2. ROOM SETUP & CONFIGURATION A one-time setup of the room will be provided based on one of the following configurations listed below. ANNEX A TERMS AND CONDITIONS 1. The following terms and conditions shall apply to all persons or organisations ( Lessee ) wishing to rent the training room facilities of Singapore Productivity Association ( SPA ). 2. All requests for the booking of the facilities shall be made in writing to SPA. 3. All applications received are provisional until the rental charges, as determined by SPA, have been paid in full prior to the booked date. Cheque payment, made payable to Singapore Productivity Association, must reach SPA before the facilities are released to the Lessee. 4. The Lessee must notify SPA of any cancellation of a confirmed booking in writing. The following cancellation charges apply: a) A cancellation charge of 10% of the rental fees will be levied if the Lessee notifies SPA more than 7 working days in advance of the date of use. b) If SPA receives the cancellation notice less than 7 working days prior to the utilisation of the facilities, a cancellation charge at 70% of the rental fees will be levied c) If SPA receives the cancellation notice less than 3 working days prior to the utilisation of the facilities, a cancellation charge at 100% of the rental fees will be levied. 5. Upon confirmation of booking, all arrangements including set-up, attendance and registration, audio visual requirements and other logistics arrangements 3

must be confirmed no later than 5 working days prior to the utilisation of the facilities. 6. The Lessee shall vacate the facilities on or before the expiry date and time of use of the facilities. The Lessee may request for an extension of rental after the expiry time of use of the facilities, subject to the availability of the facilities and additional charges as determined by SPA. 7. The use of the facilities for rehearsals, auditions, practice and set-up shall be subject to availability and approval from SPA. Additional charges may apply as determined by SPA. 8. The Lessee shall ensure that all acts and activities to be performed in SPA s premises have been duly licensed, censored, passed as the case may be, for production by the relevant Authorities and to show documentary proof thereof to SPA when demanded. The Lessee shall also comply with all rules and regulations governing such performances. 9. No livestock of any description shall be admitted to SPA. 10. The Lessee shall indemnify and keep indemnified SPA in full from and against all losses and damages to the room/equipment and all property caused directly or indirectly by the Lessee, its employees or contracted suppliers; The Lessee shall bear the cost incurred by SPA for any repair and rectification of such damages. 11. SPA shall under no circumstances, make good or accept responsibility or liability arising in respect of any damage or theft or loss of any property, goods, 4

articles or things deposited in the rooms by the Lessee or any other person in one way or another connected to the Lessee. 12. SPA shall not be liable for any loss due to any breakdown of machinery, failure of supply of electricity, leakage of water, fire, government restriction, act of God which may cause the facilities to be temporarily closed, hiring interrupted or cancelled. 13. No electrical apparatus or fittings of any kind shall be attached to, or used in conjunction with existing electrical fittings without the permission of SPA. 14. The Lessee shall be entirely responsible for the use of his own equipment and stage properties and such like articles and shall be liable for all claims for damages for injuries suffered by persons in connection with its use. 15. The Lessee shall put up banners only on designated hooks in the respective facilities. All banners must be removed immediately at the end of the event. A charge of $50.00 will be levied upon the Lessee who fails to do so. 16. Authorised SPA personnel shall have the right at any time to attend and observe the Lessee s event. 17. Lessee shall not use SPA s name or logos in any way either directly or indirectly in its advertising, promotion or any other activities, other than for the purpose of providing the addresses of SPA, prior to written permission from SPA. 5

18. No nails, adhesives thumb-tacks and such like materials are allowed to be used on any part of the facilities, unless approved by SPA. If approved, Lessee shall be liable for any damage arising out of such use. Goods, equipment, furniture and such like articles should be properly handled and the Lessee shall be liable for any damage to the floors or other part of the facilities. 19. No smoking is not permitted anywhere within the Lifelong Learning Institute building. 20. In the event that related services are required, these will be computed separately and added to the basic rental charges. 21. SPA reserves the right to waive, add, amend, and cancel any of the terms and conditions of rental without prior notice to the Lessee. SPA reserves the right to make changes to the rates without any prior notice. 22. SPA may, in its absolute discretion, grant, refuse or withdraw its approval for use of the facilities without giving any reason. 23. All bookings are non-transferable and non-refundable. 24. Notwithstanding that the booking has been confirmed, SPA may, in its absolute discretion, cancel the booking by giving 2 weeks notice in writing. In such case, the amount if paid will be refunded in full by SPA and Lessee will have no further claims against SPA in connection with cancellation. 6

25. In addition to the above, the following terms and conditions apply for booking of SPA s facilities from 6pm to 10pm, during the weekends and public holidays: a. A 50% deposit upon confirmation on the booking, refundable upon return of the facility/facilities, subject to the following conditions: i) Lessee to bear full costs of replacement of damaged furniture: Chair S$150 each; Table S$310 each ii) iii) iv) S$50 penalty per occurrence for failure to turn off lights and switches after class S$50 penalty for loss of facility room key S$100 penalty for loss of access card.
