Be sure to also read the general terms and conditions.) 1) What is this product about? TAKAFUL IKHLAS BERHAD TAKAFUL AMANI Date: 07/05/2014 This plan is exclusively designed for Bank Kerjasama Rakyat Malaysia s (BKRM) customers to provide personal accident cover happening anywhere in the world. This coverage provides protection 24 hours a day, 365 days a year. This Takaful covers the Covered Person against Bodily Injury resulting in death or permanent disablement caused solely and directly by Accidental means unless otherwise specifically excluded. Tabarru means donation for the purpose of solidarity and cooperation among the Participants and to be used to help Participants in times of misfortune. In the context of Takaful IKHLAS, Tabarru will be allocated into the Risk Fund. Wakalah means agency. In this context, it refers to a contract between Company and the Participant where the Participant authorizes the Company to manage the fund on his/her behalf, which allows the Company to charge a Wakalah fee. Note: Please refer to the Takaful Certificate contract for more information. 3) What are the covers / benefits provided? This Plan covers: Death Permanent Disablement Immediate Death/Funeral Expense 4) How much contribution do I have to pay? The total Contribution that you have to pay may vary depending on the package chosen as per item 1. 5) What are the fees and charges that I have to pay? The following charges are applicable: What you have to pay in addition to the Contribution amount:- Stamp duty : RM10.00 1

What is included in the Contribution amount:- Wakalah Fees - Commissions : up to 25% of Contribution Management Expenses : up to 35% of Contribution What you have to pay if you cancel the Certificate :- Cancellation Fee : RM10.00 Note: Our charges exclude tax that would be imposed by the government and/or the authority in the future (including Goods and Services Tax (GST)). Upon implementation of the GST, we shall be entitled to recover the GST from you in respect of this Takaful Certificate that is required under the law. 6) What are some of the key terms and conditions that I should be aware of? Importance of Disclosure You must disclose all material facts that you know or ought to know such as your occupation, your personal pursuits which would affect the risk profile and number of personal accident certificates that you have participated from other Takaful operators or insurance companies; otherwise your Takaful Certificate may be invalidated. Memorandum In the event that the Participant or the Covered Person is covered under more than one (1) Personal Accident Certificate (whether group or individual) issued by the Company, the Participant or the Covered Person shall not be entitled to any additional benefits in addition to the Benefits hereunder to the intent and effect that only one (1) claim is made in the case of each Accident. However, payment death benefit shall not preclude payment of compensation under this Endorsement. Subject otherwise to the terms, exceptions and conditions of the Certificate. Contribution Warranty It is fundamental and absolute special condition of this Certificate of Takaful that the Contribution due must be paid and received by the Takaful operator within sixty (60) days from the inception date of this Certificate/ Endorsement/ renewal Certificate. Where the contribution payable pursuant to this warranty is received by an authorized agent of the Company, the payment shall be deemed to be received by the Company for the purposes of this warranty and the onus of proving that the contribution payable was received by a person, including a Takaful agent, who was not authorized to receive such contribution shall lie on the Takaful operator. Subject otherwise to the terms and conditions of this Certificate. Surplus The Company shall charge a Surplus Administration Charge (SAC) of 50% of the gross distributable surplus arising at the end of the financial year. However, the Company may at its discretion and where appropriate charge SAC less than 50% of the gross distributable surplus. Any net distributable surplus arising (after deducting the SAC) from the Risk Fund will be allocated in full (100%) to the Participant. If the amount due to Participant is less than RM10 per Certificate, the amount shall be retained in the Risk Fund. 7) What are the major exclusions under the certificate? This Certificate does not cover death or permanent disablement due to: war, invasion, act of foreign enemy, hostilities (whether war be declared or not) civil war, rebellion, revolution, insurrection, mutiny or usurped power, strike, riot, civil commotion, military or popular uprising where the Participant directly or indirectly participates or is involved howsoever in such 2

