FIRST FINANCIAL BANK DEBIT CARD AGREEMENT EFFECTIVE AUGUST 21, 2017 IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT The USA PATRIOT Act has paved the way for financial institutions to help fight the funding of terrorism and money laundering activities. Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you is that when you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We will also ask to see your driver s license or other identifying documents. Your cooperation is appreciated when you open a new account or request a loan. ATM/VISA CHECK CARD TERMS AND CONDITIONS The following Terms and Conditions ( Agreement ) govern the use of your First Financial Bank ATM Card ( Card ), which also is a Visa Check Card ( Visa Check Card ) if it contains a Visa logo. You may use your Card to access any qualifying checking account ( Checking Account ) or savings account ( Savings Account ) (collectively Deposit Accounts ) you have with us. We will tell you which Deposit Accounts qualify for use with your Card, and whether your Card is a Visa Check Card, when you apply for a Card. Nothing in this Agreement shall be deemed to annul, limit, or in any way modify the terms of any other relationship (including any Deposit Account) you may now or hereafter have with us. You authorize us to obtain any information we deem necessary to process your application for a Card. Your application for a Card and any documents we send with your Card are part of and incorporated into this Agreement. The words you, your, and yours refer to the Card holder(s) individually and jointly. The words we, us, and our refer to First Financial Bank including its successors or assignees. I. DEFINITIONS In the following disclosures, these words are used often and are defined as follows: Electronic Fund Transfer or EFT means any transfer of funds that is initiated through an electronic terminal, telephone, computer, or magnetic tape for the purpose of ordering, instructing, or authorizing a financial institution to debit or PAGE 1

credit a consumer s account. The term includes, but is not limited to, point-of-sale ( POS ) transfers, automated teller machine ( ATM ) transfers, direct deposits or withdrawals of funds, transfers initiated by telephone and transfers resulting from debit card transactions, whether or not initiated through an electronic terminal. Electronic Terminal or EFT Terminal means an electronic device, other than a telephone operated by a consumer, through which a consumer may initiate an EFT. The term includes, but is not limited to, POS terminals, ATMs, and cash dispensing machines. II. ISSUANCE OF CARD AND PIN We will issue to you a Card for your use only, subject to the terms of this Agreement and the Electronic Fund Transfer Act and Regulation E, all as amended from time to time. You will be assigned a Personal Identification Number (PIN). At no time will you reveal or make available, directly or indirectly, the PIN to any other person. When you receive your Card PIN, memorize it. Do not write it on your Card. Destroy any written record of your PIN. The Card is the property of First Financial Bank and may be revoked or canceled without notice. You agree to surrender the Card upon demand. You may cancel your Card by notifying us in writing and physically surrendering your Card. III. USE OF CARD As explained below, you may use your Card to make EFT transactions at electronic terminals or (ii) debit transactions at participating merchants. The types of transactions you may make and relevant limitations also may differ depending on the type(s) of Deposit Account(s) you have with us. A. EFT Transactions You may use your Card and PIN to: 1. Withdraw cash from your individual or joint personal Checking Account or Savings Account. We may limit the daily amount of your cash withdrawals to your available account balance (including any overdraft protection product, if applicable) or $510, whichever is less; PAGE 2

2. Make deposits to your Checking Account or Savings Account; 3. Transfer funds between your Checking Account(s) and/or Savings Account(s). You are permitted to make an unlimited number of such transfers using an ATM; and 4. Pay bills directly from your Checking Account to parties which have officially agreed to accept your payments, provided you make these payments in accordance with the procedures set forth by us. You are not permitted to make bill payments from your Savings Account. Some services may not be available at all EFT terminals. B. Debit Transactions 1. Cards You may use your Card and PIN to make POS debit transactions at participating Interlink merchants. The amount of each transaction will be deducted from your Checking Account. You agree that we may hold funds representing transactions authorized until posted to your Checking Account. This means that we may reduce the amount of funds available to you by the amount of the transaction from the time the transaction is authorized, which may be earlier than the time the transaction is posted to (and the funds are deducted from) your Checking Account. We may limit the daily amount of your transactions to your available Checking Account balance (including any overdraft protection product, if applicable) or $2,500, whichever is less. 2. Visa Check Cards Only In addition, if your Card is a Visa Check Card, you may use your Visa Check Card instead of a check to purchase goods or services at any merchant who has agreed to accept the Visa Check Card. You agree not to use your Visa Check Card for any transaction that may be illegal, or any transaction in connection with Internet gambling. The amount of each purchase will be deducted from your Checking Account (and not your Savings Account). You agree that we may hold funds representing purchases authorized until posted to your PAGE 3

