The Allstate 2016 Canadian Soccer Association VIP Team Experience Contest (the Contest ) will begin on Monday, April 25 th, 2016 at 9:00 AM Eastern Standard Time ( Contest Starting Date ) and will end on Friday, May 20, 2016 at 5:00 PM Eastern Standard Time (the Contest Closing Date ) (collectively, the Contest Period ). This Contest is being conducted by the Allstate Insurance Company of Canada (the Sponsor ). 2. Before entering the Contest, each entrant must review and agree to be bound by the Contest Rules as set out herein. By entering the Contest, entrants accept and agree to be bound by the Contest Rules, and accept the decisions of the Sponsor in connection with this Contest as final and binding in all respects. Eligibility 3. The Contest is open to residents of Canada, who reside within 100 kilometres of Toronto who are eighteen (18) years of age or older as of the Contest Starting Date and are a member or a parent/legal guardian of a member of a provincially recognized soccer club. Employees, representatives, agents, shareholders, officers, and directors of the Sponsor, its advertising and promotional agencies, and those with whom any of the above reside are not eligible to enter this Contest. 4. By entering the Contest, the winner consents, to the extent permitted by law, to the use of his or her name, image, voice, likeness, city of residence, and/or photograph without compensation in any publicity or advertising carried out in any medium worldwide by the Sponsor and/or its advertising or promotional agencies without limitation or further notification. How to Enter 5. Enter by completing an electronic ballot (the entry ) on www.canadasoccer.com/allstate. To complete the entry, you must provide your first name, last name, email address, postal code, phone number, date of birth, club or association name, and team name. 6. Only one (1) entry is permitted per individual, regardless of the number of email addresses that individual may have associated with his or her name, or variations thereof. Page 1 of 5
Prize 7. All entries must be received by the Contest Closing Date in order to be eligible for the Contest. 8. Entries that do not conform to or satisfy any condition of these Contest Rules shall be disqualified. 9. The Sponsor is not responsible for any lost, late, delayed, incomplete, stolen, misdirected, damaged, altered, or destroyed entries. 10. All entries become the sole property of the Sponsor and will not be returned. 11. One (1) VIP Team Experience for a soccer team of no more than twenty (20) players who are aged ten (10) to sixteen (16) as of the Contest Closing Date and five (5) chaperones, who, with the exception of the selected entrant must be at least thirty (30) years old as of the Contest Closing Date, which includes: a) A backstage tour of BMO Field in Toronto, Ontario on June 4, 2016; b) Twenty- five (25) tickets to a National Women s Team game on June 4 th, 2016 at BMO Field in Toronto, Ontario (approximate value $40 per person), followed by a meet and greet with the National Women s team; c) Exclusive opportunity to attend a national team practice followed by a meet and greet with national team on May 30 th, 2016 (date subject to change). d) Twenty-five (25) Canada Soccer Association Team Canada Soccer t-shirts 12. The winner must participate in the prize as a chaperone. 13. All other chaperones must be at least thirty (30) years of age or older as of the Contest Closing Date. 14. A minimum of one (1) chaperone aged thirty (30) or older is required for up to four (4) players on the Contest Closing Date. 15. The prize must be accepted as awarded. The Sponsor reserves the right to substitute a prize of equivalent or greater value, including cash equivalent to the advertised retail value, in the event of the unavailability of the advertised prize. The prize may not be transferred, assigned, or redeemed for cash. Sponsor s rulings are final and without appeal in all matters related to the promotion and the awarding of the prize. 16. The prize has an approximate value of 9,250 (CAD). 17. All incidental costs and expenses associated with the Prize that are not specifically referred to herein, including (without limitation and as applicable) transportation to and from the game, practice, and/or tour, meals, beverages, gratuities, souvenirs, incidental Page 2 of 5
and personal expenses and items of a personal nature are not included and are the sole responsibility of the selected entrant, players, and chaperones as applicable. 18. The selected entrant, players, and chaperones must follow all directions of the Sponsor with respect to the applicable Prize or any aspect thereof. Failure to do so may result in termination of participation, or continued participation, in the Prize or any aspect thereof. Draw 19. A random drawing to award the prize will be held at approximately 2:00 p.m. Eastern Standard Time on Monday, May 23 rd, 2016 by the Sponsor. A selected entrant will be contacted by telephone within forty-eight (48) hours of the draw. If the selected entrant declines the prize or cannot be contacted by telephone within seven (7) days of the draw date his/her entry will be disqualified and another entrant will be randomly selected until such time as contact is made by telephone with the selected entrant or there are no more entries. 