To do this, we provide tools and resources that help you proactively take charge of your health and wellness, save on your health care costs, and reward you for your healthy lifestyle efforts. Below is a convenient summary of the exciting new and existing tools and resources available to better connect you with our health and wellness programs and partners: At Cummins, we want to empower our employees to be healthier, more energetic, creative, and productive. Our Anthem Health Guides will either have the answer for you, or will direct you to the appropriate resource. Your Health Hub: New Anthem Health Guides Cummins is committed to providing you with best-in-class health partners to help you get and stay healthy, and keep you motivated throughout the year. But there are a lot of resources available to you and your family, and it can get confusing. Not anymore! You now have a single resource for all of your health and coverage questions with Anthem s Health Guides. The Anthem Health Guides can look at your health history and make suggestions on how to get the care you need and find ways to save you money on that care. They can also answer questions about your claims and coverage even schedule appointments for care. And if they don t have the answer, they will connect you to the community or Cummins resource that does. Can I use my HSA dollars to pay for eye glasses? I need help finding a provider in my area and scheduling an appointment. Has my spouse accumulated enough HealthMiles to earn the incentive? I just found out I have high blood pressure. What now? Great Ways to Manage Your Health Care Expenses Preventive care is always covered at 100% under both the HSA 3000 and HSA 1500. However, both plans also include an annual deductible. The annual deductible is simply the amount you must first pay in medical and prescription drug expenses (excluding preventive care) each year before the plan starts paying the majority of your costs in the form of coinsurance. To manage your expenses during this deductible phase, Cummins offers a number of resources to help. 1 2 3 4 5 YOUR HEALTH SAVINGS ACCOUNT (HSA) DOLLARS THE HEALTH TRACKS INCENTIVE ENHANCED CASTLIGHT TOOL ANTHEM NURSELINE CUMMINS HEALTH CENTERS Your account receives contributions from Cummins based on your pay and level of coverage, that you can save and spend tax free You can save $600 on your health plan contributions, as well as earn an additional $200 in HealthCash You can search to find the best value and quality when you need care Support is available 24/7 when you have health-related questions and could save you an unnecessary trip to the ER or doctor s office You will pay less for visits to the Cummins Health Centers compared to other providers in your community 1 Your Health Savings Account (HSA) Dollars Cummins Contributions Cummins makes a contribution to your HSA based on your pay and coverage level: Base salary $35,000 or less: $800 employee only/$1,600 all other coverage levels Base salary more than $35,000: $500 employee only/$1,000 all other coverage levels The full amount of the Cummins HSA contribution is made in early January, so it s all available to use early in the year. 1 Your Health Savings Account (HSA) Dollars (continued from page 3) HSA A Tax-Free Way to Save You can also fund your account through before-tax payroll deductions, which reduces the amount you pay in state and federal taxes. Your contributions are automatically taken out of your paycheck you don t have to do anything. If you are single, you can save up to $3,300 this year (including Cummins contributions); families can save up to $6,550 (again, including Cummins contributions). A Tax-Free Way to Spend Withdrawals from your HSA are tax-free when they re used for eligible health care expenses. So you re not taxed when you put money into your account, or when you take money out. The next time you need to pay for services, you know that you can pay for it with your HSA dollars tax-free! Once You Have It, It s Yours You own the funds in your HSA both your before-tax contributions and the contributions from Cummins. This means that any dollars in your account at the end of the year roll over for you to use the following year. And if you leave Cummins for any reason, you take your account dollars with you. Because of the account s portability, and because you do not forfeit funds at the end of each year, your HSA is an important savings vehicle for your long-term health care expenses. 2 The Health Tracks Incentive Cummins always wants to help you stay motivated and focused on your health. In 2014, you can again earn a $600 health plan incentive (a discount on your payroll contributions in 2015) if you earn 12,000 HealthMiles and your covered spouse/domestic partner (if applicable) earns 6,000 HealthMiles. If you earn the incentive by September 30, you will see your 2015 health plan discount when you enroll for coverage in the fall during Open Enrollment. If you earn the incentive after September 30, but before the end of the year, your health plan discount will be effective February 1, 2015. What can $600 in savings do for you? It can pay for: 1 annual gym membership 3 pairs of prescription eye glasses You can start or increase your HSA savings at any time! All you have to do is: Go to healthspan.cummins.com. Select Guides/Forms on the right side of the home page. Download the 2014 HSA Contribution Change Request Form. Here s what you need to do: Wellness Online Health Assessment Go to healthspan.cummins.com and select Online Health Assessment in the Apps section. Vital Numbers Watch for events at Cummins sites. 