These benefits give you the opportunity to support your health care and personal needs. Health Savings Account The Health Savings Account (HSA) IRS contribution limit for an individual is increasing to $3,450 (increase of $50 from 2017). How do I log on? Go to https://portal.adp.com If you are a first-time user, select SIGN UP in the portal. You will use this Registration Code: nhacademy-1234 You will be asked to provide a personal registration code.the ADP portal will walk you throught this process. To complete your enrollment, click the confirm elections button after you have selected your benefits. You will need to electronically approve/ sign to complete the enrollment process. How do I enroll or make changes to my benefits? 1. Select Benefits/Health and Welfare. 2. Select the event that allows you to enroll (Annual Enrollment, New Hire, etc). 3. The changes to your medical options are a step toward that goal. This year we will offer two medical plan options to fit your personal health care needs. Traditional PPO, otherwise known as a Copay Plan HSA-qualified High Deductible Health Plan Both plans include comprehensive health care benefits and 100% coverage for preventitive care services. They will be offered through our new partnership with Medica, an affiliate of the national United Healthcare network. This network provides coverage to ALL employees across the US. HSA What is a Health Savings Account? A Health Savings Account (HSA) is an account that can be set up to pay for current and future medical expenses. In order to participate in the HSA, you need to elect the High Deductible Health Plan (HDHP). What are the benefits of a Health Savings Account? You can make pre-tax contributions through payroll deduction up to an annual limit: $3,450 for single coverage (additional $1,000 catch up if over age 55) $6,900 for family coverage (additional $1,000 catch up if over age 55) You own your HSA, so your balance can be carried over year after year, even if you terminate employment with New Horizon Companies. This is unlike the use it or lose it rule with Flexible Spending Accounts. You can use the money in the HSA to pay for qualified medical expenses without paying any tax on the withdrawal of these funds. Review eligible expenses at SpendingAccounts.info Does the Company Contribute to my HSA? Yes, We will deposit up to $300 into your HSA account to help you pay for future eligible medical expenses. The annual contribution amount is pro-rated based on effective date. You pay expenses out-of-pocket including prescription drugs. In-Network preventative care is 100% covered. Medica s pharmacy program will be administered by CVS Caremark. Caremark will maintain Medica s drug list, process claims and help to better coordinate your care. Here is what you can expect: 90-Day Refill Option: For your convenience, you can get a 90-day supply of a drug that you take regularly. 24/7 Access to Digital Tools & Resources: The free CVS Caremark app, by Caremark, L.L.C, can be downloaded from the Apple App Store or on Google Play. Dental Plan Dental benefits provide coverage necessary to help you maintain good dental health. The company offers comprehensive dental coverage for preventative care, along with other services. Flexible Spending Accounts Two Flexible Spending Accounts available: Health Care and Dependent Care There are many benefits available to you by participating in a Flexible Spending Account (FSA) program. By taking advantage of Flexible Spending Accounts, you can: Increase your take-home pay Reduce your tax liability Medical FSA Provider WageWorks 800-678-6684 MySpendingAccount.WageWorks.com Summary of Coverage What is a Flexible Spending Account? A way to save money on your out-of-pocket medical and/or dependent care expenses. How is it funded? You select your annual election, which will be split by the number of pay periods remaining in the year and withheld from your paycheck on a pretax basis. Your annual election will be available for reimbursement on your effective date of eligibility. Can the account be funded with pre-tax salary deduction? Yes, payroll deductions that are pre-tax allow you to save between 20-30% on every dollar. Can unused amounts carry over? No, unfortunately the IRS does not allow it. Please plan your annual election accordingly. What expenses are considered eligible for medical FSA? A few examples include deductibles, prescription drug costs, and other costs not covered by your medical, dental, or vision insurance. See spendingaccounts.info for a detailed list of eligible expenses. Any restrictions with a flexible spending plan? If you are enrolled in a HSA medical plan, you can only contribute to your own Health Savings Account, not Medical Flexible Spending. This communication is not intended as legal or tax advice. Keep in Mind: Because these accounts are tax advantaged, there are many rules to keep in mind. Make sure you understand the rules and restrictions before you enroll. Plan carefully the IRS requires that you forfeit any money remaining in your Health Care and/or Dependent Care FSA at year-end. (This is known as the use-it or lose-it rule.) If you choose to enroll in the Dependent Care FSA, you may not claim the Dependent Care Credit on your taxes. Most people will save more money by using the Dependent Care Flexible Spending Account. Other rules may apply. Additional Information: Details on Flexible Spending Accounts can be found on your employee intranet site. See Benefits/Flex Spending Medical or Dependent Care. Remember Each year you will need to determine your annual contribution. Once a plan election is made, it cannot be changed until the next open enrollment period, unless there is a life qualifying event. You have 30 days from the life qualifying event to make a change accordingly. Vision The voluntary vision plan provides great coverage towards hardware, such as frames, lenses, and contact lenses. Plus, you have the opportunity to get your annual eye exam for $10, if you are not covered under our medical plan. Our medical plan covers your eye exam at 100%. Term Life Insurance Term life insurance can provide additional protection to your family if something were to happen to you. Enroll through the ADP portal. Individual Short Term Disability Insurance* Individual short-term disability insurance can help replace a portion of your monthly income if you are unable to work due to a covered injury, illness, or birth of a child. Accident Insurance* Accident insurance can pay you money based on the injury and the treatment you receive, whether it s a simple sprain or something more serious, like an injury from a car accident. The money is paid directly to you and you decide how to spend it. You can also purchase coverage for your spouse and dependent children. * To enroll in Unum Short Term Disability and/or Accident Insurance, log on to the ADP portal and click on the Unum link under benefits. www.portal.adp.com 13

When can I enroll? You can enroll, make changes, and cancel at any time. There is no open enrollment period. How do I enroll? You will not enroll via the ADP Benefits Portal for this benefit. You can enroll by calling 1-800-GET-MET8 or by going online to mybenefits.metlife.com. Cost of coverage varies depending on which plan(s) you purchase. Please Note: This is an after-tax payroll deduction. This discount is available to all employees, regardless of full or part time status. Tuition Reimbursement We offer tuition reimbursement for full time New Horizon Academy and Kinderberry Hill employees. Tuition reimbursement is to compensate the recipient for courses to be used towards an early education degree. See employee intranet for details. Once a plan election is made, it cannot be changed until the next open enrollment period, unless there is a life qualifying event.
