HSBC Credit Card Application Acknowledgement Form I/ We hereby agree to take up the credit card(s) issued by The Hongkong Banking and Shanghai Banking Corporation Limited, Macau Branch (the Bank ) and the following terms and conditions: Important Notes 1. All primary card applicants must be Macau Special Administrative Region residents and aged 18 or above. 2. To be eligible for the credit card, primary card applicant s annual income must meet the following requirements: Visa Gold card MOP150,000 or above; Visa Classic card MOP60,000 or above. 3. The new card provides free use of phonebanking service. A personal identification number (PIN) will be mailed separately upon card issuance. Please note that without the PIN, cardholders will not be able to access the phonebanking service. The phonebanking service can be linked with the account via credit card service hotline (853) 2832 2813. 4. The following fees and charges apply as appropriate and may change from time to time: a) Finance charge: If the Cardholder fails to pay the Bank the whole of the Statement Balance by the Payment Due, a finance charge will be applied (a) to each Card Transaction (excluding cash advance) making up the Statement Balance, as well as each new transactions (excluding cash advances) entered into by the Cardholder since the last Statement, from the transaction date and (b) to any other amounts making up the Statement Balance from the last Statement, until the Statement Balance is paid in full. The finance charge will accrue daily and be calculated at the interest rate of 2.33% per month. b) Late charge: If the minimum payment due is not received by the Bank on or before the payment due date, a late charge of 5% of the minimum payment (subject to a minimum of MOP/HKD80 and maximum of MOP/HKD200) will be levied on your card account. c) Overlimit handling fee: If the statement balance exceeds the credit limit for the time being assigned to the card amount, an overlimit handling fee of MOP/HKD130 will be debited to the card account on the statement date. d) Cash advance fee: Cash advances include all cash withdrawals made from the Card Account including withdrawals from funds credited to the Card Account. They will be subject to a handling fee of 3% on the amount of advance (a minimum of MOP/HKD70 for an over-the-counter cash advance and a minimum of MOP/HKD30 for an advance made from an ATM or through any other channel) plus a cash advance fee of 2% on the amount of advance. The charges are flat and shall be debited to the card account as at the date of the advance. If any additional services (such as card replacement, extra copies of monthly statement, etc.) is required, other fees and charges may apply. For details and up-to-date charges, please refer to Bank Tariff Guide for HSBC Retail Banking and Wealth Management Customers available on the Bank s website www.hsbc.com.mo and at any HSBC branch in Macau. 5. The Bank may, at our sole discretion, decide whether to approve my/our application, and whether to issue a Visa Gold or Classic card. If my/our application is not approved, the Bank may issue to me/us a Visa Classic card (for Visa Gold card applicants). 6. Applications from the Bank s staff will be subject to HSBC s staff policy Declaration My/Our information 1. I/We confirm that the information given in this application form is correct and complete. I/We authorise the Bank, among other members of the HSBC group as described in the Credit Card Cardholder Agreement available at the website of the Bank, to obtain and verify information relating to me/us from or with any other person or source at any time and from time to time as the Bank may decide. Annual fee 2. I/We accept that the annual fee of each credit card to be charged by the Bank is as follow and may be amended from time to time (as shown in the Bank Tariff Guide for HSBC Retail Banking and Wealth Management Customers.): (i) Visa Gold primary card MOP/HKD480; (ii) Visa Classic primary card MOP/HKD220; (iii) Visa Gold additional card combined account MOP/HKD240; (iv) Visa Classic additional card combined account MOP/HKD110.