Checking Account. This means that we may reduce the amount of funds available to you by the amount of the purchase from the time the transaction is authorized, which may be earlier than the time the transaction is posted to (and the funds are deducted from) your Checking Account. We may limit the daily amount of your purchases to your available Checking Account balance (including any overdraft protection product, if applicable) or $2,500, whichever is less. You may not place a stop payment on any purchase transaction conducted with your Visa Check Card. If you have a problem with the quality of goods or services that you have purchased with your Visa Check Card at a Visa merchant, you may have the right to reimbursement from that merchant. Certain limitations may apply. You agree to cooperate in our investigation and to provide us with receipts and signature exemplars upon request. For more information about this feature, please contact us at 1-877-322-9530. The Visa association will convert any transaction in foreign currency into U.S. dollars using an exchange rate for the applicable central processing date that is (i) selected by the association from the range of rates available in wholesale currency markets, which rate may vary from the rate the association receives, or (ii) the government-mandated rate. We may provide your personal data to Visa U.S.A., its Members, or their respective contractors for the purpose of providing Emergency Cash and Emergency Card Replacement Services. IV. REPORTING OF UNAUTHORIZED TRANSFERS Tell us at once if you believe your Card or PIN has been lost or stolen or used without your authority. Telephoning us is the best way of keeping your possible losses down. You could lose all the money in your Deposit Account plus your maximum overdraft protection credit line, if you have one. If you tell us within two business days after you learn of the loss or theft of your Card, you can lose no more than $50 if someone used your Card without your permission. If you do not tell us within two business days after you learn of the loss or theft of your Card, and we can prove we could have stopped someone from using your Card without your permission if you had told us, you could lose as much as $500. Also, if your statement shows transfers that you did not make, including those made by card, code or other means, tell us at once. If you do not tell us within 60 days after the statement was mailed to you, you may not get back any money you lost after the 60 days if we can prove that we could have stopped someone from taking the money if you had told us in time. If a good reason (such as a long trip or a hospital stay) kept you from telling us, we will extend the time periods. However, the rules in the paragraphs above apply only to EFT transactions and debit transactions that require the use of a PIN. For debit transactions that do not require the use of a PIN or any other transaction processed over the Visa network, you will not be liable for any unauthorized transactions on your Card unless it is determined that you have been grossly negligent or have engaged in fraud. V. TELEPHONE NUMBER AND ADDRESS; BUSINESS DAYS If you believe your Card has been lost or stolen or that someone has transferred or may transfer money from your Deposit Account without your permission, telephone us, toll free at 1-877-322-9530, or write: First Financial Bank Attention: Client Services P.O. Box 18127 Fairfield, Ohio 45018 Our business days are Monday-Friday; holidays are not included. VI. DOCUMENTATION OF TRANSFERS POS Terminal Transfers: You will get an acknowledgment at the time you make any purchase at a POS terminal. ATM Transaction: You may get a receipt at the time you make any transfer to or from your Deposit Account using an EFT terminal. Periodic Statement: You will get a monthly Checking Account statement. You will get a monthly Savings Account statement for any month in which an electronic transfer has occurred. If there is no electronic activity on your Savings Account, you will receive a Savings Account statement quarterly. VII. PREAUTHORIZED TRANSFERS A. Preauthorized Credits PAGE 5

If you have arranged to have direct deposits made to your Deposit Account at least once every 60 days from the same person or company, you can call us at 1-877-322-9530 to find out whether or not the deposit has been made. B. Preauthorized Debits 1. Right to stop payment and procedure for doing so. If you have told us in advance to make regular payments out of your Deposit Account, you can stop any of these payments. Here s how: Call us at 1-877-322-9530, or write us at PO Box 18127, Fairfield, Ohio 45018, in time for us to receive your request three business days or more before the payment is scheduled to be made. If you call, we may also require you to put your request in writing and get it to us within 14 days after you call. We will charge you $37.00 or the most current fee as noted on our Special Handling/Electronic Banking Disclosure of Charges which can be found on our website www.bankatfirst.com for each stop-payment order you give. 2. Notice of varying amounts. If these regular payments may vary in amount, the person you are going to pay will tell you, 10 days before each payment, when it will be made and how much it will be. (You may choose instead to get this notice only when the payment would differ by more than a certain amount from the previous payment, or when the amount would fall outside certain limits that you set.) 3. Liability for failure to stop payment of preauthorized transfer. If you order us to stop one of these payments three business days or more before the transfer is scheduled, and we do not do so, we will be liable for your losses or damages. VIII. BANK S LIABILITY FOR FAILURE TO MAKE TRANSFERS If we do not complete a transfer to or from your Deposit Account on time or in the correct amount according to this Agreement (and, as applicable, all rules and regulations governing your Deposit Account), we will be liable for your losses or damages. However, there are some exceptions. We will not be liable, for instance: 1. If, through no fault of ours, you do not have enough money in your account to make the transfer; PAGE 6

