Please address your envelope to Father Fallon - Financial Aid. Only complete applications received by February 1 will be considered for Financial Aid. Funds are limited. Incoming Freshmen must have an enrollment application on file with the Registrar in order to apply for Financial Aid. An application CANNOT be processed without the requested documentation. Since the financial needs of families vary, parents, step-parents or guardians desiring financial aid for their child(ren) are required to complete this form and provide the necessary documentation. Fill out one form per family. Answer items completely and honestly. Do not leave any item blank unless it does not apply to the family. Feel free to attach an explanation, which may help us better understand your family situation. Each family must submit the required documentation with the application. Do not send originals, as these documents will not be returned. A. If a parent is CURRENTLY EMPLOYED and living with the family, you must send: 1. a copy of your most recent Federal income tax form (1040 s) including all schedules (2012 taxes are ok.) Do not submit State tax forms. 2. most recent W-2 for each working parent for each job, or year-end pay stub (showing year-to-date earnings) 3. most recent pay stub for each working parent for each job, or verification of pay from an employer (verification of pay must indicate gross amount of pay each paycheck and amount withheld for taxes.) B. If the family received WELFARE OR FOOD STAMPS you must send: 1. a photocopy of a recent welfare check or a letter from your welfare technician. 2. a photocopy of a food stamp bonus card or a letter from your welfare technician. (Note: Be sure to indicate number of times each month you receive food stamps.) C.
