There will be no pre-bid meeting for this request for proposal. 3. Sealed Proposals will be received not later than 12:00pm Thursday, May 9, 2019 at which time the proposals will be opened and recorded as received. 2. Contractor shall place state and federally approved covered containers in the numbers noted above at the various locations. Should additional container be required at a later date, they will be provided at a post per yard, per container, per month as indicated on the final page of this bid document. 3. Contractor agrees to abide by the schedule noted above. In instances where this schedule cannot be met because of unusual circumstances, the contractor will advise the school district by telephone and will confirm by letter. However, this action will not relieve the contractor of their responsibility to provide such services as are necessary to completely remove any and all accumulated trash and refuse caused by failure to maintain the schedule. 4. Contractor will be held responsible for the care and maintenance of all containers under this contract and shall replace any and all containers that are, or become, unfit for storage, and for protection against vermin. 5. All dumpsters will be cleaned or replaced at the discretion of the Director of Operations to maintain a clean appearance. Dumpsters will be power washed as needed. 6. Contractor shall submit appropriate documents of insurance, workers compensation $100,000.00, liability $300,000.00 coverage pertaining to vehicles, property damage and public liabilities, personal injury, etc. and shall hold the school district blameless in any incident involving the contractor, his employees, his equipment, and vehicles. 7. Contractors bidding on the trash and refuse services to will be expected to visit each site and familiarize themselves with container locations and with access to the area. 8. Refuse picked up by the contractor under the terms of this contract will be disposed of in a state-approved land-fill. 9. Contactor shall provide with the bid, a list of all materials that shall be considered Recyclable Materials. 10. All dumpsters must be in place by July 1, 2019. 11. Contractor agrees that upon acceptance of the proposal, no adjustment to the price may be made for the full term of the contract. The contractors agree to the length of the contract. Terms may not be adjusted or auto-renewed at any time during the agreement 12. reserves the right to terminate the contract at any time because of the contractor s failure to comply with the terms of the contract, i.e., placement of approved containers, maintenance of established schedules, care and maintenance of all containers, and appropriate insurance coverage. 13. All equipment, transportation, disposal fees, and other pertinent expenses necessary for the proper execution of the contract are the responsibility of the contractor and are to be included and considered part of the bid price. 14. Operators handling dumpster service equipment shall be experienced and shall exercise prudent safety precautions in the performance of their duty when students are present. When school is in session operators are at no time allowed on the premises of any elementary school during the following hours; Student Entry 8:00 a.m. 9:00 a.m. Dismissal 3:15 p.m. 4:15 p.m. 15. The contractor and/or equipment operators shall perform services in a manner which leaves the premises of each district site, and adjacent private property in a clean and orderly condition. The contractor shall be responsible for clean-up of debris and/or oil spillage from pavement caused by his vehicle to complete satisfaction of the owner. They shall not receive monthly payment for services until the necessary cleanup is completed. 16. None of the services covered by this contract shall be sub-contracted without the prior written approval of the District. Such covenant shall not constitute a basis for privy between the school district and the sub-contractor and the contractor agrees to hold the school district harmless from any such claims initiated pursuant to any sub-contracts it enters into in performance of this contract. 17. That no member of the Board of Education, City, State, or any officer, employee or person whose salary is payable in whole or in party from the treasury of said Board of Education is directly or indirectly interested in this bid or in the supplies, materials, equipment, work services or any portion of the profits thereof to which it relates. Bidders must submit signed and Notarized Sworn and Notarized Familial Disclosure Statement, Iran Economic Sanctions Act, which is enclosed. 18. PROHIBITION OF SMOKING IN PUBLIC SCHOOL FACILITIES- In the interest of providing a safe and healthy environment for the employees, students and visitors, smoking is prohibited throughout school buildings, grounds and vehicles (includes construction workers and delivery people).
