Instruction to Proposers Invitation to Propose: MCR Health Services invites your firm to submit a proposal in accordance with this (RFQ). Your response to this request will be evaluated to determine your qualifications of your firm. Proposals must adhere to the format and content of this RFP. Proposals will not be evaluated unless all parts requested are submitted in a complete package. The information requested is the minimum required in order to qualify for consideration. The successful proposer(s) will enter into a construction manager/general contractor agreement with MCR Health Services. Background Information: Manatee County Rural Health Services (MCR Health Services) is a private, not for profit medical group comprised of 188 medical providers, 553 employees and 102 independent contractors with 25 health care centers, 2 mobile units and 6 administrative sites spread over Manatee, Sarasota and Desoto Counties. MCR Health Services is soliciting proposals for a construction manager/general contractor for the construction of various projects as needed throughout the company properties.
Scope of Services: The following services will be required from the successful firm: Preconstruction Services Review conceptual design and recommend alternative solutions. Provide recommendations of construction feasibility. Provide a preliminary construction schedule Provide value engineering services for projects. Provide cost estimates for construction projects General Contractor Services Provide permitting for projects. Provide guaranteed price for projects. Provide labor materials and supervision of all aspects of constructions projects. Provide all close out documentation and warranties for projects
Submission of Proposals: In order for your proposal to be considered all information requested must be submitted. Required Submittals: Firm name and address Primary contact and contact information Number of years in operation (Minimum 5 years) List of key personnel and resume s List of similar project experience List of references List any experience of working with a not for profit List Licenses List Insurances Fee Proposal Construction Management (Hourly Rate)$ General Contractor (Cost plus Markup Percentage) % General Conditions % All proposals must be submitted by August 14, 2015 before 5:00 PM Proposals must be received at 700 8 th Avenue West Suite 101, Palmetto Florida by the indicated time and date. Any proposals received after this date and time will not be considered.
Proposer Information: MCR Health Services may request additional information samples or presentations in support of your proposal. MCR Health Services may perform an interview with proposers under consideration or references submitted to clarify any information provided or to gather evidence of abilities. If selected for this procurement a certification of no conflict of interest with MCR Health Services will be required to be completed. Cancellation: MCR Health Services reserves the right to reject any and all proposals; request new proposals which meet the general specifications set forth and waive any informality. It also reserves the right to award the proposal in any manner that serves the best interest of MCR Health Services. In the event of unsatisfactory performance of any contract awarded via this request for proposal MCR Health Services reserves the right to terminate the contract upon giving 30 days written notice to the contractor. Site visits: None required for submission of proposal. Multiple Agreements : MCR Health Services may award multiple awards regarding this request for proposals. Firms selected by this request for proposal will be given tasks for
services to be performed on an as needed basis by the procurement department of MCR Heath Services. Duration of Agreement: The term of this master agreement shall be one (1) year and renewed annually for a (12) month period at the discretion of MCR Health Services. Minimum Requirements of Proposers: 1. Possession of a CGC license issued by the State of Florida 2. Has a minimum of (5) years in this business 3. Possess the insurance minimums as outlined herein Insurance Requirements: Throughout the term of this agreement the Contractor shall maintain at its expense and provide proof of coverage to MCR Health Services. Said insurance shall be secured from a company licensed to do business in the locale of the services being provided. Such insurance shall be in the amounts stated below: 1. Workers Compensation Insurance in the amounts required by Florida law. 2. General Liability insurance in the minimum amount of $1,000,000 per occurrence during the life of this contract. 3. Automobile Liability Insurance Commercial Automobile liability insurance with a minimum coverage of $1,000,000 per occurrence. Proof of insurance: contractor shall provide a certificate of insurance from a company licensed to do business in the state of Florida indicating coverage is in
place. The insurance certificate must be provided at the time the proposal is submitted. All proposers are instructed not to contact any member of the MCR Health Services Board of Directors, management or staff member other than the contact person indicated above. Any such contact in violation of this requirement will be cause for rejection of your proposal
