A Message from Beth Green October 2017 Dear Cooper Employee: Open Enrollment is the time when you can make changes to your current benefit elections for the 2018 plan year. We understand that these choices are important to you and your family. We have made every effort to offer excellent health benefits while keeping your premiums affordable. Open Enrollment will take place from Wednesday, October 25 through Monday, November 27, 2017. 2018 Program Highlights: This year is an active Open Enrollment. All current and newly hired employees must enroll online during Open Enrollment to have coverage or to drop coverage for 2018. Consistent with previous years, you must attest to being either a non-tobacco or tobacco user. Non-tobacco users enrolled in the Cooper Basic, Core or Standard PPO plans will receive a $25 discount per pay period a total cost savings up to $650 per year! Employee medical contributions will be increasing slightly, but at a rate lower than the national trend. There are no increases to Dental, Vision or Life Insurance premiums. Your Open Enrollment materials include the 2018 bi-weekly contributions for the medical, dental and vision plans. New for 2018: Anyone hired 1/1/2018 or thereafter cannot enroll a spouse in medical coverage if they are offered coverage through their employer. If you elect spousal coverage, you will be required to attest that your spouse does not have available coverage through their employer. New for 2018: AmeriHealth Administrators is introducing a new web portal. You MUST register on the web site to track claims, review benefit options or find a provider. To register, go to https://secure.healthx.com/myahatpa-cooper.aspx New for 2018: To protect you and your family, AmeriHealth Administrators will be issuing new identification cards that exclude any personally identifiable information. New for 2018: A new enhancement to the pharmacy benefit will be available to members effective January 1, 2018. The Smart90 Walgreen s Program allows 90 day maintenance fills from Walgreen s in addition to the Express Scripts mail-in option. Members will be allowed only 2 courtesy fills of long term medicines from a typical 30 day retail pharmacy. After two courtesy fills, members MUST use either Walgreen s Retail or Express Scripts mail in program for 90-day maintenance fills. The complete 2018 Benefits Highlights Booklet will be available online beginning October 25, 2017. You may access it through Employee Self-Service > My Benefits Tab > Benefit Handbook. This resource will provide you with the detailed information you need to select the plan that best meets your needs and your budget. Get Ready. Be Prepared To ensure a seamless enrollment process, please complete the following steps: 1. Confirm your access to Employee Self-Service (ESS): Log on to the ESS, either from work or home, and enter your User ID and Password. If you need a new password, please contact the Cooper IT Help desk at 1-856-968-7166.. 2. Verify your contact information: Log on to ESS and verify that your address is correct. If you need to update any information, use the My Address Change tab to make corrections. 3. Verify Your Dependents and Gather the Required Documents: If you want to continue covering or add additional dependents for 2018, review dependent eligibility rules in the 2018 Benefits Highlight Booklet. Log on to the ESS>My Benefits> Benefit Handbook. If you are adding dependents during open Enrollment, you will need to gather the following documentation: - Spouse: Marriage certificate or the 1 st page of your 1040 tax return (conceal social security numbers and financial information) - Dependent Child (ren): Birth certificate or the 1 st page of your 1040 tax return - Domestic/Civil Union Partner: Domestic/civil union license or domestic partnership certificate from NJ 4. Review your current 2017 benefits and determine if these benefit elections are right for you and your family in 2018: Log on to the ESS> My Benefits> Review the 2018 Benefits Highlights Booklet to review all benefits offered during Open Enrollment. Example: If your dependent needs braces or if you need new glasses or contact lenses in 2018, you may consider enrolling in a Flexible Spending Account to pay for the out-of-pocket cost with pre-tax dollars.
Get Set. Be Informed Your Action is Required During Open Enrollment: 1. You must attest if you are a Non-Tobacco User or a Tobacco User. 2. You MUST select a health insurance plan or waive coverage. If you do not take any action, you will automatically be defaulted as a tobacco user and will be enrolled in the Cooper Core Health Plan, Employee Only coverage. 3. You must re-enroll in a Flexible Spending Account in order to save money by using pre-tax dollars. If you had a Flexible Spending Account in 2017, you MUST select a new amount for 2018 during Open Enrollment. 4. Upon completing your enrollment, print your confirmation statement. 5. Attend a Benefits Fair; meet our plan vendors and the Human Resources Benefits Team. If you need assistance with enrolling, come visit us at an Enrollment Session. If you need assistance to enroll, come visit us at an Enrollment Session when Open Enrollment begins on Wednesday October 25, 2017.
