Forms can be found at www.florenceaz.gov. Search special event vendor packet BUSINESS OWNER INSURANCE REQUIREMENTS Please direct all insurance questions to Melody Yahrmarkt at 520-723-6654 or myahrmarkt@aamaz.com. Business vendors must complete and sign waiver of liability on page 4. Business owner participants shall provide and maintain prior to and during participation at least the following types and minimum amounts of insurance: 1.