My/Our employment and financial status 3. I/We declare that: (i) I am/we are not delinquent in repaying any credit facilities from any financial institution or lender; (ii) I am/we are not a bankrupt or discharged bankrupt; (iii) I/we have no intention to declare bankruptcy; and (iv) I am/we are not aware of any bankruptcy proceedings made against me/us. My/Our linked account for ATM facilities 4. I/We confirm that: (i) my/our signature(s) provided on this Acknowledgement Form is/are the same as on each of my/our savings or current accounts for which ATM facilities are requested; (ii) each such savings or current account is a single name account in the name of the related cardholder, or is operated by the related cardholder singly if it is a joint or multiple names account; and (iii) I/we will use the ATM facilities subject to the ATM Card Terms and Conditions. 5. A PIN mailer for ATM cash advance and my/our banking account access (if applicable) will be sent to me/us upon credit card issuance. If I/we do not wish to use the ATM and facilities, I/we should visit the branches for arrangement. I/We understand that without an ATM PIN, I/we will not be able to access the ATM facilities (including the credit card cash advance function through ATM). Additional card(s) 6. I/We understand that if an additional card is applied for in the application form: (i) the primary cardholder will be liable for the use of both the primary card and the additional card while the additional cardholder will only be liable for the use of his/her additional card; (ii) an additional card may be terminated by either the primary or additional cardholder by giving the Bank written notice and returning the additional card to the Bank; (iii) use of an additional card may be suspended by the Bank upon the report of loss, theft, disclosure or unauthorised use of the primary card, the additional card or any related personal identification number; and (iv) until the additional card is returned to the Bank or the Bank is able to implement the necessary procedures following a report in (iii) above, the primary cardholder may be liable for any transactions and payments arising from the use of the additional card. Terms and conditions governing credit card(s) 7. I/We acknowledge that the provision and use of the credit card(s) are subject to the terms and conditions of the Credit Cards Cardholder Agreement, Information on your Credit Card, Terms and Conditions for Using the Services of the Credit Card Customer Service Hotline, ( Terms and Conditions ). I/We understand that the Terms and Conditions, whose terms I/we have fully acknowledged and expressly agreed, are available on the Bank s website, and printed copies will be given to me/us with my/our credit card(s) upon approval of the application. 8. By signing this Acknowledgement Form, I/we confirm that I/we have read and understood the following documents related to this application and agree to be bound by them: (i) the Important Notes and Declarations set out in this Acknowledgement Form (ii) promotional offer terms and conditions (if applicable); and (iii) the Terms and Conditions. 9. I/We understand that a card collection notification was sent to me/us once the card application was approved, notifying me/us that the approved credit card and a welcome pack are available for collection. The Welcome Pack shall include the Terms and Conditions. 10. Without prejudice to the representation as made in clause 9, I/We will further read the contents of the Welcome Pack and in particular the Credit Card Cardholder Agreement and any other terms and conditions governing the provision and use of the credit card(s). Authorisation for collection and signing 12. I/We understand that I/we may authorise a person in writing to collect my/our credit card(s) on my/our behalf in the form of the delegation letter available from the Bank or the Bank s website. I/we will be responsible for any misuse of the card(s) or other consequences arising from this arrangement. Documents attached and Signature 13. I/We understand and agree that all documents supported will not be returned to me/us. Promotional Terms & Conditions Eligibility 1. The promotion period is from 4 March 2019 to 15 March 2019, both dates inclusive ( Promotional Period ). 2. A customer must submit the application form to apply for a primary card of the Macau Patacas / Hong Kong Dollar Visa Gold / Classic or Gold MasterCard / MasterCard Standard credit card issued by The Hongkong and Shanghai Banking Corporation Limited, Macau Branch (and its successors and assigns) (the Bank ) ( Eligible Credit Card ) during the Promotional Period with the application successfully approved ( Eligible Cardholder ) in order to enjoy the first two-year annual fee waiver. If an Eligible Cardholder fulfills the relevant spending requirement specified, he / she will
be entitled to a Welcome Gift according to the terms and conditions ( Promotional Offer ). All the Promotional Offers will not be granted for any application received after the Promotional Period. 3. If a designated customer s application for an Eligible Credit Card is declined by the Bank or if the customer has cancelled any of the personal primary cards of card types specified above in clause 2 on or after 1 February 2018, the customer will not be eligible for any Promotional Offer. The Promotional Offers do not apply to applications for additional cards. 4. New Customers refer to customers who do not possess any personal primary credit card issued by the Hongkong and Shanghai Banking Corporation Limited, Macau Branch ( the Bank ) in Macau at the time of the application and do not meet the restrictions referred in 3 above. 