PUBLIC EMPLOYEES RETIREMENT SYSTEM MONTHLY PAYROLL REPORT INSTRUCTION SHEET Please complete all forms in blue ink. Always send original forms to PERS. Please write neatly and clearly. DETAILED TRANSACTION REPORTS Ensure Social Security Numbers and Names are correct and listed in alphabetical order on the appropriate detailed transaction report. List each member's monthly gross compensation paid in the month being reported. List each member's retirement deductions withheld. Must be 4.5% of gross compensation for Tier I members first hired and first participates prior to July 1, 2015 and 6.0% of gross compensation for Tier II members first hired and first participates on or after July 1, 2015. List number of hours worked if employee is hourly. List number of days worked if employee is salaried or elected. Total the compensation column, the employee retirement deduction column and the #hours worked column on each detailed transaction report. List the number of contributing employees on each Monthly Retirement Report. (Refer to the PERS Participation Requirements Flowchart to determine participation requirements). If you do not have any TIER II employees, complete top part of report, write NA on number of employees and submit report. Line 1: Record total gross compensation from the Detailed Transaction Report for each TIER on the appropriate Monthly Retirement Report. Line 2: Record total employee contribution deductions from the Detailed Transaction Report for each TIER on the appropriate Monthly Retirement Report. Line 3: Use only for adjustments required by PERS on each Monthly Retirement Report. Do not use for rounding. Line 4: Total Employee Contributions Remitted (Line 2 + Line 3) on each Monthly Retirement Report. Line 5: Record total employer contributions remitted (must be 11.0% of total gross compensation (Line1). *Note: To check accuracy, multiply gross compensation by 11.0%. Total employer contributions remitted must be no more than 1 cent off for each contributing employee. Line 6: Use only for adjustments required by PERS. Do not use for rounding. Line 7: Total Employer Contributions Remitted (Line 5 + Line 6) on each Monthly Retirement Report. Line 8: Total Remitted (Lines 4 + Line 7). Must match amount of check(s). Complete the Certification and Signature section at the bottom of each Monthly Retirement Report. Signature stamps will not be accepted. Signatures must be originals and in blue ink. State of West Virginia PUBLIC EMPLOYEES RETIREMENT SYSTEM MONTHLY RETIREMENT REPORT TIPS & REMINDERS There are two tiers in the Public Employees Retirement System. Tier I: Employees first hired and first participaties in PERS prior to July 1, 2015 (4.5% of gross compensation) Tier II: Employees first hired and first participates in PERS on or after July 1, 2015 (6.0% of gross compensation) To ensure that members receive the correct amount of credited service, your Monthly Detailed Transaction Reports must include the number of hours worked for hourly employees, and the number of days worked for salaried and elected employees. Do not leave this section blank on your Detailed Transaction Reports. Total each column on each Detailed Transaction Report: gross compensation, employee retirement deductions and #hours worked. The Employee Contributions Withheld (Line 2) on each Monthly Retirement Report and the total employee retirement deductions from each Detailed Transaction Reports must match to-the-penny. If off by even one cent, plan rules require that an adjustment be made. Lines 3 and 6 on the Monthly Retirement Report are to be used for adjustments requested by PERS only. Do not use to correct rounding errors. In the event an adjustment is requested by PERS, you must take the credit and/or debit on the next submitted report following receipt of the invoice in the COMPASS system. All monthly payroll contribution remittances must be received by PERS by the 15 th of the following month to avoid delinquency surcharges. If the 15 th falls on a weekend or holiday, the due date will be the next business day. (Example: January s report is due on or before February 15 th ) Signature stamps are not permitted. Reports must be signed in blue ink only. Forms are available for download from our website at www.wvretirement.com. Be sure to choose the Public Employees Retirement System and then click on Forms link. You are not required to use the blank Monthly Detailed Transaction Report. You may instead, submit a print out from your payroll system so long as the data contains the same required information. (Information listed by 4.5%, then alphabetical order on one list; listed by 6.0%, then alphabetical order on second list). Contributions should be withheld and remitted only on compensation paid to a member in the month being reported. Any one-time flat or lump sum payment or any payments that are not guaranteed to be paid to members on a regularly scheduled basis are not considered compensation and, therefore, contributions must not be withheld and remitted to PERS.
