To begin your registration, visit https://my.adp.com. If you are a First Time User, click Register Now you will need the following registration code: SoGAMedCtr-ess If you have already registered, enter your username and password and click Sign In.
Follow the instructions to Register if you are a First Time User.
When your Registration is complete, you should click on the MyADP tab to access your Benefits Portal. It is important to write down or store your User ID for future reference because you can access this portal at any time throughout the year, and also download the ADP Mobile Solutions Mobile App. Use your User ID and password to log in to the ADP Mobile Solutions Mobile App.
When you go to the MyADP tab, this will bring you back to the login page at https://my.adp.com. Enter your User ID and password. Upon logging, in you will see the Home Page. The Current Benefits section of the home page is not currently reflective of your 2015 benefit elections, and will update on January 1, 2016 with your 2016 benefit elections following Open Enrollment. The only benefits that will be reflected in this area are the guaranteed issue benefits Supplemental Term Employee and Spouse Life, Child Life, and Basic Term Life, as well as Long Term Disability. Below is an explanation of each of the tabs in blue on the Home Page of the Benefits Portal. Current Benefits & Coverage Shows current benefits & coverage (2015 benefits will not be reflected here unless the benefit is a guaranteed issue benefit) Declare a Life Event If you get married, for example, and need to make changes to your benefits, you can declare a life event via the SGMC Benefits Portal Forms & Documents All Benefits-related forms and documents are posted in this section Additional Benefits You can access your Allstate Benefit Enrollments by clicking on the Additional Benefits tab. You can begin enrolling by click on the Change button under any of the plans.
When you are selecting a plan, ensure you check the appropriate dependents that you want to enroll on the plan under the Who is Covered? section. Select the plan you wish to enroll in. If you do not want a benefit, select I Don t Want This Benefit. If at any point, you wish to go back to the Annual Enrollment page, click Annual Enrollment.
When you are selecting plans that require an Evidence of Insurability (EOI) form, you will see the following notification. This means that the plan requires you to be approved for the coverage. To obtain the Evidence of Insurability form (EOI), visit the Home Page, Documents & Forms, and obtain the applicable EOI form for either Supplemental Term Life Insurance or Long Term Disability coverage. If EOI is required, the overview page will have this notification in yellow. Return to the Home Page, select the Forms & Documents section, and click on the applicable Cigna EOI Form. You can get this form during your enrollment, or once you are finished and have submitted your enrollment.
If you need to add a dependent or beneficiary, scroll to the bottom of the Overview Page to see Who is Covered. You can add or remove dependents or beneficiaries. Adding a new person will allow you to add a dependent or beneficiary. Adding an Organization will allow you to add an estate, etc. as a beneficiary.
Make sure you designate your beneficiary (or beneficiaries) on the Basic Life Insurance, and any Supplemental Term Life Insurance, if applicable.
When you have made all of your benefit elections, click Complete Enrollment. If you are not finished with your enrollment and you are going to come back to it, click Save & Finish Later.
Review your final benefit elections, and if you are finished, click Complete Enrollment. If you are not finished with your enrollment and you are going to come back to it, click Save & Finish Later.
Once you receive your confirmation number on this page, your enrollment is complete and has been submitted. Print your Confirmation Statement by clicking Print your Confirmation
