NEW HIRE BENEFIT ENROLLMENT INSTRUCTIONS (for Full Time Employees) IMPORTANT! You must complete online benefits enrollment within 30 days from FULL TIME date of hire. Failure to complete online enrollment prior to effective date of coverage will result in forfeiture of coverage. The next opportunity for benefit election enrollment will be during the next annual Open Enrollment period (November of each year). If you complete all enrollment requirements, your benefits will be effective on the 1 st day of the month following 60 days of service. 1. From the BENEFITS tab at the top of the screen, select MY DEPENDENTS from the drop down menu. Click on the Add icon and enter your spouse and/or dependent children if you plan to include them on any benefit plan(s) as a covered dependent. If you are not covering any dependents on your benefit plans then skip to step five. NOTE: You must include social security number, date of birth, and gender!
2. When entering dependent information, enter/type ADD DEPENDENT in the Reason for Change box at the bottom of the screen. 3. After entering dependent information, YOU MUST SAVE/SUBMIT before adding the next dependent. Do this by clicking on the submit icon that looks like a floppy disk.
4. After you have added a dependent, click the Continue arrow to add an additional dependent. 5. Next you must add any beneficiary(ies) which you wish to name for life insurance. From the BENEFITS tab, choose MY BENEFICIARIES from the drop down menu. Just like adding dependents, you will now add necessary beneficiary information; however, in the Reason for Change box, type/enter ADD BENEFICIARY. Save each beneficiary by clicking on the submit icon that looks like a floppy disk. After entering all beneficiaries, YOU MUST SAVE/SUBMIT by clicking on the Submit icon that looks like a floppy disk (see #7).
6. Once you have added and submitted ALL DEPENDENTS and ALL BENEFICIARIES, select NEW HIRE ENROLLMENT under the drop down menu from the BENEFITS tab at the top of the screen. 7. You will be prompted to enter your FULL TIME Hire Date in the Benefit Change Date box and select Newly Eligible in the Benefit Change Reason box. Then click the Continue arrow in the upper left corner of the screen.
8. Elect one option for each Benefit Type. Notice that as you make an election, the cost per pay period will be reflected. If you don t want to participate in a plan, please select Decline or Not Selected for that plan. IMPORTANT! If you decline Short Term Disability, Long Term Disability, Optional Life Insurance, and/or Optional Dependent Life Insurance and request to enroll later, your request will be subject to medical underwriting review and approval by the carrier and coverage may be denied. The only time coverage is guaranteed is at New Hire Enrollment. You can access the EMPLOYEE BENEFITS GUIDE from the Human Resources website for details regarding each plan.
9. After you have made your benefit elections, click the Continue/Forward arrow at the top left of the page and you ll be directed to the DEPENDENT/BENEFICIARY FORM page. You must check the Enroll box for each dependent/beneficiary that you wish to add for each Benefit Type/Plan. 10. After identifying dependents/beneficiaries for each plan, click the Continue/Forward arrow at the top left of the page and you ll then be directed to the BENEFICIARY WORKSHEET page to confirm your beneficiaries and percentages. The total of your percentages must equal 100%. When finished, click the Continue/Forward arrow again.
11. IMPORTANT! IMPORTANT! IMPORTANT! Review and confirm that your elections are correct Type NEW HIRE ENROLLMENT in the REASON FOR CHANGE box Enter your password TWICE NEW HIRE ENROLLMENT ENTER PASSWORD TWICE HERE! 12. Click the SAVE/SUBMIT icon to submit your elections for processing.
13. If you are sure that your elections are correct, select OK. If you are unsure, press Cancel. 14. You may print this page as your confirmation that you have successfully submitted your elections by clicking on the printer icon (your computer must be connected to a printer).
15. Click Home to return to the WELCOME/MESSAGES page. Double-click on the light blue message to see message details. IMPORTANT: Wait 24-48 hours to again check your SELF SERVICE MESSAGE CENTER for notification regarding your confirmation of approval, or to find out if more information is needed in order to approve your elections. This is the only notification that you will receive in the event that there is a problem with your enrollment. IF MORE INFORMATION IS NEEDED FROM YOU, AND YOU DO NOT SUPPLY THAT INFORMATION, YOUR BENEFITS WILL NOT BE APPROVED. 16. When viewing your Benefits confirmation message, use the scroll bar on the right (in the gray message box) to scroll down to see all benefits submitted.
