Welcome to the UD online enrollment platform! Click on the Enroll Now button to begin enrollment or feel free to browse the tabs on the page for valuable information. You will need to have the legal names, social security numbers and birth dates for all of your dependents before you start.
To start your Open Enrollment, click on the Get started button. Before enrolling in the plans you should first review your current dependents and their information or add new dependents to your records. Please be sure all dependents have a Social Security number listed. To add dependents to your benefits, click on Add Dependent.
Enter all required information for your dependent. Anything that is required will be marked with an *. Continue to add dependents until all are loaded. Social Security number is a required field going forward, as a provision of the Affordable Care Act requires that employers send an annual record of healthcare costs to each employee and to the government each year. That record must include all covered member Social Security numbers. When all dependents have been added, click Save to move to the next section.
Review the information on this screen and if needed click Add Dependent to enter anyone who you missed. If all dependents are showing, click Next. Click the radio button to have your medical and dental premiums deducted either on a pretax or post tax basis. Pretax deductions will be subtracted from your pay prior to federal and state income taxes. Post tax deductions will be taken from your pay after calculation of federal and state income taxes. The 2 nd question prompts you to answer whether your spouse is eligible for their employer s or former employer s (if retired) healthcare plan. Click on the appropriate radio button. The 3 rd question asks you to identify whether you use tobacco related products. Click on the appropriate radio button for this question. These messages will display only once for you. After you have made an election, you will not
be prompted to answer the question again if you return to make changes. You can access the questions on your profile page in the Questionnaire section. Once you have answers all questions, click Save and Continue to move on. Beth will need to add wording about 2 new survey questions. To choose your Medical coverage, click on Begin enrollment.
Click the check box next to any dependents you wish to cover. You may also add dependents by clicking on the Add Dependents button. Click the Decline Coverage button if you wish to waive Medical coverage. If you wish to enroll in the healthcare plan, click on Next. If you need help choosing the right plan, click on the Get started button to use the new modeling tools available.
You can either use the National Average or click on the Customize Usage to model you own healthcare usage from previous experience. Click on the white circle and slide it to the number of your choosing in each category. You can also view your tax savings by clicking on + Add Contribution.
If you click to view tax savings, you will see this screen. Enter your filing status and household income, and the appropriate tax bracket will populate for you. You can also add a contribution to the HSA or FSA. Click Done to see the comparison. Click on Compare check boxes in the plans you d like to compare and click on the Compare plan & estimate your cost box. The number of plans you ve selected will appear within the circle in the box. The 9 dot box/3 line box in the upper right corrner changes the way the plans are displayed.
A comparison will display with plan information as well as costs and employer contributions to the HSA where appropriate. Return to the medical plans by clicking on Return to Medical plan shopping.
You can review plan details by clicking by clicking Plan details, choose a plan by clicking Select plan, or decline coverage by clicking on Decline Coverage. You can also click Previous or Cancel for other options.
If you choose the CDHP, you will be shown a screen with the Health Savings Account information. It will show UD s contribution amount and you can add your contribution amount by clicking + Add Contribution. You may contribute repeating contributions by selecting the Repeating radio button and entering the amount to contribute per paycheck. To have your contributions spread ovr less than the full year, click on the Change contribution start and end date button and enter the date range that you prefer. This is a way to front load your contributions to the HSA so they are available to use earlier in the year.
Or, you can choose to contribute a one time contribution by selecting the One time radio button and entering the amount. Either Add your contribution or Clear it by choosing the appropriate button. Clicking Add contribution will bring you to a summary page with both estimated contributions for you and UD. Click Next to continue.
You will now move to the Flexible Spending Account elections. Click Select plan to enroll or Decline Coverage to waive the plan. Enter the total amount you want to contribute without any commas for the calendar year of 2017 and click Next. The system will calculate the per pay amount for you.
Click Select plan to enroll in the Dependent Care FSA or Decline Coverage. Once you have entered the amount you wish to contribute (without commas) to the Dependent Care FSA for calendar year 2017, click Next to move forward.
This screen will confirm what you have selected. Review what you see on the screen and if it is correct, click the Save button at the bottom of the page. You may also expand the different sections and make changes to dependents or edit the plan choice. You will notice that the Cart Summary box along the right side will begin to show the specific per paycheck costs for the coverages you have elected. This will continue to add up as you elect dental and/or Flexible Spending accounts.
To begin your Dental selection, click the Begin enrollment button.
Click on the dependents you wish to cover on the dental plan. To waive dental coverage, click the Decline Coverage button OR to enroll in the plan, click Select plan.
Review what you have selected and like the medical you may review persons covered or edit the plan choice. Once you have made your decision, click Save to move on.
This page displays your elections. Please review and If they are correct, check the box next to I have reviewed the information above and click the green Complete Enrollment button at the bottom of the page.
Once you have the green check and confirmation number, you have completed your enrollment process! You may click on Please review and print your Benefit Detail Report for your records for a confirmation of your elections. It may take a minute or two for the confirmation page to load.
All of your benefit elections will show on this page. Please disregard any information on plans other than those that you just enrolled in. We are still working on adding all other benefits to the platform and hope to have them available in early 2017. Stay tuned after the first of the year for more information about using this platform for ongoing changes.
