Make the right choice for you and your family For 2018, we re introducing new medical plan options and expanding access to quality providers across the L.A. area. And we re adding incentives that help you save money $780 a year or more when you take actions to support your health. Your medical plan options For 2018, you ll have high-quality, affordable new options that include USC medical providers and other providers carefully chosen to give you the best care, conveniently located all around Southern California. Here are your medical plan options: 2018 MEDICAL PLAN PROVIDER NETWORK USC Trojan Care EPO All new, low-cost plan designed specifically for USC employees USC PPO Formerly called Network Medical Plan Most choice, highest cost USC HMO Replaces Anthem BlueCross HMO Lower cost than Kaiser HMO Anthem MyChoice HMO No paycheck cost Offers access to outstanding Keck Medicine of USC and other carefully selected providers No out-of-network coverage except for emergencies Tier 1: Keck Medicine of USC providers Tier 2: Blue Cross Prudent Buyer/BlueCard providers Tier 3: Out-of-network Tier 1: Custom network of carefully chosen providers Tier 2: Same Anthem providers as before No out-of-network coverage except for emergencies Anthem MyChoice HMO providers only No out-of-network coverage except for emergencies

Checklist There s a health care plan for everyone but you need to act now to get the right plan for you at the best price. Use this checklist to make sure you don t miss anything: LEARN P Read this guide for an overview of what s changing and what you need to do. Go to benefits.usc.edu/open-enrollment-keck-norris-union for all the details. Attend a benefits presentation or on-campus benefits fair. ENROLL Log in to Workday between October 30 and November 21 to: SAVE Choose the medical plan that s best for you and your dependents. Designate your tobacco-free status to save $25/month.* Indicate whether your covered spouse or registered domestic partner has employer-sponsored medical coverage available.* Opt out of vision coverage if you don t want it in 2018. Select the amount you want to contribute to your health care and dependent care Flexible Spending Accounts (FSAs) in 2018. Print your enrollment confirmation statement when you re finished. Save $40/month with the health assessment credit:* Register at usc.bioiq.com, complete the online health questionnaire, and schedule your in-person health screening. Complete your health screening before March 31, 2018. Save $10 off your copay every time you see the doctor (except specialists): Enroll in the USC Trojan Care EPO. Email healthplans@usc.edu after January 1, 2018, to designate a primary care physician (PCP) for yourself and each of your dependents. * Because the Anthem MyChoice HMO is offered at no cost to you, incentives and surcharges are not applicable if you are enrolled in that plan. If you don t enroll by November 21: You will be enrolled automatically based on your 2017 medical plan election, covering the same dependents you cover now: - IF YOU RE IN THE 2017 Network Medical Plan YOU LL BE ENROLLED IN THE 2018 USC PPO - IF YOU RE IN THE 2017 Anthem HMO YOU LL BE ENROLLED IN THE 2018 Anthem MyChoice HMO - IF YOU RE IN THE 2017 Kaiser HMO YOU LL BE ENROLLED IN THE 2018 Anthem MyChoice HMO - IF YOU RE IN THE 2017 Anthem MyChoice HMO YOU LL BE ENROLLED IN THE 2018 Anthem MyChoice HMO If you waived medical coverage in 2017, you will not receive medical coverage in 2018. You will not qualify for the tobacco-free credit (savings of $300 per year).* You will pay the working spouse surcharge for your covered spouse or registered domestic partner, if any (cost of $600 per year).* You will be enrolled automatically in vision coverage (cost of about $110 per year for employee-only coverage). You will not be able to use a health care or dependent care Flexible Spending Account (FSA).

The USC Trojan Care EPO and prescription drugs The USC Trojan Care EPO prescription formulary is designed to provide a wide range of medications while helping to keep the EPO a lowcost plan. There s a chance that if you switch from the Network Medical Plan to the EPO, your specific prescription may not be on the EPO s formulary. Depending on whether switching medications would have an effect on your care, your current drug may be covered for up to 90 days, or until the course of treatment ends. Get more details and the full USC Trojan Care EPO formulary at benefits.usc.edu/openenrollment-keck-norris-union. You can also contact the HR Service Center at (213) 821-8100. Time to consider your options? Your health plan choices have changed this year, so it may be a good time to take a close look at your needs and think about which plan would be best for you. These are some factors to consider. QUALITY: All our plans offer quality health care services you can be confident that all your medical plan options get you access to top-quality providers. COST: Keep in mind the per-paycheck cost is only part of the picture. There s also how much you ll pay out of pocket when you need care like deductibles, copays, and prescription costs so be sure to compare. USC DOCTORS: If you want to choose world-class Keck Medicine of USC providers, you ll need to enroll in the USC Trojan Care EPO or the USC PPO. PROVIDER CHOICE: Do you like the idea of choosing any doctor you want? If you want coverage for out-of-network providers, you ll need to enroll in the USC PPO. KEEPING YOUR CURRENT DOCTOR: Staying in the same plan is one way to keep the same doctor, but with the new USC custom network, your doctor may also be available in a different plan. Check the online provider directory to find out.

Other things to know for open enrollment Healthy choices incentives: Protect your health. Save money. The new healthy choices incentives reward you for taking an active role in your health care. Take these simple steps to improve your health, and save $780 or more.* 2018 HEALTHY CHOICES INCENTIVES To get these savings: Do this: Where and when: Tobacco-free credit: $25/month ($300/year) off your medical plan contributions* Verify that you do not use tobacco products, or commit to enroll in a cessation program In Workday while you re enrolling in 2018 benefits Health assessment credit: $40/month ($480/year) off your medical plan contributions* Complete the online health questionnaire and Complete the in-person health screening At usc.bioiq.com and at an on-campus event, a nearby lab, or your doctor s office before March 31, 2018 PCP discount: $10 off your copay every time you see your primary care physician (PCP) or mental health provider Enroll in the USC Trojan Care EPO and select a PCP Email healthplans@usc.edu after January 1, 2018, and at least 15 days before you visit the doctor Your privacy is protected, as always. No one at USC who is not involved in your clinical care will see any of your individual results from the online health questionnaire or in-person health screening. Your individual results will not impact your eligibility for health care coverage or the amount you pay for your coverage (other than the $40/month credit for completing the assessment*). * Because the Anthem MyChoice HMO is offered at no cost to you, incentives and surcharges are not applicable if you enroll in that plan. NEW Automatic vision enrollment and exclusive savings For 2018, you will be enrolled automatically in vision coverage unless you actively opt out during open enrollment. Your monthly cost for employeeonly vision coverage will be about $9 per month, so if you don t want coverage, be sure to indicate that during enrollment. The vision plan will still be provided through the same VSP network, but you ll enjoy lower copays and higher allowances for frames and contact lenses when you choose USC Roski Eye Institute providers. NEW Working spouse surcharge* In 2018, if your spouse s or registered domestic partner s employer offers medical coverage and you choose to enroll him or her in your USC plan, you ll pay a surcharge of $50 per month. This charge is in addition to the normal paycheck contribution for the medical plan you choose. You must indicate during open enrollment whether your spouse or partner has employer-sponsored coverage available. * Because the Anthem MyChoice HMO is offered at no cost to you, incentives and surcharges are not applicable if you enroll in that plan. Other benefits There are no changes to these other USC benefits.
