Seniors Plus plans complement your government sponsored benefits with extra coverage to help you maintain your health and manage costs. Seniors Plus includes coverage for dental and vision care, preferred hospital accommodation, eye glasses, chiropractor and podiatrist, and also includes an accidental death benefit. Seniors Plus Plan A $500 per participant each benefit year. Chiropractor: Up to $20 per visit to a maximum of $150 per participant each benefit year for services provided by a chiropractor. Chiropodist/Podiatrist: Up to $12.50 per visit to a maximum of $125 per participant each benefit year for services provided by a chiropodist or podiatrist in excess of government program maximums. Blood Testing Monitor: Up to a maximum of $150 once in any five year period. 65% basic dental for check-ups, cleanings, fillings, extractions and root canals to a maximum of $500 per participant each benefit year. 70% coverage up to a maximum of $150 per participant $10,000 in the event of an accidental death of a underwritten by the Blue Cross Life Insurance Company of Canada. Seniors Plus Plan B $1,000 per participant each benefit year. Chiropractor: Up to $20 per visit to a maximum of $250 per participant each benefit year for services provided by a chiropractor. Chiropodist/Podiatrist: Up to $12.50 per visit to a maximum of $250 per participant each benefit year for services provided by a chiropodist or podiatrist in excess of government program maximums. Travel Coverage Discount All Seniors Plus participants are eligible for a 10 per cent discount on Alberta Blue Cross out-of-province emergency medical travel coverage. It s easy to apply individual health 1. Select the plan that suits your needs. 2. Refer to the Rate Chart that accompanies this brochure. 3. Complete all parts of the application form. Be sure to sign and date the application. Incomplete information will cause delays. 4. Forward your fully completed application by e-mail, fax or mail. When your application is accepted, coverage will begin the first day of the month following the acceptance of your application. Dental coverage waiting periods apply.
