Open Enrollment Begins January 1, 2013 Enrollers will be on various Campuses on January 15-17, 2013 Open Enrollment Ends January 25, 2013 2013 Benefit Updates Effective February 1, 2013, there are NO plan changes to your Dental Insurance, Vision Insurance, Disability Insurance, Group Term Life Insurance, AD&D Insurance, Accident Insurance, Cancer Insurance, Heart & Stroke Insurance and Universal Life Events Insurance.

PPO Dental Plan Provider Cigna Dental PBT ISD offers a PPO dental plan with the freedom to chose any dentist. Plan offers a $50 deductible per person or $150 per family and an increasing calendar year maximum as long as you have one preventative procedure per year. Year 1 -$1,250; Year 2 - $1,350; Year 3 -$1,450 and Year 4 -$1550 calendar year maximum. Covers dependent children to age 26. Plans pay at the 90th percentile for out-of-network reimbursement. Plan pays 100% Preventative & Diagnostic care, after deductible pays 80% for Basic Restorative Care, 50% Major Restorative Care and 50% Orthodontia with a Lifetime maximum of $1,000 for dependent children to age 19.. There is a $10.00 Eye Exam Co-payment and a $25 Eyewear Co-payment, Contact Lens Fitting Co-payment $25 (standard & specialty) Vision examinations and lenses are covered in full as long as participants go in network. There is a $100 retail frame allowance and members can get up to $100 retail allowance on contact lenses in lieu of eyeglasses. Eye exams, lenses and contact lenses are covered each 12 months.. This is a supplemental policy that pays in addition to any other insurance you have. There are three options available on the cancer plan with options to add Intensive Care coverage. Some of the plan options include $50.00 per year wellness benefit, up to $5,000 first occurrence benefit rider, up to $20,000 per calendar year for radiation, chemotherapy and immunotherapy benefit, up to $200 a day hospital confinement benefit and 38 specified diseases. Disability Insurance Provider Unum Disability Insurance is designed to provide a monthly income to an individual that is disabled due to an accident or illness. There are different plan designs available with benefits becoming available for the 1st day of disability to as late as the 180th day. Benefits are payable to age 65 for injury and sickness. All employees can enroll for up to $7500 (based on current salary) without evidence of insurability. New coverage or increases in coverage are subject to pre-existing condition limitations. For further information and rates see plan brochure at Accident Insurance Provider American Public Life (APL) Accident Insurance is designed to be a supplement that pays benefits directly to you. Coverage is available for ages 18 to 64 and is portable, so it is a benefit that you can choose to keep even if you change jobs or retire. The accident insurance pays a lump sum benefit for Accidental Injury, Ambulance Benefit, Hospital Confinement and also pays an Accidental Death and Dismemberment benefit. Coverage is available in 1 to 4 units. Employees and spouses applying for coverage are subject to underwriting conditions and must complete an Evidence of Insurability form. New employees of the district can purchase up to $150,000 group term life insurance on themselves, $50,000 on their spouse and $10,000 on their children on a Guarantee Issues Basis (No Health Questions Asked) as long as the election is made within 31 days of hire date. The group term life through Hartford is portable which means that you can keep your life insurance when you leave the district. Employees can also purchase up to $500,000 not to exceed 10 times annual salary of Accidental Death and Dismemberment Life Insurance separate from their Group Term Life. For further information and rates see plan brochure at Universal Life with Long Term Care Trustmark The Universal Life Events combines two very important benefits into one affordable product. With the Universal Life Events, your benefits can be paid as a Death Benefit, as a living benefit for Long Term Care, or as a combination of both. LifeEvents is unique and designed to match needs throughout your lifetime. The Long Term Care is calculated at 4% of your death benefit. Example; if you elect to purchase $100,000 in life insurance through this plan, your Long Term Care Benefit would pay 4% of your death benefit ($4,000) a month directly to you for 25 months. This coverage is available for you, your spouse, eligible children or grandchildren and you may elect coverage on your dependents without having to cover yourself. Heart and Stroke Insurance Provider Allstate Heart Attack and Stroke insurance is designed to supplement your medical coverage easing the financial impacts by covering some of the additional expenses associated with a heart attack, stroke or coronary artery disease. Coverage is offered on an individual or family basis. Children can be covered up to age 25. Employees and their spouses can apply for this coverage between the ages of 18 and 64. This coverage requires an application and does have pre-existing condition limitations. For further information and rates see plan brochure at Monthly Rates for 1/2 Unit of Coverage - Individual $8.98 and Family $17.32 Monthly Rates for 1 Unit of Coverage - Individual $17.96 and Family $34.64 Medical Reimbursement Flexible Spending Provider NBS Flex Medical Reimbursement allows you to tax-shelter money you set aside to pay for future health care, dental, vision and pharmacy out of pocket expenses. Eligible expenses must be incurred within the plan year (2/1/13-1/31/14). The maximum amount you can shelter in the Medical Reimbursement Plan is $2,400 a year. Please allow up to 14 days following February 1, 2013 for your funds to be available. Dependent Care Reimbursement Flexible Spending Provider NBS Flex Dependent Care Reimbursement allows you to set aside dollars to pay for child care expenses on a pre-tax basis. The Maximum Dependent Child Care is $5,000 a year (married, filing jointly) or $2,400 a year (filing single). For further information and rates see plan brochure at 2013-14 PBT ISD Cafeteria Plan Your school district has adopted a cafeteria plan which allows you to deduct certain premium amounts for benefits from your gross earning before federal withholding taxes are figured. It is a way for you to pay for certain benefits while lowering your taxable income. Changes to benefits: Mid-Year changes in benefit elections can occur only if your experience a family status change (i.e. Marriage, Divorce, Birth, Adoption, Death, Termination or change in employee or spouse employment, change in eligibility status of a dependent or loss or curtailment in heath coverage of employee or spouse due to change in spouse s employment and upon meeting a required eligibility period).