activities; any act of terrorism; insanity, suicide (whether sane or insane) or any attempt thereat by the Participant; pre-existing physical or mental defect or infirmity to the Participant; bacterial or viral infections, any disease or sickness, medical or surgical treatment (except such as may be necessitated solely by injuries covered by this Certificate and performed within the time provided in the Certificate) to the Participant; childbirth, miscarriage, pregnancy or any complications thereof to the Participant; provoked murder or assault by the Participant which caused his/ her death; or any breach of Civil or Shariah Law and principles by the Participant; pre-existing permanent disablement or bodily injury to the Participant; misuse or abuse of substance, alcohol and/or drugs by the Participant; any other causes prohibited by Shariah Law and principles. Note: This list is non-exhaustive. Please refer to the Takaful Certificate contract for the full list of exclusions under this Takaful Certificate. 8) Can I cancel my certificate? You may cancel your Takaful Certificate by giving written notice to us. Upon cancellation, you are entitled to a refund of the pro rata Contribution for the Period of the Takaful Certificate which has been in force. 9) What do I need to do if there are changes to my contact details? It is important that you inform us of any change in your contact details to ensure that all correspondences reach you in a timely manner. 10) Where can I get further information? TAKAFUL IKHLAS BERHAD TAKAFUL AMANI Tarikh: 07/05/2014 Pelan ini diperkenalkan secara ekslusif untuk pelanggan-pelanggan Bank Kerjasama Rakyat Malaysia (BKRM) di mana ianya menyediakan perlindungan kemalangan diri yang berlaku di mana sahaja di dunia. Perlindungan ini meliputi tempoh 24 jam sehari dan 365 hari setahun. Perlindungan Takaful ini menyediakan kemudahan perlindungan kepada Orang yang Dilindungi terhadap Kecederaan Tubuh Badan mengakibatkan kematian atau hilang keupayaan kekal. Musibah ini mestilah semata-mata disebabkan oleh Kemalangan namun bergantung kepada pengecualian yang dinyatakan secara khusus. Tabarru - bermaksud derma bagi tujuan perpaduan dan kerjasama di kalangan Peserta-peserta dan akan digunakan untuk membantu Peserta yang ditimpa musibah. Di dalam konteks Takaful IKHLAS, Tabarru akan diperuntukkan ke dalam Dana Risiko. Wakalah - bermaksud agensi. Di dalam konteks ini, ia merujuk kepada kontrak di antara Syarikat dan Peserta di mana Peserta melantik Syarikat untuk menguruskan dana bagi pihak Peserta, yang membenarkan Syarikat untuk mengenakan yuran Wakalah. Nota: Sila rujuk kepada Sijil kontrak Takaful untuk maklumat lanjut. 3) Apakah perlindungan/manfaat yang ditawarkan? Produk ini melindungi : Kematian Hilang Keupayaan Kekal Perbelanjaan Pengkebumian 4) Berapakah jumlah sumbangan yang perlu dibayar? Jumlah Sumbangan yang anda perlu bayar akan bergantung kepada jenis pelan yang diambil seperti yang tertera dalam perkara 1. 5

5) Apakah yuran dan caj yang perlu dibayar? Berikut adalah yuran dan bayaran yang dikenakan: Bayaran yang perlu dijelaskan di samping jumlah Sumbangan:- Duti Setem : RM10.00 Bayaran lain yang termasuk dalam jumlah Sumbangan Yuran Wakalah - Komisen : sehingga 25% dari Sumbangan Perbelanjaan Pengurusan : 35% dari Sumbangan Bayaran yang perlu dijelaskan jika anda membatalkan Sijil ini :- Yuran Pembatalan : RM10.00 Nota: Caj kami tidak termasuk cukai yang dikenakan oleh kerajaan dan/atau pihak berkuasa pada masa akan datang (termasuk Cukai Barangan dan Perkhidmatan (GST). Setelah perlaksanaan GST, kami berhak mengenakan caj GST bagi maksud Sijil Takaful ini yang dikehendaki dibayar di bawah undang-undang. 