2,000 HealthMiles BMI At or below 30 Blood Pressure At or below 140/90 Total Cholesterol At or below 220 mg/dl Blood Glucose At or below 139 mg/dl Certify Tobacco-free (for the past 3 months) These measures recorded during your Vital Numbers screening. Steps Wear your GoZone and upload your steps regularly. 20-100 HealthMiles per day Self-enter Activity Self-enter activity in your VHM LifeZone account. 10-100 HealthMiles per day Physical Activity Remember: You and your covered spouse/domestic partner must create a Virgin Pulse account to get credit for completing these activities. If you already have an account, you can continue to use it in 2014. Coaching* Healthyroads Lifestyle Coaching Work with a Healthyroads Coach to lose weight, quit tobacco for good, manage stress, or just live healthy. Call 1-877-330-2746. per month Anthem Maternity Management (Future Moms) Work with a Future Moms nurse to have a healthy pregnancy and delivery. Call 1-866-691-8432. per month * Health coaching is available to enrollees in the Cummins health plan. Support the Cummins Every Employee, Every Community (EEEC) effort and earn when you or your spouse/domestic partner complete four hours of community service. Employees can record their hours on the EEEC website (My Cummins > My Community > Log your hours). Spouses/domestic partners can record their hours on the Life Zone page of the Virgin Pulse website. wellbeing If it is unreasonably difficult due to a medical condition for you to participate in an activity or coaching program for the reward under this program, ask your physician to complete and return the Reasonable Alternative Physician Form as directed on the form. The form is available on HealthSpan (healthspan.cummins.com) under Guides/Forms in the Health Tracks section. Virgin Pulse reserves the right to investigate conspicuous performance. If you falsify information or inappropriately use the GoZone, you are in violation of the Cummins Code of Business Conduct. BONUS Participate in Challenges Watch for monthly opportunities to participate in challenges and earn HealthMiles. Visit Your Health Station Each time you visit a Health Station at your location, your HealthMiles will be updated with: 250 HealthMiles for taking your measurements (1x per month) 200 HealthMiles for ideal/improved BMI (1x per month) 200 HealthMiles for ideal/improved blood pressure (1x per month) 5

The quality ratings ensure you receive care from providers with the best outcomes, reducing misdiagnosis and unnecessary follow-ups saving you time and money! New for 2014: You can now leave reviews for your providers, helping others make informed decisions on where to get high quality care. Castlight is partnering with Angie s List to increase provider reviews by more than 1 million. You have access to refined search features and more detailed cost information, including information on what Cummins pays and what you pay. Based on the imaging provider your doctor recommends, you may pay higher out-of-pocket costs. And if you haven t yet met your annual deductible, you ll pay 100% of those higher costs until the deductible is met! The AIM Program helps you save by finding quality imaging services that cost you less by shopping around for the best price on MRIs or CT Scans that are prescribed by your doctor. Here s how it works: Your doctor will give you a referral to an imaging provider for an MRI, MRA, CT scan, or CTA. When Anthem reviews your referral, they will check to see if the imaging provider is your area s most cost-effective, quality choice. If it isn t, a Health Outreach Specialist will call you to let you know there is a lower cost, and high quality imaging provider close by. Anthem will call you you don t have to do anything! You can choose to follow your doctor s referral, or go to one of the imaging providers Anthem suggests.

4 Anthem Nurseline It s 2:00 a.m. and your daughter has a high fever. What should you do? It s 10:00 p.m. and you have questions about a new medication. Who can you ask? Thanks to the Anthem Nurseline, taking care of yourself and your family is a whole lot easier and can save you money. You have a 24/7 resource for non-emergency health information. Call the Nurseline to talk with a registered nurse who can answer questions about your health, help you understand your prescription medications, or recommend treatment options and it costs you nothing! Call 1-866-691-8432. As you know, your HSA with Bank of New York Mellon is now administered by BenefitWallet, a Xerox Company. BenefitWallet offers you a number of education materials and videos to help you save on your health care expenses and taxes.
