STEP 1 Employee Usage Agreement Select Continue to proceed to the next step.
STEP 2 Legal Agreement Select Continue to proceed to the next step.
STEP 3 Welcome to Open Enrollment 2016 There is a video introducing you to the portal, as well as a forms library that can be accessed to review all 2016 benefit options. Select Continue to proceed to the next step.
STEP 4 Employee Demographics Red boxes indicate required fields Select Save & Continue to proceed to the next step.
Step 4 Continued A change box will appear to ensure you would like to proceed with the change. A confirmation box will appear verifying the change has successfully occurred.
STEP 5 Add a Spouse Select Add to include your spouse s demographics. Select Continue if you do not have a spouse and would like to proceed onto the next step.
Step 5 Continued Red boxes indicate required fields. Enter spouse information into form and select Save & Continue once complete to proceed.
STEP 6 Add a Dependent Select Add to add include your dependent s demographics. Select Continue if you do not have dependent(s) and would like to proceed onto the next step.
Step 6 Continued Red boxes indicate required fields. Enter dependent information into form and select Save & Continue once complete to proceed.
STEP 7 Emergency Contact Enter emergency contact information and select Save & Continue to proceed to the next step.
STEP 8 Healthy Campus Program This is an information page regarding the Healthy Campus Program. No action is required to be taken. Select Continue to proceed to the next step.
STEP 9 Medical Enrollment You can enroll, waive and compare plans
Step 9 Continued A confirmation box will appear upon waiving or enrollment into the medical plans.
STEP 10 Spousal Affidavit Be sure to read the affidavit thoroughly before selecting an option. Select Save & Continue to proceed onto the next step.
STEP 11 Criteria for Surcharge Answer all questions regarding your spouse s employment and healthcare options and electronically sign in the box provided.
STEP 12 Spousal Surcharge Confirmation A confirmation page will appear if you are required to pay the $125.00 monthly spousal surcharge. Select Continue to proceed to the next step.
STEP 13 Vision Enrollment This step is to confirm you are automatically enrollment into the vision plan if you enrolled into one of the medical plans. Select Continue to proceed to the next step.
STEP 14 Dental Enrollment A confirmation box will appear verifying the change has successfully occurred. If you enrolled into a medical plan than you will automatically be enrolled into the basic dental plan unless you enroll into the buy-up option.
STEP 15 Health Savings Account High Deductible Plan Participants Only A confirmation box will appear verifying the change has successfully occurred. Enter a monthly contribution amount and select Enroll to participate in a HSA.
STEP 16 Flexible Spending Account A confirmation box will appear verifying the change has successfully occurred. Enter the annual amount you would like to contribute into a Flexible Spending Account and select Enroll.
STEP 17 Dependent Care Spending Account A confirmation box will appear if you would like to waive this benefit. Enter the annual amount you would like to contribute into a Dependent Care Spending Account and select Enroll.
STEP 18 ONU Covered Life Insurance This is an information page regarding the ONU covered Life Insurance benefit. No action is required to be taken. Select Continue to proceed to the next step.
Step 19 Voluntary Life Insurance Use the pull-down tab to select voluntary life insurance coverage. If you currently participate, re-enrollment is not necessary. This benefit can be waived if you are not interested in enrolling.
STEP 20 AFLAC Enrollment Complete the required fields by selecting the pull-down tab and selecting an option. If you express interest in a policy, HR will reach out to you to set an appointment.
STEP 21 Long Term Care This is an information page regarding the Long Term Care policy. If you would like to enroll, please go to www.genworth.com or call 1.800.416.3624.
STEP 22 TIAA-CREF Contributions Complete the required fields by selecting the pulldown tab and clicking on the contribution amount you would like to begin contributing in 2016. Electronically sign in the box and select Save & Continue to proceed onto the next step.
STEP 23 Beneficiary Information Select the pencil to edit your beneficiary information. Add, change or remove beneficiaries from your UNUM life insurance coverage.
STEP 23 Continued After editing your beneficiaries, select Save & Continue to proceed onto the next step.
FINAL STEP Confirmation Page The confirmation page will list your current enrollment elections, future enrollment elections, and any benefit elections you may have waived for the 2016 enrollment year.
FINAL STEP Continued Select Finish once you are ready to submit your enrollment election for 2016. A conformation box will appear verifying your enrollment was submitted successfully.
MAIN PAGE Changing Elections If you would like to change your benefit elections prior to the December 7 deadline, select the Change option above.