2. If the transfer would go over the credit limit on your Overdraft Protection product(s); 3. If the EFT terminal where you are making the withdrawal does not have enough cash; 4. If the EFT terminal or system was not working properly and you knew about the breakdown when you started the transfer; 5. If the circumstances beyond our control (such as fire or flood) prevent the transfer, despite reasonable precautions that we have taken; or 6. If the transaction falls within an exception stated in this Agreement or another agreement we have with you. IX. CONFIDENTIALITY You agree that we will disclose information to third parties about your Checking Account for the purchases you make: a) where it is necessary for completing purchases; b) in order to verify the existence and condition of your Checking Account for a third party such as a credit bureau or merchant; c) in order to comply with a government agency or court order; or d) if you give us your written permission. X. ERROR RESOLUTION In case of errors or questions about your electronic transfers, telephone us at 1-877-322-9530, or write: First Financial Bank Attention: Client Services P.O. Box 18127 Fairfield, Ohio 45018 as soon as you can, if you think your statement or receipt is wrong or if you need more information about a transfer listed on the statement or receipt. We must hear from you no later than 60 days after we sent the first statement on which the problem or error appeared. 1. Tell us your name and Card account number (which is shown on the front of your Card). 2. Describe the error or the transfer you are unsure about, and explain as clearly as you can why you believe it is an error or why you need more information. 3. Tell us the dollar amount of the suspected error. PAGE 7

If you tell us orally, we may require that you send us your complaint or question in writing within 10 business days. We will determine whether an error occurred within 10 business days after we hear from you and will correct any error promptly. If we need more time, however, we may take up to 45 days to investigate your complaint or question. If we decide to do this, we will credit your account within 10 business days for the amount you think is in error, so that you will have the use of the money during the time it takes us to complete our investigation. If we ask you to put your complaint or question in writing and we do not receive it within 10 business days, we may not credit your account. For errors involving new accounts, point-of-sale, or foreign-initiated transactions, we may take up to 90 days to investigate your complaint or question. For new accounts, we may take up to 20 business days to credit your account for the amount you think is in error. (Your account is considered a new account for the first 30 days after the first deposit is made unless you already have an established account with us prior to this account being opened.) We will tell you the results within three business days after completing our investigation. If we decide that there was no error, we will send you a written explanation. You may ask for copies of the documents that we used in our investigation. XI. GOVERNING LAW The laws of the United States and the laws of Ohio govern this agreement regardless of your place of residence. This card is issued under the laws of the United States and the laws of Ohio, without regard to Ohio s conflicts of law provisions. XII. ELECTRONIC BANKING CHARGES 1. We will charge you a fee for a withdrawal at any ATM not owned or operated by us as follows: a. efree, Indexed efree, Simple and Signature Accounts One free withdrawal outside of our system per month, $2.50* per additional withdrawal b. Other Personal Checking Accounts - Four free withdrawals outside of our system per month, $2.50* per additional withdrawal c. Savings Accounts - withdrawals outside of our system, $2.50* per withdrawal 2. You may be charged a fee by another party, such as an ATM operator or network, for any transaction that you initiate from an EFT terminal that is PAGE 8

not operated by us (and you may be charged a fee for a balance inquiry even if you do not complete a fund transfer). 3. We reserve the right to amend these charges by sending notification when and as required by law. * Checking account service package features may impact ATM and POS fees. REMINDERS: (1) DO NOT WRITE YOUR PIN ON YOUR CARD OR KEEP ANY WRITTEN RECORD OF YOUR PIN ON ANY MATERIAL KEPT WITH THE CARD. (2) DO NOT AUTHORIZE ANOTHER PERSON TO USE YOUR CARD. PAGE 9
