Welcome to your Alberta Health Services Savings Plan Start planning your financial freedom! You may spend up to half your adult life in retirement that s one long vacation. Will you be ready to take it? You re busy, and retirement planning may not be the first thing on your mind, but it s the key to achieving freedom from financial worry. You re already one step ahead because AHS has teamed up with The Manufacturers Life Insurance Company (Manulife) to provide you with a group savings program that is easy to set up, and even easier to manage. And because it s a group program you ll benefit from: Lower Investment Management Fees (IMFs). Group plans generally charge lower management fees than individual plans, which can make a big difference to your savings. Here are some examples: Simplicity. With Manulife s Avenue portfolios choosing investments and keeping them on track couldn t be easier. Your assets are automatically rebalanced quarterly to keep them aligned with your investment instructions, and are adjusted over time in line with your target retirement date. Immediate tax relief. Every time you contribute to your group plan through automatic payroll deductions, you reduce the amount of tax deducted from your paycheque. Spousal option. If your income is higher than your spouse s, you can also choose to contribute to a spousal account. You ll get a larger tax deduction now and other important benefits later, like income-splitting opportunities and the ability to maximize key deductions and reduce the effects of clawbacks from various means-tested programs. Contributions you make to this plan will reduce your available RRSP contribution room. Step 5 Provide your investment instructions Select Option 1 or Option 2 First, complete the Investor Profile questionnaire on page 3 of this brochure. Option 1 Select the portfolio that corresponds to the type of investor you are (Conservative, Moderate or Aggressive) and how many years you have until retirement. Then simply check the appropriate box on the back of the enrolment form (page 6). Step 6 Option 2 If you are more familiar with investing and are comfortable with the process and the risks involved, you may choose Option 2 and build your own investment mix by selecting funds from the list on the back of the enrolment form (page 6). Sign and date the back of the form, detach it and return it to Manulife at the address indicated on the front of the form (page 5). Please note: If you would like to transfer assets from another financial institution to your AHS Group Savings Plan and take advantage of lower investment management fees, simply complete a Transfer Authorization form (T-2033). You can find this form at manulife.ca/ahs. Indicate a percentage to show how you would like your employee contribution split between your spousal account and your employee account (for example, 50). You (the AHS employee) must sign and date this section. Provide your spouse s basic information. Your spouse should name a beneficiary. Indicate a percentage for each beneficiary if there is more than one. Your spouse must sign and date this section. Your spouse will need to provide investment instructions on page 8 Select Option 1 or Option 2 Step 8 First, your spouse should complete the Investor Profile questionnaire on page 3 of this brochure. For more information, see Step 5 of the instructions on how to complete an enrolment form for your personal account. Your spouse must sign and date this section. Please note 3 signatures are required on the Spousal enrolment form (Step 3, 6 and 8). 2

The Investor Profile Questionnaire Circle the number of points corresponding to your answer for each question. Then total up the points to determine your investor profile. If you plan to retire soon or withdraw a significant portion of your savings before retirement, you need to give special consideration to your asset allocation and your risk tolerance. Don t hesitate to consult your financial advisor or Manulife if you feel you need more guidance. 1. When it comes to investments: 1 pt I am not very familiar with retirement planning concepts. 4 pts I am familiar with some basic concepts, and am learning to apply them. 5 pts I understand the different investment risks, so I manage my portfolio accordingly. 6 pts I follow financial news and am very comfortable making my own investment decisions. 2. If I had $50,000 to invest in either a high-risk fund (e.g. potential returns ranging from -25 to 20) and/or a low-risk fund (e.g. potential stable returns of 3 to 4), I would invest this much in the high-risk fund: 2 pts Less than $5000 4 pts Between $5000 and $21,999 6 pts Between $22,000 and $49,000 8 pts Over $49,000 3. If the value of my investments dropped from $50,000 to $38,000 over six months: 0 pts I would move all remaining assets to a guaranteed fund. 4 pts I would move half my assets to a guaranteed fund. 6 pts I would move nothing and hold my investments as is. 10 pts I would move nothing and make additional contributions. 4. Step 5 Investment Instructions Before completing your investment instructions, we suggest you consult your enrolment guide, or call Standard Life at 1-800-242-1704 ext. (30)4000 to discuss your investment strategy.
