The Plan offers a managed account solution, agebased target date funds, or you can select your own investments from the NMH fund lineup (investment options offered by Fidelity and TIAA). Step 3: Complete your enrollment. Here, you will be given the opportunity to review the contribution amount you entered and your investment selections. An example of the Review Your Elections screen is provided on the following page. In the middle of the page, in section Investment Selections by Retirement Provider, Fidelity will calculate the amount to be allocated to each investment election as a percentage of the total contribution amount for each retirement plan provider (Fidelity and/or TIAA). STEP 1 Set Contributions Decide how much you d like to contribute from each paycheck. STEP 2 Choose Investments We ll help you understand your Plan s investment options. STEP 3 Complete Your Enrollment Confirm your enrollment choices and consider next steps. Begin X Cancel Enrollment 10

New Hires and First-Time Enrollees When you are finished reviewing all your elections as shown below, click Submit at the bottom of the screen. If you did not select any investments offered by TIAA, once you click Submit, your enrollment is complete. After you click Submit, you will be able to view and print an enrollment confirmation page for your records. Note: If you select investment options offered by TIAA (see blue arrow below), you must continue to TIAA s website to complete your enrollment (details on page 12). 11

New Hires and First-Time Enrollees If you did not select any investments offered by TIAA, once you clicked Submit (as shown on page 11), your enrollment is complete. If you did select investments offered by TIAA, you have two more steps to complete your enrollment. Step 1: Click the text View and print your confirmation (see blue arrow below). Step 2: Click the blue TIAA-CREF text to proceed to TIAA s website, where you will finish enrolling in your selected TIAA investment options. Using your enrollment confirmation printout from Step 1, you will find a summary of your selected TIAA investment options and their associated percentages that you need to input on TIAA s website for your enrollment to be complete. If you do not complete this final step to set up your selected investment options and percentages with TIAA, your TIAA contributions will be directed to the Plan s default fund rather than the TIAA investments you selected (as shown on page 11).
