CUSTOMER GUIDE PROGRESSIVE CARE Trauma Insurance An innovative way of covering you for serious illness or injury. TOTALCAREMAX

FROM SOVEREIGN A different way of looking at trauma insurance It s unfortunately common for New Zealanders to face a trauma such as a serious illness or injury at least once in their lives. If you or one of your children were to suffer a trauma, the last thing you d want to be worrying about is money. While our public health system can provide good emergency care and treatment, individuals and their families often experience financial hardship as a result of a serious illness or injury to a family member. Having trauma insurance can provide much-needed financial flexibility to you and your family in a difficult time. Unlike traditional trauma insurance, which pays out just once, Sovereign s Progressive Care can provide continuing payments. Claim payments are linked to the severity of the condition: the more serious it is, the bigger the payment. And you needn t wait until your condition is life-threatening to make a claim. Progressive Care covers more than 60 defined medical conditions across the five categories below and is available to children as young as two years old: Cancer Heart & Arteries Brain & Nerves Loss of Function Other Health Events Covering childhood While most of us can comprehend the implications if we were to suffer a trauma, it s easy to underestimate the impact on your family and finances, should a child become seriously ill or injured. In times like these your children need more than money, they need you. The last thing you want to be worrying about when your child is seriously ill or injured is how you ll afford time off work to be by their side while they receive treatment, and cover unexpected costs while continuing to look after the rest of your family. Sovereign s trauma insurance for children provides a lump sum of up to, and is available to children as young as two years old, without having to be linked to a parent s policy. So, if you re a parent, having Progressive Care for your children provides even greater protection and peace of mind giving you the ability to be there for your loved ones, should they need you. PAGE 2

HOW IT WORKS Here is a quick snapshot of the categories in Progressive Care and the severity levels within each. You can claim for any of the specified conditions throughout the life of your policy. Depending on your condition, you might claim 10%, 25%, 50%, 75% or 100% of the cover within each of the categories. You could also receive additional payments if: > > Your condition becomes worse > > You suffer from a more severe, related condition > > You suffer from an unrelated condition. Cancer Heart & Arteries Brain & Nerves Loss of Function Other Health Events This is an illustrative guide only. Claims are subject to the terms and conditions of the policy document. Benefit payments Lump sum payments are made in line with severity of medical condition. Severity levels Level 5 (least severe) through to level 1 (most severe) with benefit calculated on 10%, 25%, 50%, 75% and 100% of the sum assured available. Quick facts about Progressive Care Minimum sum assured $50,000 Maximum sum assured for children aged 2-15 years $1,500,000 for over 15 year olds Eligible ages From 2 years onwards with individual consideration from age 70 Term of cover Renewable each year with no age limit Accessibility of cover You could receive multiple payments if your condition gets worse, you suffer from a more severe, related condition, or you suffer from an unrelated condition Choice of cover Progressive Care is available as either Personal retail cover for ages 2 years onwards or as Business Protection from ages 16 years onwards as key-person cover for employers. From age 16 Progressive Care can be taken as a standalone benefit or as an accelerated benefit linked to a TotalCareMax Life Cover policy PAGE 3

Your cover continues even after you claim When your policy begins, the amount of cover in each of the five Categories of Conditions is the same. The following examples show how claims might be managed and amounts affected. Stand down periods and other conditions apply. Please refer to the policy document for details. First claim If a claim is paid under a specific Category, the amount of insurance remaining within that particular Category for future claims decreases by the claim payment amount. If you receive a 100% claim payment, the insurance for that particular Category is exhausted. However, the amount of cover for all other Categories remains unaffected. Tom s claim Tom holds a policy with of insurance and he suffers a heart attack of severity level 3 (a 50% claim payment). That then means the cover available within each of the five Categories of Conditions following the claim payment is as follows: Cancer Heart & Arteries $125,000 Cancer: i.e. less a claim payment of $125,000 ( X 50%) Brain & Nerves Loss of Function Other Health Events PAGE 4

