HEALTH Item 1.a. Prescription Drugs List Enclosed is a form for listing prescription drugs. Please fill out one copy for each family member. Make sure to include both prescription drugs AND over the counter medicines. If a family member does not use any prescription drugs or over the counter medicines, please indicate on the form. Make sure to include all necessary information and provide details. The prescription drugs list is important because drugs may have adverse reactions to one another and may result in serious health complications. Item 1.b. Living Will Include a copy of your Living Will. Item 1.c. Medical Power of Attorney Include a copy of your Healthcare Proxy and Medical Power of Attorney. Item 1.d. Health Insurance Include a copy of your health insurance policy and copies of your insurance cards (front & back). You may also store claim information in this section. Item 1.e. Emergency Contacts Enclosed is a form for listing emergency contacts. Please list at least three important contacts. Make sure that you list at least one contact that you normally do not travel with.

INVESTMENTS Item 2.a. Financial Plan Include a copy of your investment summary or financial plan. Your financial plan may display most of your investments along with the institutions that they are housed with. This will help create a roadmap for a beneficiary or executor of your estate. Item 2.b. Checking & Savings Enclosed is a form for listing your open checking and/or savings accounts. Please designate the financial institution, owner(s), account type, and account number. Item 2.c. Personal Investments Enclosed is a form for listing your personal investments. Please designate the financial institution, owner(s), type of investment, and account number. You may also print a consolidated report of your investments held with Wealth Guidance, Inc. The consolidated report can be obtained by utilizing your electronic login through Wealth Guidance, Inc. website, www.wealthguidanceinc.com. Item 2.d. Retirement Investments Enclosed is a form for listing your retirement investments. Please designate the financial institution, owner(s), type of investment, and account number. You may also print a consolidated report of your investments offered by Wealth Guidance, Inc. The consolidated report can be obtained by utilizing your electronic login through Wealth Guidance, Inc. website, www.wealthguidanceinc.com. Item 2.e. Annuities Enclosed is a form for listing your retirement investments. Please designate the financial institution, owner(s), type of investment, account number, and whether or not a healthcare rider has been added. You may also print a consolidated report of your investments offered by Wealth Guidance, Inc. The consolidated report can be obtained by utilizing your electronic login through Wealth Guidance, Inc. website, www.wealthguidanceinc.com.

INSURANCE Item 3.a. Life Insurance Include life insurance policy(ies) in this section. Item 3.b. Disability Insurance Include disability insurance policy(ies) in this section. Item 3.c. Long Term Care Insurance Include long term care insurance policy(ies) in this section. Item 3.d. Homeowners Insurance Include homeowners insurance policy(ies) in this section. Make sure to include photos of your home and personal property. Photos will help you and your insurance adjuster determine the value of your possessions in case of a fire or loss of property. Item 3.e. Other Insurance Include any other insurance policy(ies). This may include auto insurance, error and omission insurance, umbrella insurance, renters insurance, etc. If you have rental property, you may also want to include photos of the property and any necessary possessions.

ESTATE Item 4.a. Durable Power of Attorney Include Durable Power of Attorney information and paperwork. Item 4.b. Will or Trust Include Will and/or Trust information and paperwork. Item 4.c. Business Documents Include any necessary business documents. This may include buy-sell agreements, partnerships, contract for deed, corporation information, etc.

IMPORTANT DOCUMENTS Item 5.a. Forms of Identification Include in this section original/official birth certificates for each family member (or at least copies if the original is not obtainable), Social Security cards, adoption paperwork, copies of driver s license, and passports. You may also want to include photocopies of your credit cards and debit cards. Item 5.b. Marriage License Include the original marriage license. Item 5.c. Titles to Vehicles Include titles for each vehicle (cars, motorcycles, boats, RVs, snowmobiles, four-wheelers, etc.) Item 5.d. Real Estate Deeds Include home, condo, or other real estate deed(s). You will also want to include your mortgage paperwork and/or an apartment lease agreement if applicable. Item 5.e. Tax Returns Include your last three 1040 tax returns. Copies of your tax returns are necessary for obtaining financing or refinancing. Item 5.f. Websites & Passwords Enclosed is a form for listing any security logins, passwords, and website information. This section may be especially important for information related to banking and finances. Item 5.g. Other Important Documents Include military records, contract agreements, prenup/postnup agreements, divorce decrees, death certificates, outstanding loans to family members or friends, and any other important documents.
