REQUEST FOR PROPOSAL TO PROVIDE COMPREHENSIVE STUDENT TRANSPORTATION SERVICES 1. Background Transportation Services Choice Foundation solicits proposals to provide comprehensive student transportation services for the 2017 2018 School Year for the following schools: Esperanza Charter School, 4407 S. Carrollton Ave., New Orleans, LA 70119 Lafayette Academy Charter School, 2727 S. Carrollton Ave., New Orleans, LA 70118 All services shall consist of but not limited to the following: Show 3 years of audited financials Show solvency Provide a computer based route management system Provide a monthly updated bus roster showing current ridership Ensure maximum capacity of each bus Coordinate all student transportation activities; Provide buses for daily pickup and drop off of students; Provide buses for daily drop off of students who participate in after school and extracurricular activities; Provide buses equipped with surveillance cameras; Provide buses for weekend pickup and drop off of students who participate in after school and extra-curricular activities; Provide buses for transport of school sport teams to and from school; Provide student transportation for all field trips; Ensure the safety of all students while riding on buses; Provide all vehicles that will be utilized to provide student transportation; Provide all fuel, maintenance and repairs for vehicles that will be utilized to provide transportation; Provide information for all services and person(s) that will participate (with credentials and experience) in student transportation services. Include how long the business has been in operation in this field. Include all information pertaining to your past services; Assist the school in its efforts to address and deter all behavioral issues on the buses; Ensure that drivers are professional and courteous at all times; Provide pricing per day per bus; Provide safe, clean vehicles; Provide proof that all vehicles comply with all local, state and federal regulations including licensure and registration requirements; Ensure that all employees are drug- screened by a Choice Foundation-approved vendor prior to employment; Ensure that all employees receive a local, state and FBI background screening by a Choice Foundation-approved vendor prior to employment; Provide proof of company s registration with the Louisiana Secretary of State; Provide proof that company is in good standing with the Louisiana Secretary of State; Provide proof of licensure with the appropriate federal, state and local licensing agencies;

Comply with all applicable Federal, State and Local laws, statutes and ordinances including, but not limited to the rules, regulations and standards of the Occupational Safety and Health Act of 1970, the Federal Contract Work Hours and Safety Standards Act, the Fair Labor Standards Act, the Uniformed Services Employment and Reemployment Rights Act, Louisiana Statutes, and all rules and regulations promulgated there under. Agree not to discriminate against any employee or applicant for employment because of race, sex, religion, color, age, disability or national origin. Name Choice Foundation, Inc. as an additional insured in commercial insurance policy; provide sufficient insurance coverage that will minimize Choice Foundation s risk exposure including: 1. Workers Compensation Employer s Liability insurance to cover bodily accidents in the amount of $500,000 per accident. 2. Comprehensive General Liability Insurance in the form of comprehensive, contractual insurance, personal injury, broad from property damage, premise operations and completed operations in an amount of not less than $1,000,000 combined unit. 3. Automobile Liability and Physical Damage insurance for an amount of not less than $1,000,000 for each bodily injury and property damage combined, single limits, and extensions of comprehensive coverage for all leased, owned and hired vehicles. 1.1 Services All services shall consist of but not limited to the following: a. Vendors are required to fill every seat on every bus prior to wait-listing any student. b. Vendors are required to have cameras on every bus. c. Vendors are required to provide updated rosters by bus by the 15 th of every month. 1.1 Purpose The purpose of this Request for Proposal (RFP) is to obtain competitive proposals from bona fide, qualified proposers who are interested in providing comprehensive student transportation services under the guidance of Choice Foundation s CFO and Business Office consisting of providing all student transport, vehicles, labor, materials, supplies, fuel, repairs and maintenance. 1.2 Goals and Objectives Choice Foundation Schools desires to receive professional and comprehensive student transportation services to ensure the safe transport of students for school-related activities. Term of Contract: The contract will be for a period of one (12) months effective July 1, 2017 to June 30, 2018. No automatic renewal clauses will be allowed nor honored. Pricing: Bid must include all labor, material, supplies (consumable and non-consumable), plant equipment and services necessary. The successful Bidder will be issued a Blanket Purchase Order covering the products and services contained in this proposal. Payment: Choice Foundation will pay Net (45) from completion of services

Contract Initiation Wait for Choice Foundation Board Contract Approval NOTE: Choice Foundation reserves the right to deviate from these dates. 1.4 Evaluation Criteria Primary factors used to decide the award will be price, qualifications, availability, and responsiveness. Other factors that may be used in the evaluation of this bid will be: (1) administrative costs incurred by Choice Foundation in association with the discharge of any subsequent award; (2) alternative payment terms; (3) Bidder s past performance. Choice Foundation reserves the right to accept or reject any proposal in its entirety or in part, and to waive minor irregularities if the proposal is otherwise valid. In the event of a price extension error, the unit price will be accepted as correct. Choice Foundation has sole discretion in determining testing and evaluation methods. The school may consider in conjunction to any award hereunder, those products, services and, prices available to them through contracts from state, federal, and local government agencies within the State of Louisiana. 1.5 Proposal Submittal All proposals shall be received by the Choice Foundation s Business Office no later than the date and time shown in the Schedule of Events. Choice Foundation is not responsible for any delays caused by the proposer s chosen means of proposal delivery. Proposer is solely responsible for the timely delivery of its proposal. Failure to meet the proposal opening date and time may result in rejection of the proposal.

Choice Foundation reserves the right to terminate the Contract with 48 hours advance written notice as a result of inferior quality of services, products, workmanship, and/or reductions / termination of funding. Choice Foundation reserves the right to immediately terminate the Contract if there are unresolved safety and liability concerns. The successful Bidder has the right to terminate the Contract with thirty (30) days advance written notice as a result of Choice Foundation not fulfilling its obligation as defined in the Contract.
