CRC BENEFITS November News 2017 2018 OPEN ENROLLMENT Now through November 15th Medical and Dental Plans CRC partners with employees to provide comprehensive, affordable medical coverage by paying approximately 85% of the costs, with employees picking up the remaining 15% in the form of monthly contributions. For 2018, both CRC and employees will continue to share the cost of the medical plan at the same levels. Monthly contributions will increase as shown in the chart below, due to an approximate 10% increase in medical plan costs. Since our medical costs are largely driven by the actual medical services utilized by CRC families, we can all help control the future increases in our medical plan costs by taking a few simple steps (see page 5) to manage how we access medical care. Additionally, effective January 1, 2018, the annual deductibles under the High Deductible Medical Plan will increase from $1,300/$2,600 individual/family to $1,350/$2,900 individual/family, which are the minimum allowed under the law for high deductible health plans. The out-of-pocket and annual maximums, if any, are unchanged for the Core Medical Plan and the High Deductible Medical Plan. NEW PLANS FOR 2018 Effective January 1, 2018, we are excited to announce two new benefit plan offerings vision and group legal. CRC Vision Plan through Aetna Vision SM Preferred, provides two plan options and features an extensive network of vision care providers, including doctors and eyewear suppliers. MetLaw through Hyatt Legal Plans, provides unlimited comprehensive legal coverage for many common personal legal needs of a member and their family. Refer to pages 2 and 3 for more information. More than ever, vision care is not just about seeing more clearly. After downloading the app, you can change your CRC Savings Plan contributions and investments, manage your CRC Employee Stock Purchase Plan participation, and much more, anytime, anywhere. CRC SAVINGS PLAN UPDATES 2018 Annual Contribution Limits The government limits the annual before-tax and Roth amounts that you may contribute to 401(k) plans like the CRC Savings Plan. For 2018, this limit is increased to $18,500. The compensation limit is increased to $275,000. CRC s matching contributions will be made on both your before-tax and after-tax contributions. Catch-Up Contributions Employees who will be age 50 or older by December 31, 2018, are eligible to make additional before-tax and/or Roth catch-up contributions to the CRC Savings Plan during 2018 up to a $6,000 maximum. EMPLOYEE STOCK PURCHASE PLAN Remember...you must re-enroll each quarter to participate in the CRC Employee Stock Purchase Plan. The enrollment period is the first week of the month prior to the beginning of each quarter. Set your calendar reminders for December 5, 2017, March 5, 2018, June 5, 2018, and September 5, 2018 to ensure you meet the enrollment deadline. To enroll, log in to your Fidelity NetBenefits account at 401k.com. Select Enroll now under Stock Plans, or in your ESPP Contribution Balance screen. Enter your election in the field on the row specific to the next quarter s offering period, accept the terms of the Plan and then select Next to submit your enrollment. Things to know... Use your 2017 FSA balances now...to qualify for reimbursement, services must be rendered on or before December 31, 2017, and PayFlex must receive your claims no later than March 31, 2018. If you change medical plan options...changing between Aetna and an HMO will require a new prescription, so make sure you allow enough time to satisfy your new plan s requirements. Summary Annual Reports (SARs)...2016 SARs for individuals covered under the CRC benefits program are enclosed. The government requires plan sponsors to distribute these disclosures to plan participants annually. Page 4

MANAGING HEALTH CARE COSTS Some factors driving medical cost trends are an increase in the utilization of out-of-network hospitals, emergency room services for non-emergency care, and an increase in prescription drug use, specifically high cost specialty pharmaceuticals. You can do your part to help manage health care costs by: Using network providers You and CRC will pay a lower rate when you use a network provider. Before getting care, verify that a provider or hospital is in-network by going to aetna.com or calling Aetna Customer Service at 877-278-0460. Getting preventive care check-ups Regular preventive care can help you stay well and catch problems early. CRC s medical plan options provide for annual exams at no cost when you use a network provider. Saying goodbye to unnecessary ER visits Choosing the right setting for care is an easy way to save money on health care expenses. Before heading to the emergency room with an urgent but non-life-threatening illness or injury, consider other more cost-effective options like urgent care centers or retail walk-in health clinics. Many of these health care options are open 7-days a week including holidays no appointment necessary. To find an Aetna urgent care center or a walk-in clinic, go to aetna.com, click Find Care and select Find Urgent Care. For HMOs, refer to their website for a list of urgent care centers in your area. Trying generics versus brand name drugs While brand name drugs get a lot of press, generic drugs meet the same standards for effectiveness and safety, and usually cost less than their brand name equivalents. Ask your doctor if a generic can work just as well as a brand name drug for your treatment. OPEN ENROLLMENT ACTION IS REQUIRED Open Enrollment is taking place now through November 15, 2017. The 2018 Open Enrollment Guide summarizes your available choices for next year. To review your current coverage and the choices available to you in 2018, log on to MyInfo at myinfo.crc.com and select Employee Self Service > Benefits Enrollment Event. The site and the Open Enrollment booklet will guide you through the enrollment process. New Vision Plan ACTION REQUIRED. To take advantage of the companypaid Vision coverage you must enroll yourself and any eligible dependents. New MetLaw ACTION REQUIRED. You may enroll for the MetLaw group legal services during Open Enrollment. Flexible Spending Account (FSA) ACTION REQUIRED. You must enroll to have a 2018 Health Care or Dependent Care Spending Account. Health Savings Account (HSA) ACTION REQUIRED. If enrolled in the High Deductible Medical Plan, you may enroll for a 2018 HSA. Medical and Dental NO ACTION required to retain your current coverage. FEDERAL NOTICES WOMEN S HEALTH AND CANCER RIGHTS ACT CRC s medical plan options provide participants with medical and surgical benefits for mastectomies. If a participant has a mastectomy and, in consultation with the physician, elects to have subsequent breast reconstruction, the plan also covers all stages of reconstruction of the breast on which the mastectomy has been performed; surgery and reconstruction on the other breast to produce a symmetrical appearance; prostheses; and treatment of physical complications for all stages of the mastectomy, including lymphedema. MEDICARE CREDITABLE DRUG COVERAGE The medical plan provides drug coverage that is considered creditable, meaning the plan provides coverage that is expected to be as good as or better than the lowest level of drug coverage authorized under a Medicare Part D plan. If you or one of your dependents becomes eligible for Medicare by virtue of a disability, remember to email to ensure you receive the required Certificate of Creditable Coverage. MATERNITY BENEFITS CRC s medical plan options provide minimum maternity stay benefits for the mother and newborn of 48 hours following a normal delivery and 96 hours following a casarean section. The attending physician, in consultation with the mother, may decide to discharge the mother and newborn earlier. These benefits are subject to the same annual deductibles, coinsurance and/or copayment provisions that apply to any other medical condition. SUMMARIES OF BENEFITS COVERAGE As required under health care reform, a Summary of Benefits and Coverage (SBC) for each medical plan and the standardized Uniform Glossary in the government-prescribed formats are available on the MyInfo website. SPECIAL ENROLLMENT RIGHTS If you have health coverage under Medicaid, or your dependent has health coverage under the Child Health Insurance Program (CHIP), and that coverage is terminated because you are, or your dependent is, no longer eligible, you or your dependent may request coverage under CRC s medical plan within 60 days of such termination of coverage, rather than the 31 days normally required upon a loss of health coverage. In addition, under CHIP, financial assistance may be available if you meet that program s eligibility requirements. If you or your dependent becomes eligible for Medicaid or financial assistance under CHIP, you or your dependent may request coverage under CRC s health plan within 60 days, rather than 31 days, after your eligibility determination date.
