partners of the same sex. C. If you are in any doubt as to whether or not you are a Crown Servant performing duties abroad, you should check with your payroll department. D. An occupational pension scheme is an employer s pension scheme which promises to provide you with a pension or a pension and a lump sum on your retirement. It does not include: (a) this scheme or any other stakeholder or personal pension scheme even if your employer makes payments to it; (b) any scheme which pays benefits only on your death; or (c) any scheme which is not registered with HM Revenue & Customs. The scheme is not necessarily run directly by your employer. We need to ask you for the gross payment you will make to your pension plan. The gross figure is not the actual amount you will pay, for example, by Direct Debit. What you actually pay us is known as your net payment. HM Revenue & Customs pays us your tax relief automatically, which we add to give the gross amount. Notes: If you don t have any relevant UK earnings, the maximum amount that you, or a third party can pay into a Stakeholder Pension Plan and any other pension plan is 3,600 gross each year which works out as 12 monthly payments of 240 net.