Related claims If your medical condition worsens or you suffer from a related medical condition, Progressive Care enables you to receive an additional claim payment (subject to cover being available within the Category). In order to receive a payment, the severity level of the claim must be higher than the severity level of the preceding related claim. In these cases, the claim payment is based on the increase in the severity level percentage (subject to cover being available within the Category). Expanding on Tom s claim If Tom was to suffer another heart attack of severity level 3 then no claim would be payable since there was no increase in severity. However, if Tom was to suffer a heart attack of severity level 2 (a 75% claim payment), then you would receive a benefit of $62,500, i.e. (75% - 50%) x = $62,500 Following this second claim payment, the cover available within each of the five Categories of Conditions is as follows Cancer Brain & Nerves Heart & Arteries $62,500 Cancer: i.e. less the initial claim payment of $125,000 and the further claim payment of $62,500 Loss of Function Other Health Events Unrelated claims If you suffer a medical condition unrelated to any previous claims, you may be eligible to receive an additional claim payment. The amount of the claim payment will depend on the severity of the condition, whether the claim event occurred within 12 months of the previous unrelated claim, and the cover available within the Category. This means claims paid for a claims event which occurred within the previous 12 months, regardless of Category, will be deducted from any amount payable for any new, unrelated claim. This deduction will not apply if either the current or previous claim is for a condition which resulted from an accident. Please refer to the policy document for further details. PAGE 5

For more information about these and other exclusions and stand downs, please refer to the relevant policy wordings which can be found on sovereign.co.nz or speak to your Adviser. PAGE 7

FLEXIBLE WAYS TO INCREASE YOUR COVER Life changing events can increase your financial responsibilities. The Special Events Increase Facility and the Future Insurability Benefit offer you the opportunity to increase your Progressive Care up to once per year without having to provide further medical information. Please note that these increases are not available if the life assured has already suffered a condition under Progressive Care. Where a claim of another type has been notified or accepted for the life assured, Sovereign will exercise its discretion in determining if the increase can be applied based on the nature of the claim. These benefits are accessible from age 16 and up to age 55. Special Events Increase Facility Enables you to increase the sum assured under your Progressive Care following a significant event in life which results in increasing financial responsibilities. These significant events include: Having a child, getting married, taking out or increasing a residential home loan, receiving a significant salary increase and other major events. Future Insurability Benefit You can apply to increase your sum assured at every third policy anniversary without providing further medical information. Note that limits and conditions apply and every increase in cover will require an increase in premium. PAGE 8

OTHER BENEFITS YOU CAN ADD ON We understand everyone s needs are different. Below are some optional benefits you can add to your insurance to give support where you need. Note: Optional benefits can be applied for once a life assured is 16 years old or over. Children s and Maternity Benefit This provides additional children s trauma insurance up to a further 50% of the sum assured (up to $50,000) per child of the life assured (for up to 10 children). This benefit is paid if a named child of the life assured suffers any covered condition under Progressive Care except for the Optional Total Permanent Disablement condition (if applicable). This optional benefit is paid once per child per benefit and is paid in addition to the built-in Children s Trauma Benefit. It can be added to either Progressive Care or Comprehensive Living Assurance, but not both for the same life assured. The Optional Children s and Maternity Benefit also provides cover for three defined serious pregnancy complications and a bereavement support if the child of the life assured should pass away before age 3. A 14 day survival period applies. Exclusions apply for any congenital conditions. Total Permanent Disablement Condition You can add cover for own occupation Total Permanent Disablement (TPD) as a claimable condition under Progressive Care. If you select this, an own occupation TPD condition is added as a severity level 1 condition (i.e. 100% payment) under the Progressive Care loss of function category. This enables you to claim under the loss of function category for a wider set of conditions not otherwise covered under Progressive Care, which may lead to totally and permanent disability. Waiver of Premium This can remove the burden of paying premiums when you are unable to work due to total disability. We will waive your premium payments, while your insurance remains in place. Accidental Injury Cover This optional benefit provides a single up-front payment, should you be injured accidentally. You can use it for whatever you need: covering treatment costs, getting taxis to work, hiring someone to help with children, or simply taking time off work to recover. You choose how much cover you need, up to $5,000 a month, and the lump sum you receive is based on the treatment you require. For example, a broken leg that requires a cast entitles you to a three month payment, totalling a maximum of $15,000. This is paid as a lump sum once your claim is approved. Making a claim under this optional benefit is not affected by payments from ACC or other insurances you may have. Specialists and Diagnostic Testing with Best Doctors This benefit provides coverage up to $5,000 per year for specialist consultations, diagnostic imaging and tests. Best Doctors* offers the chance to seek a second opinion from an international network of medical specialists. *Best Doctors is a trademark of Best Doctors, Inc., in the United States and other countries, and is used under license.
