Applications can be obtained from OS/Establishment department, 11 th Floor, Zonal office, LIC of India, Chennai by remitting Rs 250/- by cash/dd at cash counter during cash hours. If application fee is remitted, DD for Rs 250/- should be enclosed along with Sealed quotations. The application should be returned to us duly filled and signed along with enclosures: 1. TERMS AND CONDITIONS OF THE TENDER 1. The average Annual Turnover should be above 3 Lakhs in the last three financial years. Audited Financial statements for the years 2017-18, 2016-18 & 2015-16 should be enclosed 2. The Tenderer should have an experience in the respective field for a minimum period of 3 years. 3. The tenderer should be providing similar services for the minimum of one Govt/PSU presently. Copy of work order, TDS certificates should be enclosed as a proof. 4. Income tax returns certificates of last three consecutive years as a proof for IT filing(assessment year 2018-19, 2017-18 & 2016-17) 5. Agency should have local office at Chennai with sufficient personnel to attend to work 6. The applicant must have valid License to stock and use restricted Insecticides for Commercial Pest Control Operations. Copy of license to be enclosed 7. A copy of the registration with the Regional Labour Commissioner (Central Government) for engaging contract labour as required under the Contract Labour (Regulation and Abolition) Act, 1971. If applicable, the firm will have to submit the labour license within a week of awarding the work. 8. An undertaking to the effect that if the AMC is awarded, the tenderer will comply to all the requirements conforming to various other Acts related to the employment of Contract Labour including, the Employees Provident Funds and Miscellaneous Provisions Act 1952; the Payment of Wages act, 1936; the Central Minimum Wages Act; ESI etc. in so far as the same may be applicable. 9. The contractor should deploy the skilled person or carryout the work in supervision of a skilled person, who have the technical knowledge of the work i.e. mixing proportions of pesticides chemicals, method of treatment etc. 10. The contractor shall be required to show the mixing proportions of pesticides, whenever LIC desires to check the composition of the Pesticide chemicals. 11. The contractor shall follow the safety regulations to apply permitted pesticides of Indian Standards. He shall provide necessary safety appliances, gloves, mask, shoes, safety goggle etc to his employees for carrying out the pesticides services instructed by the LIC. 12. Any person found under the influence of alcohol or any intoxicating drugs on duty is unfit for duty and should not be allowed to work.

13. General pest control means eradication of Cockroaches,Mosquitoes, Flies, Spiders, Lizards, bedbugs etc through standard products (such as BAYER etc) as permitted insecticides by Government of India and WHO norms. The Pest control should cover all the places with application of gel, spray, fumigation under the beds, tables, chairs, on and around the pile of files, on wooden furniture, on false ceiling, on all staircases, on all toilets drain ducts, in all places Office premises, Staff quarters & Guest houses and should leave no space unattended. 14. Fogging: Fogging in the entire open area whenever required to control insects 15. Rodent Control: Rodent controlling should be done with rat traps/pads as per orders and instructions on the subject.. 16. For the works that cannot be done from the ground level, necessary means should be provided by the tenderer. Drum or other make shifts must not be used in place of ladders with proper support. 17. First-Aid kits or boxes and stretchers should be readily available at all times with the contractor while carrying out the work 18. Work is to be carried out in office premises and staff quarters. 19. The contractor will be fully liable to compensate all concerned for any loss, damage or destruction of structures, persons, property etc. including third party risk arising due to causes attributable to the contractor as may be decided by the LIC whose decision in this regard shall be final. No claim shall be entertained by LIC on this account. 20. The agency should bring the required pesticides or chemicals, spray them through a spray gun or place them if they are dry cubes or carry out such chemical treatment through insecticide sprays. Only company made chemicals / Insecticides and quantity as per Government of India and WHO norms are to be used. The pesticides etc. used for pest/rodent control should not have adverse impacts on human health. 21.The Firm/Company shall take the workmen compensation policy for the workers engaged in the work on the basis of the monthly salary of the worker. The policy shall also cover the personal accidental death, permanent total disability, permanent partial disability, temporary disablement or any other accident during the execution of the work and medical extension of the worker 22. Tender fees of Rs.250 to be submitted along with application for empanelment, by way of cash at ZO or by Demand Draft in favour of LIC of India. 23. The tender should be dispatched so as to reach the office which called for the same, on or before the due date specified. 24. Please note the description and specification of stores carefully and your offer should be in accordance with the same. If you quote any stores with deviation in

specifications, the same may please be carefully brought out in your tender; otherwise, it will be deemed that you have quoted for the specified stores. 25. Any dispute arising out of or relating to this tender shall be deemed to have arisen at the headquarters of the Corporation s Office placing the order and shall be subject to adjudication by a Court in that city. 25. LIC of India is not responsible in any manner for the postal delay/loss/ non receipt of the tender 26.
