Eligibility Requirements To be eligible for benefits discounts the following must be completed: -2019 Wellness Program Enrollment Form -New Hire (Hired before April 1 st 2019 must complete All requirements, however those hired after April 1 st 2019 will be expected to do as much as they can in good faith if signed up for the program). -Completion of the 2018 Eligibility Requirements qualified you for the 2019 benefits discount. -Future benefits discounts are contingent upon completion of the prior year s Eligibility Requirements. -Those that do not complete the 4 remaining qualifying events, such as webinars or lunch-n-learns, etc for 2019, but sign up for the 2019 program will not receive the premium discount for 2020. General Quarterly Outline 1 st Quarter -Announcement of program along with general 2019 overview for associates (by week of Jan 21 st ) -6 Week On-going Health Management Program Targeted A1C & Blood pressure Program -January 21 st February 25 th - Targeted A1C Take advantage of the opportunity to Target one of your most important numbers for cold hard cash. If your A1C is already in range, simply maintain this number. If it is out of range, simply improve it by the designated percentage. You will be rewarded for either maintaining a healthy A1C or improving it by the designated amount. Blood Pressure is a perfect compliment to this as Health Assessments are just around the corner. Your location should make entering an accurate BP on the Health Assessment the end goal for this program. SEWS wants to raise awareness of just KNOWING YOUR NUMBERS! -Behavior Change Competition Each location will go over their strategy/programming for the year -Health Assessments (March 1 st April 1 st 2019) 2 nd Quarter -8 Week On-going Health Management Program Pick Your Plan & Stick With It -March 11 th April 29 th (Just in time for Summer) - Pick Your Plan & Stick With It Half the battle is picking a plan and sticking with it. Often we fail because we are all over the place. Example, for most the goal will be weight loss, but lets make the 1 st goal sticking with your plan. It might be Keto, It might be CrossFit, it might be Jazzercise, or walking four times a week and dropping processed foods. We ll ask you to pick your plan (a reasonable plan) and just stick with it! Lets see what happens. -Associate Blood Work (April 1 st May 1 st 2019) -Spouse Blood Work VOLUNTARY. For Details visit www.1stopwellness.net (Click on the Blood Work tab under the SEWS tab for details). Spouses must register by March 1 st in order to be eligible for free blood work. -4 Week On-going Health Management Program Declutter & Give Back Challenge -May 14 th June 3 rd - Declutter & Give Back Challenge - This is your chance. 1 st assess what needs to be decluttered, 2 nd assess what you could do with all that extra stuff. Maybe your location would like to pick a charity and give back a specific item or items. Maybe you want to donate to your own charity or local Goodwill, Salvation Army, etc. Maybe you have a lot of items you could easily sell. Well set a plan and get cleaning. Also, designate with purpose where these items will go (donations, trash, or extra cash in your pocket). If extra cash is the goal designate a purposeful place that money will go, such as emergency fund, family outing, or simply treat for yourself. 3 rd Quarter -6 Week On-going Health Management Program Shake/Smoothie Challenge -September 9 th October 14 th - Shake/Smoothie Challenge Participants will receive a free shaker cup! This is your choice, for some this is a green smoothie challenge (trying to get your greens in), or a protein shake challenge (added protein for weight loss or fitness goals), or a meal replacement challenge (better calorie and macro control for a specific meal), or healthy substitution challenge (maybe replacing nightly chips and ice cream with a healthy lower calorie alternative). Well, a Shake or Smoothie can be the answer. For this designate your goal/plan and get SHAKIN! -Delivery of Results/Health Coaching (July/August 2019) 4 th Quarter -October All about the Ladies We ll celebrate the ladies and focus on women s health issues -November All about the Fellas We ll celebrate the men and focus on men s health issues. -November 2 $1,000 Drawings. You get one entry for every non-webinar qualifying event completed. -Deadline for Completing 2019 Action Items (November 1 st 2019) -Reenrollment into the 2020 Wellness Program *You must have completed your 2019 Eligibility Requirements to receive a benefits discount in 2020 -Reporting (aggregate data regionally for Blood Work and Health Assessments) -Planning 2020 programming 3

On-going Health Management Program Programs listed in the Quarterly Outline (Count as 2 qualifying events each) Tobacco Cessation (Count as 2 qualifying events) -Independent -On www.1stopwellness.net to receive credit start with week 1 and assess your habit. In week 2 sign your personal contract to quit. Turn a copy of this into your Champion, Co-Champion, or HR Representative. This represents entry into the program. At the completion of the 12 week program please report your completion to your local Champion, Co-Champion, or HR Representative. This is done on the honor system. -Group -This is done at the discretion of each location. Please communicate with your Champion, Co-Champion, or HR Representative concerning interest in getting a group together to conduct this program. 5

Most fitness centers will provide a print out of your attendance upon request. -5K, 10K, ½ Marathon, Full Marathon, Triathlon/Iron Man, or Cycling Event -Provide proof that you participated 6
