Important! If you are currently enrolled in the Consumer Access Plan: As announced last year, the Consumer Access Plan will not be offered in 2018. Therefore, you MUST select a different medical plan during Annual Benefits Enrollment, Nov. 2-17, 2017. Otherwise, your coverage will automatically default to the Sprint Basic Medical Plan at the same family coverage level you currently have. If you waive medical coverage: No credit will be available. 2

ADDITIONAL MEDICAL PLAN CHANGES Core, Basic, and Health Account plans: Dialysis will be covered only at network facilities, consistent with other specialty procedures (e.g., fertility treatments, bariatric surgery). Kaiser (Hawaii only) mandatory changes by Kaiser include: Infertility procedures: Changing to 20% coinsurance from $20 copay Generic Rx copay: Decreasing to $3 from $5 Specialty Rx copay: Increasing to $200 from $75 PRESCRIPTION DRUG CHANGES FOR ALL MEDICAL PLANS (EXCEPT KAISER) Co-pay assistance changes for Specialty Medications: If you take a Specialty Medication and the drug manufacturer provides you with financial assistance for the cost of your medication: The amount that the manufacturer pays will not be applied to your Plan deductible/outof-pocket limit accumulators. Instead, the portion you pay after the manufacturer covers their portion, will apply to your Plan accumulators. < more > 3

SUPPLEMENTAL HEALTH INSURANCE PLANS Starting this year, partners have the option to enroll in up to three supplemental health insurance plans as part of the annual benefits enrollment process. You may only enroll in these plans during this window. NOTE: These limited benefit plans are NOT traditional health insurance and should NOT be considered a replacement for enrolling in one of the Sprint medical plans. Instead, these are additional optional plans that supplement your standard coverage. Any cash benefits received from these plans are independent of any coverage by medical insurance. 2018 Supplemental Health Insurance Plans Plan type When it applies Benefit Details Accident insurance Insured by Voya (Replaces coverage by Aflac) Pays benefits following treatment for specific injuries and events resulting from a covered accident while off the job. Examples: Fractures, dislocations, lacerations, chiropractic treatment Provides payment for covered accidents and injuries based on severity. Find more information on covered benefits in the plan highlights. Also includes an annual $60 health screening benefit. Plan highlights Hospital confinement indemnity insurance (supplemental hospitalization coverage) Insured by Voya Helps provide financial protection for covered individuals for hospital admission/stays. Provides a lump-sum payment for hospital, critical care unit or rehabilitation facility stays. See plan highlights for initial admission and daily benefit amounts. Plan highlights Critical illness insurance Insured by The Hartford (Replaces coverage by MetLife) Applies to newly diagnosed covered illnesses. Examples: Newly diagnosed cancer, heart attack, stroke. Provides a lump-sum payment for covered illnesses. The cash received can help pay medical bills, the added expenses that medical insurance may not cover, or day-to-day living expenses. VISION PLAN CHANGES Introducing Sprint s NEW vision administrator for 2018: Davis Vision With Davis Vision, partners and their family members will get to experience the same coverage as the current Vision Plan, but with additional benefits. Fully covered frames at Visionworks and with Davis Vision s Exclusive Collection As a Davis Vision member, you have access to more than 750 Visionworks stores, which offer the largest in-store frame assortment in the industry. With an average of 2,000 frames per store at no out-of-pocket cost to you, you ll can choose from a wide variety of shapes, styles, colors and brands. Members also receive 50 percent off additional pairs of eyewear. The Exclusive Collection can be found at nearly 9,000 independent provider locations nationwide. These frames are available to you at no cost and include options that have retail values of up to $195. Fully covered supply of contact lenses (about a six-month supply) from the Exclusive Collection of contact lenses Davis Vision's Exclusive Collection of contact lenses is available at participating provider locations and features many popular brands. When the Exclusive Collection of contact lenses is prescribed by a participating provider, the contact lens evaluation and fitting are also covered at 100 percent (no cost to you). Note: You still get the same great benefits you have today, if you choose not to select frames or contact lenses from the Exclusive Collection, but at participating retail and independent providers. HEALTH SAVINGS ACCOUNT Sprint s Health Savings Account (HSA) will continue to be administered by HealthEquity. New HSA maximum contribution amounts: Individual: $3,450 ($50 increase)* Family: $6,900 ($150 increase)* * Employee partners age 55 or older are eligible to make an additional catch-up contribution of $1,000. Your 2017 participation and annual contribution election (if applicable) will automatically continue as your contribution for 2018 unless you change it. Reminder Sprint contributes to your HSA: If you contribute just $24 per year to your HSA, Sprint contributes: Up to $500 toward your individual maximum contribution, Up to $1,000 toward your family maximum contribution. More information on HSA accounts: Visit www.healthequity.com/sprint, or call 844-396-0220 Watch the replay: Health Savings Account webinar LIFE AND ACCIDENTAL AND DEATH AND DISMEMBERMENT (AD&D) PLAN CHANGES In order to ensure that all employee partners have financial protection in the event of death or serious accident, Sprint s Life Insurance and Accidental Death and Dismemberment plans are changing: Sprint will now provide at no cost, Basic Life Insurance and AD&D coverage in an amount equal to 1x your eligible pay up, to a maximum of $50,000. o The waive benefit option with a waive credit and $10,000 benefit option for Basic Life Insurance and Basic AD&D plans will no longer be offered. o It s important that you review/designate beneficiary information. You can do this during the Nov. 2-17 benefits enrollment site, or at any time during the year, via Employee Self Service (ESS). < more > 6

SUPPLEMENTAL LONG TERM DISABILITY PREMIUMS Supplemental Long Term Disability premium rates are being reduced for 2018. When you view your per-paycheck premiums in the Benefits Enrollment section of Employee Self Service (ESS), your cost will reflect the lower insurance premium rates applicable to your age and your benefit-eligible earnings. Basic and Supplemental Long Term Disability coverage and benefits are not changing. FLEXIBLE SPENDING ACCOUNTS (FSA) Health Care FSA: New maximum annual contribution amount $2,600 ($50 increase). WageWorks, Inc. is now the administrator for Flexible Spending Account plans. NO CHANGES TO THE FOLLOWING PLANS: Dental plan Group Legal plan < more > 7

REMINDERS Currently enrolled in Accident and/or Critical Illness insurance? Action required: You must enroll during Annual Benefits Enrollment to continue with payroll deduction. Otherwise, you ll need to set up direct bill with current vendor or will no longer have coverage in 2018. Verify your medical-surcharge status for a covered spouse/domestic partner Confirm your medical-surcharge designation is correct: Your current election will carry forward, and you don t need to re-designate your status unless there is a change. Note: Sprint will conduct random audits in 2018. Flexible Spending Account (Health Care and/or Dependent Care) Action required: If you are enrolled in a Flexible Spending Account (Health Care and/or Dependent Care), you ll need to actively enroll during Annual Benefits Enrollment and select the amount you wish to contribute in 2018. o Prior-year elections will NOT carry forward. Life and Accidental Death and Dismemberment coverage Action required: If you had previously waived Life and Accidental Death and Dismemberment coverage, you will need to name a beneficiary for these plans during enrollment. Health Savings Account contributions do not need to be re-elected each calendar year. However, you ll need to actively enroll if you wish to increase your election to the new allowed maximum contribution amount. Benefits confirmation statement: Once you ve enrolled, you ll receive a confirmation statement via email after one business day. o Be sure to review your statement carefully.
