INVITED GUESTS: A/Professor Stuart Mackay Dr MacKay completed his training in Otolaryngology Head&Neck Surgery in 2006 and then undertook 12 months of training in Contemporary Airway Reconstruction techniques for OSA with Dr Sam Robinson and Professor Simon Carney in Adelaide. Since returning to Wollongong in 2008, he has established a Sleep Multidisciplinary Team which meets for regular patient reviews in the Illawarra. He has delivered national and international lectures on Surgery for OSA, including invited keynote addresses as far afield as India, published papers on new techniques and philosophies and co-authored multiple peer reviewed papers on different aspects of sleep medicine. He is currently the only surgeon on the Australasian Sleep Association Clinical and Conference committees. Dr MacKay resides in Wollongong, New South Wales, with his wife and three children. Dr. Andrew Jones Dr Andrew Jones is a Respiratory & Sleep Disorders Physician working in public and private practice in Wollongong in NSW. He undertook his speciality training at Royal Prince Alfred Hospital and Concord Hospital in Sydney. He has a specific interest in Obstructive Sleep Apnoea management, in particular, the role of a multidisciplinary team in defining best treatment outcomes and assessing the role of treatment options other than CPAP in the condition. He has a strong interest in Medical Education and is the current Director of Richard Lewis Dr Lewis was trained in Otolaryngology, Head & Neck Surgery in Perth. Following this he completed afellowship in Head and Neck Oncology, Microsurgery and OSA surgery in the USA (Indianapolis), followed by a Skull Base Fellowship in London. He is currently a Consultant Surgeon at Royal Perth hospital as well as working in private practice in Perth. His practice is predominantly in OSA management and Head and Neck Oncology surgery. He has ongoing research studies in both OSA surgery as well as Head and Neck surgery.

Terms and Conditions The application of the Exhibitor to exhibit is an offer which is accepted by the PA Research Foundation/Dept. of Otolaryngology, Head & Neck Surgery of the amount payable on making the application. Booking Conditions Acceptance of Sponsorship/Exhibition Applications will be subject to Committee approval. The Committee reserves the right to decline applications based on any association, direct or indirect, that does not uphold the objectives of the Meeting. Cancellation Policy In the case of the cancellation of sponsorship, unless all sponsorship is resold PA Research Foundation will reserve the right to retain monies received. In the case of the cancellation of a booth/s, please note that unless we are able to sell all space we reserve the right to retain all monies received. If the balance of payment on the booth/s is not received within the trading terms the allocated booth/s will be cancelled. Liability and Insurance The exhibitor and the servants, agents and contractors of the exhibitor occupy and use the exhibition space/booth allotted under this exhibition contract and enter the Sheraton, Noosa and act under or pursuant to this exhibition contract at their own risk. Implied Warranties In the event any statute implies any term condition or warranty into this agreement which cannot be lawfully excluded, such terms will apply to this agreement, save that the liability of the Organisers and each of them for breach of any such implied term will be limited, at the option of the Organisers, to any one or more of the following: the placement of goods to which the breach relates or the supply of equivalent goods the repair of such goods the payment of the cost of replacing the goods or of acquiring equivalent goods, or the payment of the cost of having the goods repaired Consequential Damage The Organisers will not be liable for any indirect or consequential damages arising out of a breach of this exhibition contract. Variations The Organiser may in the event of any circumstance which renders it necessary vary the booth or stand space allocation and the exhibitor will accept such re-allocation without any claim for a reduction in charges or otherwise. Unavoidable Occurrences In the event that the exhibition is cancelled or delayed through no fault of the Organiser, the Venue Manager or the Sponsors including but not limited to fire, flood, labour disputes, natural disasters, acts of God, civil disorders, riots, insurrections, work stoppages, slow downs or disputes, or other similar events then the exhibitor shall not be entitled to any refund or to claim for any loss or damage. Assignments and Subletting No exhibitor shall assign, sublet or share the whole or any part of the space booth allotted without the knowledge and consent of the Organiser. Hours The Organiser shall determine the hours during which the exhibition shall be conducted and as to hours of access for exhibitors including variations of such times as shall be necessary.
