Scan and Upload User Guide Aviva s new scan and upload in WriteNow creates greater efficiencies, improves the speed of submitting documents to Aviva and reduces the elapsed time of new protection applications.
Step 1: Capture the Data Complete the relevant new business protection documentation with your customer Section A - On-line Data Capture Form Section B Customer Application Booklet Any other relevant documents, e.g. Medical questionnaires, Trust forms etc. Important: Please do not upload or post Section A, this is not required by Aviva. Please only upload Section B. Protection Cover Information for Financial Broker Please note that Section A (pages 1-8) of this form is to be used for data capture with Section B (pages 9-14) for signatures and the Direct Debit mandate. You can send us Section B by scanning it and uploading it onto Aviva s WriteNow system. Alternatively you can send it by post. Please do not upload or post Section A. Section A - On-line Data Capture Form Please note carefully The data entered on this form is a critical part of your application for Protection Cover. Your Financial Broker will keep the form and will enter the data into the Aviva on-line application data capture system. We will send you a copy of the data entered by your Financial Broker on your behalf. It is your responsibility to ensure the data is correct; you should check it and if you do not agree with any of the data you should make the amendments, sign the document and immediately return it to us at our head office. We will acknowledge receipt of any amendments within 10 days. If you do not receive this acknowledgement from us within 10 days, please contact us to ensure we have received your amendments. This data, together with the Customer Application Booklet in Section B that you will sign, will be the basis of your application for Protection Cover. Material facts are details that we need to know so we can assess applications for cover and claims for benefits. Material facts could include medical history, smoking history and details of occupation, travel and pastimes. If you do not tell us about material facts when you apply for your plan or at the claims stage, this might mean that your contract could be cancelled without premiums being returned, or, that we don t pay a claim. Step 2: Key the Data Key a new protection application on WriteNow, using the data you have captured on Section A - On-line Data Capture Form or if you have the customer present you can use the interactive version on WriteNow. Once this information is keyed, click Submit Application on the Submit Screen. The information, you have provided in this application form, will be treated as confidential. Aviva Life & Pensions UK Limited will hold your personal data for the purpose of administering your policy. Such information may be disclosed in confidence to agents or service providers appointed by Aviva Life & Pensions UK Limited, regulatory bodies, other insurance companies (directly or via central register). You may request, in writing, a copy of your details held by Aviva Life & Pensions UK Limited. WriteNow will provide you with an initial underwriting decision. Click Continue to the tracking history screen to upload documents. (Please note - for cases with a status of Complete, you can put the policy live immediately. Follow the onscreen prompts to put the policy live and print policy documentation). 3
Step 3: Upload Documents Once Section B Customer Application Booklet has been signed by your customer, scan this to your computer and save it as a pdf. This document is now available to upload. On the Tracking History Screen, click on the upload icon. Click Browse to select the corresponding document that you have scanned and saved to your computer. Choose the document and click Open. 4
Click Upload Documents. Once successfully uploaded, the Received date will be updated. Repeat Step 3 to upload other required document(s). Step 4: Uploading Additional Documents You can upload further documents e.g. medical questionnaires, by clicking on Upload Additional Documents. 5
From the drop down list, choose the document type that you wish to upload and the corresponding Life 1, 2 or both. Click Browse to select the corresponding document that you have previously scanned and saved to your computer. Choose the document and click Open. Click Add another if you wish to upload more documentation or click Upload Documents 6
And don t forget you can put the Policy Live! Once your application Status is Complete you can put this live on WriteNow. Select the Accept/Put Live tab on the top blue menu. Here you have the option to choose either: Step 1: Review/Amend - review a number of summary screens and make amendments if required. Step 2: Accept/Put Live - 7
