DOCUMENTS TO BE SUBMITTED TO FIS Documents to be submitted with this application (ensure all appendix are signed) 1. Application fee of HK$ 2800 - non refundable, non transferable 2. A copy of the previous year reports and most recent reports from current school (application from CP) Documents to be submitted upon acceptance The admission of the child will be confirmed by email to the families. The parents will then need to confirm their acceptance by return email and pay a deposit (registration fee) of HKD 25,000 which will be deducted from the first term invoice. It is not refunded if the student is not attending the school in the following school year. Non payment of tuition fee within 2 weeks of the offer e-mail will result in the cancellation of the offer and application. Before the School Start Note : 1. A copy of student s permanent HKID card OR a copy of passport with valid dependent or student visa for non- HK permanent residents. Students without any valid Visa will not be admitted to FIS. APPENDIX 1: FINANCIAL CONDITIONS Application Fees Application fees must be paid at the time of application. The payment can be made by cheque (payable to "French International School") or by bank transfer (please find our bank details below). Application fee of HKD 2800 - non refundable, non transferable Advance Registration Fees Following the receipt of the admission confirmation for a student to FIS, a registration fee of HKD 25,000 is due within 15 days. This fee reserves the place for the date of entry mentioned on the email. This amount will be deducted from the first term tuition fees. If the registration fee is not received within 15 days following the written admission confirmation, the admission offer and the application will be cancelled. In case of withdrawal, the registration fee is non refundable, non transferable. Debenture The purchase of a debenture is compulsory before the admission of the student in class. Category "A" - a company debenture at the value of HKD 250,000 Category "B" - a personal debenture at the value of HKD 90,000 The full value of the debenture is refunded when the student s withdrawal from School is confirmed and all outstanding bills have been paid in full. Payment of tuition fees Any term that has been started must be paid in full. Tuition fees are charged and must be settled in 3 installments (payment in advance for the full academic year is also accepted). The first financial period includes the months of September, October, November and December. The second financial period includes the months of January, February and March. The third financial period includes the months of April, May and June. Fees must be paid in full before the due date. In the event of late or partial payment, interest will be charged on the amount outstanding. Exception for 3rd financial period: The school fees for a student who leaves the school between the 1st April and the Spring Break will be charged pro rata for the number of calendar days if proper written notice of leaving has been given to the Headmaster before the 31st January. Arrival during the course of the school year 1st financial period from 1st September to 31st December: Arrival before the 1st November: The school fees are due in full for the corresponding financial period. Arrival after the 31st October: The school fees will be calculated pro rata for the number of calendar days of that financial period. 2nd financial period: from 1st January to 31st March Arrival before the 15th February: The school fees are due in full for the corresponding financial period. Arrival after the 14th February: The school fees will be calculated pro rata for the number of calendar days of that financial period. 3rd financial period: from 1st April to 30th June Arrival before the 15th May: The school fees are due in full for the corresponding financial period. Withdrawal notice and penalty To withdraw a student from the French stream of the, written notice must be received by e- mail by the Admissions Office ( inscriptions@lfis.edu.hk ) no less than 2 months before the start of the next financial period. The withdrawal notice deadline for departure at the end of the 1 st financial period is 31st October. The withdrawal notice deadline for departure at the end of the 2 nd financial period is 31 st January. The withdrawal notice deadline for departure at the end of the 3 rd financial period is 30th April. As the school year ends in June, a student not returning at the start of the school year in September must give notice to the school by 30 th April at the latest. Failure to give 2 months notice will result in a penalty being imposed equivalent to 2 months school fees and the School reserves the right to deduct these monies from the redemption value of the debenture. APPENDIX 2: FRENCH INTERNATIONAL SCHOOL PERSONAL INFORMATION COLLECTION STATEMENT (PICS) Please take a moment to read the following Personal Information Collection Statement (PICS). 1. Collection of personal data and the purposes of collecting and keeping personal data 1.1 From time to time it is necessary for students, their parents or guardians and staff members ( Data Subjects ) to provide information about themselves or for the ("FIS") to collect information about the Data Subjects. Such information may include photos, audios and video images, graphics, clips, works, recordings, electronic files, and other similar contents and materials, of or relating to the Data Subjects. FIS respects personal data and are committed to implementing and complying with the data protection principles and the relevant provisions of the Personal Data (Privacy) Ordinance (Cap.486 of the Laws of Hong Kong). The information collected from the students, their parents or guardians and staff members will be used for school administrative, academic, pastoral purposes and alumni activities. 