Medical Plan design and rates are changing in all plans Anyone currently in Employee + One or Family need to review their defaulted coverage and plan elections Benefit elections made during Open Enrollment become effective January 1, 2014 If no action is taken by November 30 th All your current elections other than FSA remain in effect for 2014 Includes medical, dental, life and dependents covered Benefit elections must be completed online through: My Employment Information, My Benefits no later than November 30 th We strongly encourage you to complete your enrollment as soon as possible rather than waiting until the deadline. November 30 th is a Saturday and during the Thanksgiving holiday. No one will be available to assist you during this time. The online system will be available until midnight on November 30 th. 3
Review ALL your 2014 benefit elections and options Through your spouse s or domestic partner's employer Medical/dental plan coverages and costs Supplemental and Dependent Life Insurance Health Care or Dependent Care Flexible Spending Accounts (FSA) Reconfirm your beneficiaries and covered dependents Complete any enrollment or change forms and send to Human Resources by November 30th 4

New FSA Rollover rules for Health Care Now able to roll over up to $500 into the following year Eliminates the year end or first quarter scramble to spend remaining FSA funds Does not apply to 2013 money FSAs allow you to use before-tax dollars to reimburse yourself for eligible out-of-pocket medical and dependent care expense. Use Benefits Strategies online calculator to determine how much to contribute and your potential tax savings 10

Dental plan design and rates remain the same Life Insurance Basic and Supplemental All benefits eligible employees are covered for $50,000 basic You may purchase personal supplemental coverage in $50,000 increments to a maximum of $250,000. Dependent Life covers your spouse and/or your children Coverage is $10,000 for your spouse and $$4,000 for each child Cost is $1.26 per pay or $2.74 per month Evidence of Insurability (EOI) form needed Increases in Supplemental Life of more than $50,000 Enrolling spouse for Dependent Life All NEW Enrollments for either Supplemental or Dependent Life require completion of an enrollment form 12
