ENROLLMENT HELP GUIDE This enrollment guide will assist you through some of the navigation and selections in the enrollment system. Navigation / Help Use the Continue button in the lower right corner to move through the enrollment system (see the Green arrow below). Should you need to go back to a previous page, simply click the Back button (see the Blue arrow below). Should you need help or technical assistance with the enrollment system, click the Help icon in the lower left (see the Red arrow below). Step 1: Personal Info: Family Members o To edit a Family Member: Click the Details button and that Family Member s data elements will be available to edit. See the Yellow arrow. o To add a new Family Member: Click the Add Dependent and the data elements to add a new family member will be available. See the Green arrow. o To delete a family member: Check the Delete check box to the right of the dependent to be deleted then click the Delete Dependent button. See the Red arrows. Step 2: Coverage Options (Benefit Elections) Prior to making your benefit elections, you may choose to learn more about the benefit(s). These plan details can be viewed on the left side of each page. o See the Red arrow below. Some plan details may have a link to more information. o See the Green arrow below. To make an election some benefits may require you to identify the eligible dependents you wish to be covered under the benefit. Below shows the Medical plan that requires you to identify the eligible dependent(s). o See the Purple arrow below. If you have a current election in a benefit, it will show in a separate block for your reference. o See the Orange arrow below. Making a coverage election is shown below for a benefit that you have identified eligible dependents. Each page has a Shopping Cart whereby you may check on a summary of benefits that you have elected. o See the Blue arrow below. Step 2: Beneficiary Designations Each benefit that requires a beneficiary must have one or more primary beneficiary designation(s) that totals 100%. Contingent beneficiary designation is optional (0 is entered into the box as 0%). You may add new beneficiaries that are not currently listed by clicking the Add NEW button. You may edit the demographics for beneficiaries by clicking the first or last name. o NOTE: If the beneficiary that you are editing is also an eligible dependent, you will be notified that dependent information must be edited on the dependent page and prompted to either be sent back to the dependent page to edit the record or to not be sent back and stay on the beneficiary page. Step 3: Tax Savings (Health Care FSA, Dependent Care FSA) You may use these pages in (2) ways: o Enter an annual amount into the Annual Amount field directly. See the Red arrow. o Enter a Per Deduction Amount directly. See the Green arrow. o Use the top section to enter amounts for various health care costs (and it will auto set your Annual and Per Deduction Amount). Step 4: Review (Completing Your Enrollment) Step 4: Review consists of: o Rate Your Knowledge o Almost Complete Your Elections Your Dependent Document Status Your Authorization o Your Enrollment is Complete. All elections made during the enrollment process are recorded and retained. However, you must continue through the entire enrollment process for any elections and/or changes to be confirmed as a completed enrollment. Should you need to step away from your enrollment or return later to make a change, you must continue to the enrollment complete page to confirm all elections are completed. Step 4: Review: Almost Complete Step 4: Review: Almost Complete (Your Elections Section) o Note, this view is of the top half of the Review page showing Your Elections. o Here you review all of your elected benefits. o You may click the Edit Button (Pencil) to the left of the benfit to navigate back to make edits.
Step 4: Review: Almost Complete (Your Elections Section) Step 4: Review: Almost Complete (Document Requirement Status & Your Authorization Section) o Note, this view is of the bottom half of the Review page showing Document Requirement Status and Your Authorization. o Your Document Requirement Status shows your eligible dependents and their document status as well as their eligible status relative to their documentation status. o For Your Authorization, select an Authorization Question and enter your answer in the box to the right then click Save Enrollment. Step 4: Review: Your Enrollment is Complete o Here s a quick breakdown of what you will find on this last page. A link to your Benefit Confirmation Statement. You are provided a Confirmation Number each time you save your enrollment. Make sure to note it although it will also print on your confirmation statement. Your total deduction amount and your effective date are shown as well as other pieces of information about your enrollment. Step 4: Review: Your Enrollment is Complete
