BENEFITS SUMMARY Physician @ Physician Practices HEALTHCARE COVERAGE Individual and family coverage is available and fully paid by Florida Hospital Heartland Medical Center. Traditional Plan offers FSA (Flexible Savings Account), costs more per paycheck, pay less when services are rendered A $50 premium per month will be charged to those that attest to using tobacco. DENTAL COVERAGE Dental coverage is designed to provide you with financial assistance toward dental expenses. Dependent children are eligible up to age 26. OPTICAL COVERAGE This coverage provides vision benefits through VSP Vision Care with copays and eyewear discounts. Dependent children are eligible up to age 26. RETIREMENT Adventist Healthcare Retirement Plan (AHRP) The retirement plan includes an employer basic contribution of 2.6% and matching contribution of 50% for up to 4% on each dollar of pay you contribute. 100% vested after 5 years. Valic Retirement A tax sheltered annuity 403(b) plan offered in addition to AHRP. Services provided include: Retirement Projections, Asset Allocation Advice, Mutual Funds, Roth IRAs College Saving Program, Non-Qualified Annuities INSURANCE Cancer Insurance Employee can purchase individual or family coverage through American Heritage Life Insurance Company. Benefit paid directly to you with no coordination of benefit. Flexible Spending Accounts Medical and dependent care spending accounts available through payroll deduction. Life Insurance Employee receives one times annual salary, which includes AD&D. Employee may elect additional coverage of up to four times their current annual salary. Dependent Life Insurance is available for spouse and dependents. Plan is portable. Long-term Disability Benefits begin on the 181st day of extended illness or injury. Coordinated with social security disability guaranteeing 60% of income to age 65 or return to work date. Premium are paid for by Florida Hospital Heartland. Short-term Disability Benefits begin after the 14 th day of an accident or illness. 60% of salary is paid through 26 weeks of an extended illness or injury.

1 of 2 PAID LEAVE DAYS-PLD S Each full calendar year the provider will be entitled to 21 PLD s (paid leave days). CONTINUING MEDICAL ALLOWANCE Annually the company shall authorized $2,000 per calendar year for expenses related with attending accredited continuing medical education. In addition, Physician shall be given FIVE (5) paid days per year to attend approved continuing education activities which shall not be subject to accumulation and which are in addition to the regular 21 personal leave days provided annually. LICENSE AND MEMBERSHIP FEES For each calendar year, FHHD on behalf of the physician, remit all fees necessary to maintain Physicians license to practice medicine in the State of Florida together with such membership fees for professional organizations related to the Physicians medical practice, which collectively shall not exceed ONE THOUSAND FIVE HUNDRED DOLLARS ($1,500.00). No more than one year s dues shall be paid at any one time in any one year RELOCATION REIMBURSEMENT FHHD shall reimburse Physicians approved relocation expenses up to a maximum of FIVE THOUSAND DOLLARS ($5,000.00). The amount of relocation will be amortized on a monthly basis over the first twelve (12) months of the Physicians Employment Agreement. If termination occurs within the first twelve (12) months, then Physician will repay FHHD the amount of any unamortized portion of the Relocation Reimbursement amount within thirty (30) days of the termination date. 2 of 2
