Health Track Insurance Allows plan members who are retiring or leaving their jobs to convert certain group insurance benefits into individual benefits. There are three options to choose from based on the plan member s needs and budget. They can convert, without evidence of insurability, by submitting their application within 60 days following the date their group insurance plan ends. Online services: Less paper. More efficient.