5. Each Eligible Cardholder is entitled to receive the Welcome Gift only once even if more than one Eligible Credit Card is applied for during the Promotional Period. If an Eligible Cardholder is entitled to receive the Promotional Offer under this Promotion and the Promotional Offer(s) under another concurrent promotion run by the Bank, he/she is entitled to receive only one of the Promotional Offers at the Bank s discretion. Welcome Gift 6. Eligible Cardholders are entitled to receive the welcome gift ( Welcome Gift ) upon fulfilling the relevant spending requirement ( Spending Requirement ) specified below: Welcome Gift Spending requirement for successful HSBC credit card applicants 1 visit gift card of Plaza Premium Lounge. (Retail price: USD55 per card). Each gift card could be used for one visit to the designated Plaza Premium Lounge. Spend MOP/HKD3,000 or above within first 60 calendar days of the credit card issuance i. Each Eligible Cardholder is required to fulfil the applicable Spending Requirement by conducting transaction(s) with the newly approved Eligible Credit Card within the first 60 calendar days from the issue date of Eligible Credit Card. The spending amount is calculated by adding up all spending in MOP and HKD currencies made by the Eligible Credit Card on all types of sales transactions (including but without limitation, cash advances, bill payments, insurance payments, or posted instalment payments of an instalment plan) ( Eligible Sales Transactions ). Nonsales transactions, including but without limitation, finance charges, cash advance charges, late charges, annual fees, online tax payment or the withdrawal amount of Cash Instalment Plan are not included in the calculation of the Spending Requirement. Unposted, cancelled or refunded transactions and transactions of additional card(s) are also excluded from the calculation of Spending Requirement. ii. A redemption letter for the Welcome Gift will be sent to an Eligible Cardholder s Macau correspondence address maintained in the Bank s record at the time of mailing by local mail (overseas address will not be covered) within four months upon fulfilment of the relevant Spending Requirement. The Eligible Cardholder must bring the original redemption letter and the Eligible Credit Card as specified in the redemption letter to redeem the Welcome Gift at the Bank s designated redemption centre before the specified redemption deadline. Redemption details, including the redemption method, redemption centre address and redemption deadline, will be set out in the redemption letter. 7. The Welcome Gifts are available while stocks last. 8. The observed retail price of the Welcome Gift is the price observed on the market by the supplier. The Bank is not responsible for setting the observed retail price or any difference between on observed retail price and an actual retail price in the market. 9. The Welcome Gifts (or any alternative gifts) cannot be converted into cash and their use may be subject to the terms and conditions stipulated by the suppliers. Product descriptions and images are provided by the relevant suppliers and are for reference only. The suppliers of the Welcome Gifts (or any alternative gifts) are solely responsible for the quality and availability of services provided and/or supplied. 10. Only Eligible Cardholders whose Eligible Credit Card accounts are valid and in good standing during the entire fulfilment period will be eligible for the Welcome Gift. General 11. Any fraud and/or abuse of the Welcome Gift under this promotion or cancellation of any Eligible Credit Card within 13 months of opening of the Eligible Credit Card account will result in forfeiture of an Eligible Cardholder s entitlement to the Welcome Gift. The Bank reserves the right to deduct an amount equivalent to the value of any Welcome Gift awarded to an Eligible Cardholder
inappropriately as a result of any fraud and/or abuse and an administration cost of MOP/HKD1,000 for each primary Eligible Credit Card directly from the cardholder s account held with the Bank without prior notice. 12. The Bank reserves the sole right to approve or decline any credit card applications, and the Bank is not obligated to provide reasons for declined applications. 13. The Bank reserves the right to suspend and/or terminate granting the Welcome Gifts and/or the annual fee waiver, and/or vary these terms and conditions at any time without prior notice. The Bank accepts no liability for such change or suspension/termination. The Bank reserves the right to amend these terms and conditions at any time with prior notice by way of display in the Bank s premises or website or by such other method as the Bank may decide. If the customers expressly disagree with the amendments made by the Bank, the Bank may immediately terminate any and all of its services and promotions offered under these terms and conditions. 14. In case of disputes arising out of this promotion, the decision of the Bank shall be final and conclusive. 15. In case of discrepancies between the English and Chinese versions of these terms and conditions, the Chinese version shall apply and prevail. All the terms and conditions listed above are governed by and construed in accordance with the laws of the Macau Special Administrative Region. By signing this Acknowledge Form, I/ We acknowledge and agree that the HSBC Group may collect, use, share and transfer to a jurisdiction outside of the Macau S.A.R., relevant information about me/ us, my/ our transactions and my/ our relationships with the HSBC Group to meet any requirements, either regulatory, legal or otherwise. Any information processed in such manner shall be protected by a strict code of secrecy and security which all members of the HSBC Group, their staff and third parties are subject to and may be accessed at any time on http://www.hsbc.com.mo. In addition, I/ We shall be waiving to the bank s duties of bank secrecy regarding the information sharing under the applicable terms and agree (where applicable) to the potential tax withholding thereunder. I/ We will also be given the right to request access and correction of any of the Personal Data. Any request may be made in writing and addressed to The Hongkong and Shanghai Banking Corporation Limited, Macau Branch 639 Avenida da Praia Grande, Macau, or to PO Box 476, Macau. All as better detailed in the Credit Card Cardholder Agreement hereto enclosed and I/ we have read, agreed and consented to such Collection and Disclosure of Account Holder Information described herein and in the Credit Card Cardholder Agreement.
