Due to merger or consolidation, a transit/routing number must be changed. Due to merger or consolidation, the transit/routing number must be changed, and account number structure is no longer valid. Customer has changed their name or the ODFI has submitted the name incorrectly. Transaction code is incorrect and is causing entry to be routed to the wrong account application (demand or savings). Account number is incorrect and transaction is being routed to the wrong type of account (demand should be savings or visa versa.) Due to merger or consolidation, the transit/routing number must be changed, and account number structure is no longer valid. The transaction should be routed to another account type (demand or savings.) Individual s ID number is incorrect. (Applies to transactions initiated by the customer which may require a PIN number for identification.) R04 Invalid account Account number structure not valid. Edit or check digit/number of account digits failed. R05 Unauthorized debit to Consumer Account using Corporate SEC Code A CCD, CTX or CBR debit entry was transmitted to a consumer account and was not authorized by the receiver. R06 ODFI requests return Originating Depository Financial Institution (ODFI) has requested Receiving Depository Financial Institution (RDFI) to return a duplicate or erroneous entry. R07 Revoked authorization Consumer who previously authorized entries has revoked authorization from the Originator. R08 Stop payment Receiver requests stop of a single entry not intending to stop future payments. Definitions as of 09/01/17. R10 Advised as unauthorized Consumer advises that the entry is not authorized R14 Representative Payee Representative Payee is deceased or unable to continue in that capacity; cease future deposits through another account. R15 Beneficiary or Account Holder The beneficiary entitled to benefits or account holder (other than Representative Payee) is deceased; cease future payments. R16 Account frozen Funds unavailable due to action by RDFI or other legal action. R17 File record edit criteria Fields edited by the RDFI cannot be processed. (The field causing the error should be identified in the Addenda Record.) R20 Non-transaction Account Policies/regulations restrict activity to account indicated. R24 Duplicate entry Entry appears to be duplication; trace number, date, dollar amount and/or other date matches another entry. 6

R31 Permissible return The RDFI has been notified by the ODFI that the ODFI agrees to accept a corporate return entry. R37 Source document presented for payment Check used for an ARC, BOC or POP entry has also been presented for payment. R38 Stop payment Stop payment has been placed on a check used for an ARC entry. R39 Improper source document The RDFI determines that the source document used for a ARC, BOC or POP entry to its receiver s account is improper. Questions?