1.2 Personal data of the Data Subjects will be handled by relevant FIS staff members and will be kept confidential. 1.3 Staff members' experience and/or teaching qualifications may be made available to relevant FIS stakeholders at the discretion of the FIS Headmaster or Campus Heads. For example, on the web sites of FIS, [including but not limited to social media sites] or announcements of appointments in newsletters or emails. 1.4 FIS may use personal data (such as name and contact details) of the Data Subjects to send communications that are relevant to parents and students, including but not limited to newsletters, programmes, courses, seminars, lectures, conferences, events and extra-curricular activities and clubs of FIS or selected third party service providers. 1.5 Personal data of the Data Subjects may be stored in the FIS database systems and online portals and any such personal data provided will form part of the staff member or the student's official records (as relevant) at FIS. 1.6 FIS may place staff members' and student's photos, name or school work, activities and accomplishments in FIS publications, websites (including but not limited to social media sites) used by FIS for organizational purposes. 2. Transfer of personal data FIS may disclose some of the data of the Data Subjects to third parties such as agencies (including the Hong Kong government), service providers and contractors (whether within or outside Hong Kong) appointed by FIS to undertake some of its academic, pastoral and administrative and alumni functions. 3. Consequence of not providing data The supply of data is voluntary except those specified at the time of data collection as obligatory. APPENDIX 3: FRENCH INTERNATIONAL SCHOOL PHOTO, AUDIO & VIDEO POLICY 1. Purpose FIS recognises that parents and guardians need to be aware of how FIS may use and publish photos, audios and video images, graphics, clips, works, recordings, electronic files, and other similar contents and materials, of or relating to FIS students ( Media ). This policy is intended to support the FIS community (including its staff, students, alumni, parents and guardians) by ensuring that the Media is used, published and stored appropriately, responsibly, and with permission. FIS recognises that the Media has considerable value in terms of identifying, celebrating and promoting FIS (particularly its students, and its community s) activities, initiatives and achievements. 2. Scope This policy covers any Media, in physical and/or digital form, containing or featuring FIS staff, students, alumni, parents and guardians made by, or on behalf of, FIS. FIS, and/or others, may use and publish the Media in a wide variety of circumstances and contexts, and on a wide variety of environments and platforms, including (FIS and authorised third party) [FIS] publicity materials, FIS website, press and media, social media and [FIS] printed publications (the Platforms ). 3. Guidelines FIS will routinely capture, take, edit, publish and store the Media, of or concerning the FIS community (including its staff, students, alumni, parents and guardians) for purposes contemplated in this policy. The Media may be used on the Platforms. The Media may identify students by name, grade, and/or such other identifying data. Students, parents and guardians may report concerns they may have, regarding inappropriate or intrusive photography, to the Headmaster, who will handle such concerns in a similar manner as any other child protection concern. FIS will securely store and handle the Media, and will only grant access to those persons with the consent of the parents or guardians of the FIS students as described under paragraph 4 below. FIS will retain control and ownership in all Media it acquires, takes or captures, possesses, uses or publishes (or those FIS hires, engages or employs to acquire, take or capture, possess, use or publish media). FIS will obtain the additional specific consent of parents or guardians if the Media is to be published by a third party or is outside the scope of any consent previously provided by the parents or guardians of the FIS students. 4. Consent of parents or guardians of students and Opt-Out Procedure The collection of the Media under this policy may constitute collection of personal data under the Personal Data (Privacy) Ordinance (Chapter 486 of the Laws of Hong Kong). [When a student is admitted to FIS, parents or guardians will be provided with a Personal Information Collection Statement (the PICS ) and a copy of this policy, informing them and obtaining their prior consent in relation to the collection and use of the Media by FIS on the Platforms. If parents or guardians of a FIS student have previously consented to the collection and use of the Media by FIS and would like to revoke their consent ( Opt-out ), they may at any time revise their choice of consent by contacting communication department by emailing communication@lfis.edu.hk.]. If parents or guardians of a student do not consent to or have Opted-out the collection and use of the Media by FIS, FIS will not use any Media concerning that student. Note that it will be necessary to exclude such students from any Media-related events, including group photos, audio recordings and videos taken at school or during school activities (for example during school trips, sporting activities and any school productions and events).
