Dear Camp Parents With the camp season quickly approaching, your camp and Lakeland Pharmacy are working together with families to have their medicines Pre-Packaged to make dosing efficient and error-free. Each medication your child takes will be dispensed by our pharmacy and packaged, sealed and sorted according to Day and Time of administration; this includes prescription, non-prescription (OTC items) and vitamins. This method of dispensing medication during camp sessions will minimize potential medication errors insuring that every day each camper gets their correct medication, the correct dose, at the correct time. All medications will be shipped to the camp prior to your child s arrival, and all unused medications will be sent home at the end of camp. If your child takes over the counter (OTC) items, such as Benadryl, Advil, Tylenol, please check with the health center as they may stock them. Our on-line registration process will take you through the steps required to register the medications for your child s correct session. Please register as soon as possible to avoid late charges associated with rush service. Please note during the registration process we request a credit card be placed on file. Whenever there is a new charge for a co-payment or deductible, OTC items, etc. we will charge your card and send you an email with the details of the transactions. Upon registration you will be able to log into the Parent Dashboard and review your child s medication, camp sessions, transactions and insurance information and make changes as required. Your patient is attending camp who employs a Medication Therapy Management System. All medication in pill form will be dispensed in dose packaging (unit or multi-dose, depending on state law). The packages are labeled with name, date, dose, administration time, etc. The pre-packaged medication will be sent directly from our pharmacy department to the camp. The deadline for a prescription to be received is 30 days prior to the start of the camp season. We need your assistance in making this as smooth as possible for your patient. Please note the following prescription guidelines: 1. Please write a prescription for all medications including OTCS, a 30-day supply with enough refills to cover the entire camp stay. 2. Controlled Substance (Schedule III-IV) please write one prescription for each 30-day supply that is needed with enough refills to cover the entire camp stay. The DEA requires all prescriptions to be dated and signed on the date issued. 3. Controlled Substance (Schedule II) please provide one prescription for each 30-day supply sequentially. Do not postdate the prescription. The body of the prescription must provide written acknowledgment that indicates the earliest date that the pharmacy may fill. For example: a prescription dated for 05/15/2016 for drug X should say 'Do Not Fill Before 06/15/2016 '. (Do Not Fill Before Date Should be 2 Weeks Prior to Session Start Date) 4. Please include NPI number and DEA number on all prescriptions. 5. Disclosure Accounting: You may request a list of instances in which we (or our business associates) disclosed your health information for purposes, other than treatment, payment, healthcare operations and certain other activities, for the last 6years, but not before April 14, 2003. You must make your request in writing by sending us a letter that specifies the type of information and the time period involved. Requests should be sent directly to the address listed in the heading of this Notice. if you request this information more than once in a 12-month period, we may charge you a reasonable fee to compensate for our time and materials. Alternative Communication: You may request that we communicate with you about your health information by alternative means or to an alternative location. I acknowledge that I have read and understand the following. 1. The registration fee is non-refundable. 2. A late fee of $30.00 will be charged if registered after the registration deadline. 3. All Controlled Drug Prescriptions should be written out for up to 30 days supply ONLY. If your child is attending camp for more than 30 days, a separate prescription is required for each 30 day period. 4. DO NOT FILL BEFORE date on controlled drug prescriptions should be 2 weeks prior to start of session. 5. All OTC'S require a prescription written by the physician. 6. All prescriptions should be written for the time of day the medicine is to be administered to the camper ( i.e. breakfast, lunch, dinner, bedtime).if the medicine is taken as needed please make sure it is specified on the prescription. 7. All prescriptions will be filled generically (if available) unless otherwise specified by your physician as Do Not Substitute. 8. All prescriptions should be received by the pharmacy 3 weeks prior to start of session or late fee of $30.00 will be charged. 9. Please make sure that you send a copy of Prescription Insurance Card or the processing of prescriptions will be delayed. 10. I have received a copy of Lakeland Pharmacy's HIPPA policies.
