Facility Rental Application / Contract This is an application from the Renter to Green Bay Curling Club, Inc., (GBCC), for use of the described premises at 781 Mike McCarthy Way on the date & time requested & for the purpose identified. The application is not binding upon GBCC until accepted by an authorized member of GBCC. Upon acceptance and receipt of any required security deposit, the application shall become a contract between the Renter & GBCC. To reserve ice time, start by returning the application page from this packet to JJ Rhyme, rental administrator, at gbcurling@gmail.com Rental Fees 1. Learn 2 Curl (L2C): $20/person with a $200 minimum 2. Non-curling events: $15/person with a $150 minimum Fee covers up to 4 hours* at the club including set up and teardown, curling instructors, all equipment, and clubroom access including tables, chairs and kitchen counters. Additional event time is $60/hour. (*Time requirements vary by group size for the on-ice L2C activities. A good rule of thumb is 20-25 people = 1 to 1 ½ hours for Learn 2 Curl activities.) A member of GBCC will be designated as the Member-In-Charge for the rental event. The primary responsibility of the Member-In-Charge is to ensure the safety and well-being of guests. Guests are expected to respect and follow all instructions from the Member-In-Charge. Any problems or questions can be directed to the Club President or the GBCC Board of Directors. Contact information can be found at www.curlgb.com Security/Damage Deposit GBCC requires a security deposit of $75 to: 1. Guarantee use of GBCC facility on the agreed date and time. If event is cancelled, deposit will be returned if GBCC is able to rebook the time. 2. Provide a guaranty against loss, damage or breakage resulting from actions of the renter or the renter's agents, employees, or guests. Reasonable care in the use of the facility & grounds is expected. The total cost of any loss, repair, or replacement necessary as the result of the event is the responsibility of the renter. At the conclusion of the event, a curling club representative will inspect the premises to determine that it has been properly cleaned and left in the condition it was rented in (see cleaning guidelines below). If there is no loss or damage to the premises, if the facility and grounds have been cleaned, if the opening and closing time agreements have been observed, and there are no other charges, the full amount of the security/damage deposit will be applied to the rental fee or returned by mail to the renter within ten (10) days. Payment Payment in full must be received no later than the day of the rental. Security deposits and final payments may be made by cash, check, or online by credit card or echeck (please ask for the online link). Make checks payable to Green Bay Curling Club, Inc. and clearly label with the event title and date. Mail to: JJ Rhyme, GBCC Rental Administrator, 234 Innsbruck Ct., Green Bay, WI 54302. Final payments may also be given to the Member-In-Charge on your rental day. Page 2 Guest Responsibility on the Curling Ice To keep the ice clean, carry in a clean pair of shoes (preferable shoes worn indoors only). Street shoes and wheelchair wheels/tires must be thoroughly cleaned before going on the ice. In addition, wheelchair wheels/tires must be cooled down when first going on the ice. No shoes with slick soles (dress, heeled or leather soled). Rocks, brooms and sliders returned to proper places when finished. Before entering the icehouse, all guests participating in on-ice activities are required to sign a participant release form (provided at the end of this document). Children under the age of eighteen must have the Minor Participant Release signed by a parent/legal guardian. GUEST SAFETY IS OF PARAMOUNT IMPORTANCE Absolutely No Horseplay on the Ice Dress warmly! Learn 2 Curl takes 1 1 ½ hours in the 40 o icehouse. Recommended apparel: Athletic-type shoes with warm socks Flexible pants. No low riders (see picture) Shirts with long tails (see picture) Layers - sweatshirts, coats or jackets Thin gloves or mittens Hats

Food & Beverages GBCC does not provide any food and/or beverage service. Arrangements for food provided by the renter or by professional caterers must be approved by GBCC in advance. It is the responsibility of the renter to ensure that their food service follows the GBCC Facility Use Guidelines and Restrictions. Facility Use Guidelines and Restrictions Food and beverages are confined to the club room dining, kitchen and bar areas. NO FOOD OR BEVERAGES ON THE ICE. NO ONE UNDER THE AGE OF 21 ALLOWED BEHIND THE BAR. Alcoholic beverages may be distributed but not sold. Renter assumes responsibility for enforcement of Wisconsin state alcohol beverage laws. The Green Bay Curling Club and its accompanying grounds are rented on an "as is" basis. GBCC provides for the care and maintenance of the buildings and grounds but does not guarantee its condition on the day of rental. Renters may not undertake physical alterations to the buildings and grounds without the expressed, written consent of GBCC. NO SMOKING is allowed inside at Green Bay Curling Club. NO bird seed, rice, or confetti may be thrown inside the building. Amplified music must be pre-approved by GBCC. NO use of nails on walls, adhesive tape on wallpapered surfaces & furniture, or decorating wires on wood surfaces All rental activity and cleaning must be finished and everyone must exit the building no later than 12:00 AM. An insurance certificate is required when specifically requested by the GBCC. It is understood that GBCC members may be at the club using the clubroom and/or available ice during the rental hours unless exclusive use of the clubroom and/or ice has been made with the rental administrator. Page 3 Cleaning Guidelines All areas used are to be same-day cleaned at end of event. 1. Kitchen & clubhouse floors free of debris and swept/vacuumed/mopped if necessary. 2. All trash bagged and prepared for removal to outside containers. 3. Grounds free of all litter. Failure to clean up in a satisfactory manner will result in a cleaning charge. The amount will be determined by the GBCC Board of Directors, based on costs of cleanup.