6) Apakah terma dan syarat utama yang perlu diketahui? Kepentingan Pendedahan Anda mestilah menyatakan semua fakta penting seperti pekerjaan anda dan perihal peribadi yang boleh mempengaruhi penerimaan perlindungan, dan bilangan sijil serta jumlah Takaful kemalangan diri yang telah anda perolehi dari mana-mana pengendali Takaful / insurans; jika tidak, Sijil Takaful ini boleh dianggap tidak sah. Memorandum Sekiranya Peserta atau Orang yang Dilindungi mempunyai lebih dari satu (1) Sijil Kemalangan Diri yang dikeluarkan oleh pihak Syarikat (sama ada berkelompok atau individu), Peserta atau Orang yang Dilindungi tidak berhak untuk menuntut manfaat tambahan memandangkan pampasan hanya dibayar untuk setiap satu (1) Kemalangan. Walau bagaimanapun, bayaran untuk manfaat kematian tidak menghalang pembayaran di bawah peruntukan Endosmen ini. Peruntukan ini adalah tertakluk kepada terma-terma, pengecualian dan syarat-syarat di dalam Sijil ini. Waranti Sumbangan Ia adalah penting dan menjadi syarat khas Sijil Takaful ini bahawa Sumbangan yang perlu dibayar mestilah dibayar dan diterima oleh pengendali Takaful dalam tempoh enam puluh (60) hari dari tarikh permulaan Sijil ini atau Endosmen / pembaharuan Sijil. Bilamana Sumbangan yang dibayar lanjutan dari waranti ini, diterima oleh wakil Syarikat yang diberi kuasa, bayaran tersebut akan dikira telah diterima oleh pihak Syarikat. Tanggungjawab bagi membuktikan Sumbangan perlu dibayar telah diterima oleh seseorang, termasuk ejen Takaful yang tidak diberi kuasa untuk menerima Sumbangan, terletak pada pengendali Takaful. Tertakluk kepada terma dan syarat yang terkandung di dalam Sijil ini. Lebihan Pihak Syarikat akan mengenakan Caj Pengurusan Lebihan (SAC) sebanyak 50% daripada lebihan boleh-agih kasar dari Dana Risiko pada setiap akhir tahun. Walau bagaimanapun, pihak Syarikat boleh mengenakan caj SAC kurang daripada 50% daripada lebihan boleh-agih kasar mengikut budi bicara dan kesesuaian. Sebarang lebihan boleh-agih bersih (selepas ditolak SAC) daripada Dana Risiko akan diperuntukkan seluruhnya (100%) kepada Peserta. Sekiranya jumlah untuk Peserta kurang daripada RM10 untuk setiap Sijil, jumlah tersebut akan terus disimpan di dalam Dana Risiko. 6

7) Apakah pengecualian utama yang terdapat di dalam sijil ini? Sijil Takaful ini tidak melindungi kematian atau hilang keupayaan kekal yang disebabkan oleh : peperangan, serangan, perbuatan musuh asing, permusuhan (sama ada peperangan diisytiharkan atau tidak), peperangan sivil, pemberontakan, revolusi, pemberontakan atau rampasan kuasa, mogok, rusuhan, kekecohan awam, kebangkitan tentera atau kebangkitan popular di mana Peserta secara langsung atau tidak langsung mengambil bahagian atau terlibat walau apa jua dalam aktivitiaktiviti tersebut; Penglibatan di dalam aktiviti keganasan; gila, bunuh diri (sama ada siuman atau tidak siuman) atau apa-apa percubaan ke arah itu oleh Peserta; kecacatan atau kelemahan fizikal atau mental sedia wujud Peserta; jangkitan bakteria atau virus, apa-apa penyakit, rawatan perubatan atau pembedahan (kecuali sebagaimana yang perlu semata-mata akibat kecederaan yang dilindungi oleh Sijil ini dan dilaksanakan dalam masa yang tertera di dalam Sijil ) kepada Peserta; bersalin, keguguran, kehamilan atau apa-apa komplikasi yang berkaitannya kepada Peserta; perbuatan asakan atau serangan yang dimulakan oleh Peserta yang menyebabkan dirinya terbunuh pembunuhan atau serangan ke atas Peserta; sebarang perlakuan yang melanggar Undang-undang Sivil atau Shariah dan prinsip-prinsipnya oleh Peserta; hilang keupayaan kekal yang sedia ada atau kecederaan tubuh badan Peserta; penyalahgunaan bahan, alkohol dan/ atau dadah oleh Peserta; sebab-sebab lain yang dilarang oleh Undang-undang dan prinsip-prinsip Shariah. Nota: Senarai ini bukan senarai yang lengkap. Sila rujuk Sijil kontrak Takaful untuk senarai penuh bagi pengecualian di bawah Sijil Takaful ini. 8) Bolehkah saya membatalkan sijil takaful ini? Anda boleh membatalkan Sijil Takaful ini dengan memberi notis bertulis kepada pihak kami. Selepas pembatalan, anda layak mendapat pulangan Sumbangan untuk kadar pro rata bagi tempoh Sijil Takaful yang telah berkuatkuasa. 9) Apakah yang perlu dilakukan sekiranya terdapat perubahan kepada maklumat untuk dihubungi? Adalah penting bagi anda untuk memaklumkan pihak kami tentang sebarang perubahan mengenai maklumat anda bagi memastikan semua surat-menyurat dapat disampaikan kepada anda tepat pada masanya. 10) Bagaimanakah saya boleh mendapatkan maklumat lanjut?
