Online Fitness Assessment Complete the Online Fitness Assessment. Earn 1 000 Vitality points. Non - smokers declaration Complete the non-smokers declaration. Earn 5 000 Vitality points. Mental Wellbeing Assessments Complete the Mental Wellbeing Assessments. Earn up to 2 500 Vitality points. Kid s Vitality Health Review Complete the Kids Vitality Health Review*. Earn 2 000 points. * Applicable to members with child dependants (younger than 18 years)
Step 2: Learn about your health The next step is to learn more about your health status at pharmacies, dietitians and biokineticists in the Vitality Wellness Network. You can earn up to 7 500 points. Vitality Health Check Complete blood glucose test, blood pressure test, cholesterol test and weight assessment at a Vitality Wellness Network pharmacy near you. Earn up to 14 000 Vitality points. Vitality Fitness Assessment Complete the Vitality Fitness Assessment at a biokineticist near you. Earn 7 500 Vitality points (you can earn this twice a year). Vitality Nutrition Assessment Complete the Vitality Nutrition Assessment at a dietitian near you. Earn 5 000 Vitality points.

Cervical cancer vaccination This vaccination for females is available at your GP. Only females between the ages of 9 and 25 will earn the points. Earn up to 1 000 Vitality points.
