Basic Term Life/AD & D Employees Active Full-time Employees and Retirees up to age 70: May choose a flat coverage amount of $5,000 in Basic Term Life Receive a matching amount of AD & D coverage The cost of these benefits is shared equally between employees and the State of Louisiana
Supplemental Term Life/AD & D Employees Supplemental Coverage Amounts: Choose additional life insurance coverage in an amount equal to 1.5 x your Base Annual Earnings with matching AD & D coverage Total Basic & Supplemental Life maximum benefit is $50,000 for current active employees and employees who retired after 1/1/73 Members of the Boards and Commissions of the State of Louisiana may choose either $5,000 Basic Term Life coverage or $20,000 Basic & Supplemental coverage Payroll deductions & group rates The cost of this coverage is shared equally by employees and the State of Louisiana
Basic and Basic Plus Supplemental Term Life - Employees All late entrants who are enrolling for the first time will be required to provide Evidence of Insurability satisfactory to Prudential prior to being approved for coverage New hires who enroll within 30 days of their date of hire may enroll without Evidence of Insurability
Living Benefit Option Available if you become terminally ill with a life expectancy of 6 months or less Receive 50% of the Basic Term Life and Supplemental Term Life face amount generally income tax-free, while still living, up to a maximum of $25,000 The death benefit payable will be reduced by any benefits paid under the Living Benefit Option
Living Benefit Option The acceleration of life insurance benefits offered under this certificate are intended to qualify for favorable tax treatment under the Internal Revenue Code of 1986. If the acceleration of life insurance benefits qualifies for such favorable tax treatment, the benefits will be excludable from your income and not subject to federal taxation. Tax laws relating to the acceleration of benefits are complex. You are advised to consult with a qualified tax advisor about circumstances you could receive acceleration of life insurance benefits excludable from income under federal law.
Reduction of Coverage At ages 65 to age 69, coverage will be reduced by 25% At age 70, coverage will be reduced to 50% of the coverage amount at age 64 Accidental Death & Dismemberment coverage ends at age 70, if retired
Termination of Coverage If your employment should terminate, you may receive similar coverage under the Portability provision provided you are less than age 70 May qualify for preferred rates by submitting Evidence of Insurability Standard rates apply for those that do not provide Evidence of Insurability

Basic and Basic Plus Supplemental Term Life - Dependents Children are covered from Birth to 21 years of age unless a full-time student and wholly dependent on you, in which case coverage can extend to age 24 If your dependent is confined for medical care or treatment at home or elsewhere, coverage begins when confinement ends All spouse coverage is subject to Evidence of Insurability satisfactory to Prudential, with the exception of New Hires who enroll their spouse within 30 days of their date of hire or within 30 days of date of marriage
Accidental Death & Dismemberment Pays for certain injuries and death as a result of a covered accident Covers Employee Benefits payable in addition to other life insurance Receive matching amount of AD& D coverage under Basic Life and an additional matching amount of AD & D for Supplemental Life coverage

AD&D Exclusions and Limitations Sickness Any act of war Losses resulting from suicide or attempted suicide Medical or surgical treatment of a sickness, and or certain infections Intentionally self-inflicted or attempted self inflicted injuries Certain full-time active military duty
AD&D Exclusions and Limitations Commission or attempted commission of a felony Legal intoxication or narcotics use Certain hazardous sports Travel or flight in any vehicle used for aerial navigation which includes getting in, out, on, or off any such vehicle.
How to Enroll Carefully review your Enrollment Kit Make coverage selections for Basic or Basic Plus Supplemental Term Life for yourself and your dependents Designate a beneficiary for yourself Sign & date enrollment form Detach form and submit to your Agency contact by 5/31/01
How to File a Claim Access the appropriate form via the State of Louisiana intranet site Print the form, complete, sign & date Provide all necessary accompanying documentation (death certificate, medical evidence, proof of loss, etc.) Mail to the address listed on the form