20. Upon notification, the selected entrant must, unaided, correctly answer a time-limited, mathematical skill-testing question. In the event that the selected entrant is unable to answer the skill-testing question correctly, the Sponsor shall disqualify said entrant and to randomly draw another eligible entry, and the Sponsor shall be fully and completely released and discharged from any liability or responsibility in this regard. 21. If the selected entrant declines the prize, is not eligible, or does not otherwise meet the requirements as set out in the Contest Rules, the Sponsor shall have the right to disqualify said entrant and randomly draw another eligible entry for purposes of awarding the prize, and the Sponsor shall be fully and completely released and discharge from any liability or responsibility in this regard. General Conditions 22. By entering the Contest, the selected entrant and all individuals whom the selected entrant chooses to participate in the prize, up to a maximum of twenty (20) players and five (5) chaperones, agree to sign and return the following to the Sponsor within ten (10) days of notification: (a) A form affirming eligibility and compliance with all Contest Rules (selected entrant only). (b) A liability release form releasing the Sponsor from all liability arising from or relating to the acceptance, receipt, possession, use, or misuse of the awarded Prize, participation in any Contest-related activity, or participation in this Contest. For any players under eighteen (18) years of age on the Contest Closing Date, a liability release form completed with the player s parent or legal guardian is required. Page 3 of 5
(c) A publicity release form permitting the Sponsor or its agents to use a player or chaperone s name, image, voice, likeness, city of residence, and/or photograph without compensation in any publicity or advertising carried out in any medium worldwide by the Sponsor and/or its advertising or promotional agencies without limitation or further compensation or notification, unless prohibited by law. For any players under eighteen (18) years of age on the Contest Closing Date, a publicity release form completed with the player s parent or legal guardian is required. 23. If a selected entrant fails to submit the above within the given time period, the Sponsor reserves the right to disqualify said entrant and randomly draw another eligible entry for the purpose of awarding the prize (time permitting), and the Sponsor shall be fully and completely released and discharged from any liability or responsibility in this regard. 24. Any players or chaperones who do not submit a liability release form and publicity release form within the given time period will not be permitted to participate in the prize. If an insufficient number of chaperones have submitted a liability release form and publicity release form, certain players may not be permitted to participate in the prize. See paragraph 14, above. 25. The Sponsor reserves the right, in its sole discretion to terminate or suspend the Contest, or modify the Contest Rules at any time for any reason or should fraud or other causes beyond the control of the Sponsor corrupt the administration, security or odds of winning. 26. Chances of winning depend on the number of eligible entries received. The Sponsor is unaware of how many eligible entries will be received. 27. The Sponsor shall not be liable for any human or technical errors that may arise or occur in connection with the Contest. The Sponsor assumes no responsibility for any error, omission, interruption, deletion, defect, delay in operation or transmission, communications line failure, theft or destruction or unauthorized access to, or alteration of the entries. 28. The Sponsor is not responsible for any problems or technical malfunction of any telephone network or lines, servers or providers or entry to be received by the Sponsor as a result of technical problems or human error. 29. The Sponsor reserves the right at its sole discretion to disqualify any individual who tampers with or attempts to tamper with the entry process. CAUTION: ANY ATTEMPT BY AN ENTRANT TO UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST MAY BE A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES FROM ANY SUCH ENTRANT TO THE FULLEST EXTENT PERMITTED BY LAW INCLUDING CRIMINAL PROSECUTION. 30. By entering this Contest, each entrant agrees that the courts of the Province of Ontario, Canada shall have jurisdiction to entertain any action or other legal proceedings based on any provision of these Contest Rules or the running of this Contest. Each entrant hereby irrevocably attorns to the exclusive jurisdiction of the courts of the Province of Ontario. 32. THE SPONSOR IS COLLECTING, HOLDING AND USING PERSONAL INFORMATION PROVIDED BY ENTRANTS FOR THE PURPOSES OF ADMINISTERING THIS CONTEST; AWARDING AND DELIVERING THE PRIZE AND ANNOUNCING THE WINNER AS SET OUT ABOVE IN ACCORDANCE WITH ALLSTATE INSURANCE COMPANY OF CANADA S PRIVACY POLICY AT www.allstate.ca/en/consumer+info/your+privacy+matters.