Adult Participant Release The undersigned hereby makes the following representations: (i) that the undersigned understands that the sport of curling is played on ice and requires physical fitness; (ii) that the undersigned possesses such physical fitness; and (iii) that the undersigned understands that the risks of participating in any curling activity could involve serious injury or death. In consideration of being allowed access to the Ice House (as defined below) as a participant in any curling activity in the Ice House, I, the undersigned, for myself and my estate, successors, heirs, beneficiaries, administrators, trustees, representatives, and attorneys do hereby remise, release, acquit, and forever discharge (i) Green Bay Curling Club, Inc., a Wisconsin non-profit corporation, (the Club ); (ii) the United States Curling Association, Inc. ( USCA ); (iii) the Wisconsin State Curling Association ( WSCA ); (iv) the respective successors and assigns or each of the Club, USCA, and WSCA and (v) the respective employees, officers, and directors, but only while acting in their capacity as such, of each of the Club, USCA, and WSCA (collectively, the Releasees ) from any and all actions, causes of action, claims, demands, and liabilities, both in law and equity for damages and any court costs and legal expenses and fees associated therewith in respect of physical, mental, and bodily injury occurring to me while participating in any curling activity in the Ice House prior to the Expiration Date (as defined below); provided, however, that in the event such injury was caused, in whole or in part, by the willful, intentional, reckless, or grossly negligent action or failure to take action of any Releasee, such Releasee shall not be so remised, released, acquitted, or discharged hereby; and provided, further, that nothing herein shall be deemed to limit or exclude any action, cause of action, claim, demand, liability, payment, reimbursement, other benefit, or any court costs or legal expenses and fees that I or my estate, successors, heirs, beneficiaries, administrators, trustees, representatives, or attorneys might have or seek against (a) the Club s Participant Medical Accident insurance coverage, (b) any other participant participating in any curling activity in the Ice House, or (c) against any other person or entity other than a Releasee. The Ice House shall mean the single room containing 3 sheets of ice in which the sport of curling is played in the building located at 781 Mike McCarthy Way, Green Bay, Wisconsin owned and operated by Club. The Expiration Date shall mean the date which is one (1) calendar year after the date this Release is executed below. I certify that I am at least eighteen (18) years of age and have the legal capacity to execute this Participant Release on my own behalf. I hereby revoke any and all releases of liability, waivers, and indemnifications previously executed by me in favor of any of the Releasees. Minor Participant Release The undersigned, being a parent or legal guardian of the minor curler ( Minor ) indicated below, hereby makes the following representations: (i) that the undersigned is legally responsible for the Minor and legally empowered to act for, on behalf of, and to execute this Participant Release and thereby bind the Minor; (ii) that the Minor will comply with the rules and regulations of the Releasees (as defined below); (iii) that the undersigned understands that the sport of curling is played on ice and requires physical fitness; (iv) that the Minor possesses such physical fitness; and (v) that the undersigned understands that the risks of the Minor participating in any curling activity could involve serious injury or death. In consideration of the Minor being allowed access to the Ice House (as defined below) as a participant in any curling activity in the Ice House, I, the undersigned, for the Minor and the Minor s estate, successors, heirs, beneficiaries, administrators, trustees, representatives, and attorneys do hereby remise, release, acquit, and forever discharge (i) Green Bay Curling Club, Inc., a Wisconsin non-profit corporation, (the Club ); (ii) the United States Curling Association, Inc. I hereby revoke any and all releases of liability, waivers, and indemnifications previously executed by me in favor of any of the Releasees.
