2011-2012 Verification Tips Please read before submitting your verification documents to our office. Submit copies of 2010 tax returns. Keep originals for your records. Make sure that the Verification Worksheet is signed by you and if you are a dependent student according to the FAFSA, be sure that your parent signs it. Make sure that all sections are completed. We require Federal Tax Returns, not State. They must be signed by the filer. This means that you must sign your tax return and your parents must sign their tax return. We cannot accept a preparer s signature and we cannot accept electronic signatures. We accept the following tax forms: 2010 IRS Form 1040, 1040A, 1040EZ, a tax return from Puerto Rico or a converted foreign income tax return. We do not accept Form 8453 or other electronically filed returns that do not show all of the line items as a standard IRS tax return. If you did not keep a copy of your tax return, request a copy or transcript from the IRS or a copy from your tax preparer. The verification process can take up to 2 weeks. Keep a check on your email because we will notify you if there are problems or if any additional information is required.

2011-2012 VERIFICATION WORKSHEET Federal Student Aid Programs Your application was selected by the U.S. Department of Education for review in a process called Verification. Please submit copies of your and your parent(s) signed 2010 Federal Tax Forms if you are a dependent student. If you are an independent student, submit copies of your (and your spouse s, if you are married) signed 2010 Federal Tax Forms. Please refer to the section below for the definition of an independent student. All tax forms must be signed by the filer. If you can answer yes to any of the following questions, you are an *independent student. Documentation may be required. 1. Were you born before January 1, 1988? 2. Are you a veteran of the U.S. Armed Forces? 3. Are you currently serving on active duty in the U.S. Armed Forces for purposes other than training? 4. At the beginning of the 2011-2012 school year, will you be working on a master s or doctorate program? 5. As of today, are you married? (Answer Yes if you are separated but not divorced.) 6. At any time since you turned 13, were both your parents deceased, were you in foster care or were you a dependent or ward of the court? 7. Do you have children who receive more than half of their support from you between July 1, 2011 and June 30, 2012? 8. Do you have dependents (other than your children or spouse) who live with you and who receive more than half of their support from you, now and through June 30, 2012? 9. Are you or were you an emancipated minor as determined by a court in your state or legal residence? 10. Are you or were you in legal guardianship as determined by a court in your state of legal residence? 11. At any time after July 1, 2010, did your high school or school district homeless liaison determine that you were an unaccompanied youth who was homeless? 12. At any time on or after July 1, 2010, did the director of an emergency shelter or transitional housing program funded by the U.S. Department of Housing and Urban Development determine that you were an unaccompanied youth who was homeless? 13. At any time on or after July 1, 2010, did the director of a runaway or homeless youth basic center or transitional living program determine that you were an unaccompanied youth who was homeless or self-supporting and at risk of being homeless? What you should know: Federal regulations require that we ask you for this information before awarding financial aid. If there are differences between your application information and your financial documents, we may need to make corrections to your FAFSA. Combat pay or special combat pay. Only enter the amount that was taxable and included in your $ adjusted gross income. Do not enter untaxed combat pay.
