The HSA deposit account No set-up fee No minimum deposit required to open the HSA No minimum balance required The monthly bank account fee is waived on balances of $3,000 or more Note: Effective September 1, 2013 a monthly balance of $4,850* must be obtained to waive the monthly fee. Interest-bearing account Tiered-interest rate structure with applicable rates determined by the daily balance *The Balance Waiver Amount is calculated annually as the average between the self-only and family plan maximum contribution limits. HSA Bank debit card capabilities Accountholder s can pay for IRSqualified medical expenses from HSA funds wherever VISA is accepted. Accountholder s have the ability to withdraw up to $300/day at ATM with PIN and up to $2,000/day at point of sale with signature. To help prevent misuse of HSA funds, the debit card is only authorized at merchants that provide medical products and services.

Electronic file format The Electronic File Enrollment process allows an employer, or a third-party to open HSAs for employees by submitting an electronic data file to HSA Bank's SFTP server. HSA Bank will open the accounts based upon the employees' intent, which may be in the form of a checkbox on the benefits enrollment system or in the form of a request to make payroll contributions to the HSA. Once HSA Bank receives the electronic file and verify the identity of each employee, the account will be opened.
Auto enrollment HSA-qualified employees can be easily enrolled in an HSA by uploading a Microsoft Excel.csv file. No employee participation is required. Here's how it works: 1) Log into the Online Employer Administration Area 2) Click on "Enrollment" and "Auto Enrollment File" in the menu. 3) Download the Auto Enrollment File template. Formatting instructions are provided. 4) Enter the required data in the template and save it as a.csv file. 5) Upload the completed.csv file. If the Auto Enrollment File meets the necessary requirements, it will be uploaded and processed within two business days.
Group online enrollment (GOE) GOE is a two-part system employer set up and employee enrollment. The employer chooses a set-up option (either Post Approval or Pre-Approval) that determines when employees enroll and how their applications are approved. Once set-up option is selected, the employer will be provided with a custom enrollment link in the online Employer Administration Area. The employer set up allows them to choose the way their employees enroll, when the applications are processed and who will pay for account fees.
