SCBD 2018-19 Fall & Spring Registration Instructions Levels 5B and under Wednesday, May 23 at 1721 25 th St. Suite 120 5:30-6:30 (Group 1 Students enrolled in both Fall/Spring 2017-18 AND Summer 2018) 6:45-7:45 (Group 2 Students enrolled in either Fall/Spring 2017-18 OR Summer 2018) 7:45-8:15 (Group 3 - New Students OR Students enrolled before Fall/Spring 2017-18) Doors will open at 4:00 p.m. to line up. Please note you are registering for both the Fall and Spring semesters! If you cannot be at registration you have two options: 1. Ask another person to bring your form who is in the same registration group. Each person may bring one other family's registration in the same group. 2. Drop off your registration ahead of time and we will register you at the end of your time slot, but before the next group. Registrations may be dropped off in the locked mailbox by the office door outside Studio D before recital or the fill in form emailed to scbdwdm@gmail.com as a PDF before 10:00 p.m. on Tuesday, May 22nd. Forms submitted after Tuesday may not be registered on time. Class Choices and Wait List Classes will close so we highly recommend you list class 3 class choices on your form. This includes electives for levels 4 and up. If you are wait listed for a class(es), you will not be charged for the wait list classes. If you are added to the class following registration, you will have an amount due. We always recommend staying on the Wait List for a class you never know if a spot will open At Registration 1. While waiting in line you will be handed a color-coded sheet with your personal information such as address, phone, etc. Please make sure you review and update any information. 2. Once we begin to register, you will proceed to a registration helper. Once completed, a confirmation of classes will be immediately emailed. Please note this is NOT an invoice, but does include a summary of charges. Please review the confirmation email as this will list all of your enrolled and wait listed classes. Payments at Registration on May 23 for Levels 5B and under Because we are registering earlier this year, we are giving you two payment timing options pay at registration or pay Monday, July 16 th. There will be two boxes in the upper right hand corner of the registration form to choose Pay Now or Pay 7/16/18. Please select one. If one is not checked we will assume you are paying now. Payments will be handled the following ways: 1. Check this is the fastest! You may give your check to the person registering you and you are done. Please have your check dated and made out to SCBD, but DO NOT fill in the amount until we confirm your total. If you are choosing to pay July 16 th, please date your check 7/16/18. 2. Credit Card you must proceed to the payment line so we can swipe your credit card or if paying later, verify we have everything we need. 3. Installments you must proceed to the payment line to process a $26.50 payment via credit card at the time of registration. This is due now even if you choose the pay 7/16/18 option. All other fees, registration, recital and costume fees may be paid by either check or credit card now or 7/16/18.

Installment Option You may choose to pay via installments for a $25.00 per semester fee. To sign up for installments, you will need to provide a credit card or bank account on file on our attached form. When you register, you will pay registration fees, installment fee, recital fees and costume fees. Tuition is divided in 4 for your fall installments due 8/15, 9/15, 10/15, 11/15 and spring installments due 12/15, 1/15, 2/15, 3/15. 1. You will receive an emailed invoice on the 1 st of each installment month. This invoice will have a link to pay online through a secure website with Intuit Quickbooks. You may pay by either bank account or credit card. 2. Installment payments must be paid online. 3. Please note payments are due by the 15 th. After the 15 th, a $20 late fee will be added and the total will be charged to your bank account or credit card you have provided at registration.

I/we agree that I/we will be liable for injuries sustained or illnesses contracted by the participants while in attendance/or participating in classes at SCBD. I/we understand that, as a dance student, the student may be physically touched in the course of dance training and instruction. I/we will not hold SCBD liable for physical touching which is incidental to and in the course of dance training and instruction. I/we agree to indemnify SCBD and all employees and volunteers of both, for all liabilities, costs and judgements arising from acts or omissions committed by me/my child which result in injury or damage to any person or party. Other Important Policies: * We have a no refund policy. We do not issue refunds. * Injuries/illnesses greater than 4 weeks may receive a tuition credit with a doctors note. Installment Information 2018-19 Please keep for your records You may choose to pay via installments for the 2018-19 school year with a $25.00 per semester fee. For the Installment Option, we require a credit card on file with us. Therefore, at registration, your credit card will be charged $26.50 for the first semester installment. If your installment payment is late, you will be charged a late fee and we will process your payment. Note: if you are registering on May 23 rd and choose the Pay 7/16/18 option, the $26.50 credit card payment must still be processed at registration. Upon registration, you will pay registration fees, installment fee, recital fees, and costume fees. You may pay either by check or credit card. For those registering later, you will also pay any installments due. Tuition and the Installment fee is divided into 4 payments per semester. School of Classical Ballet and Dance Policy Information as of 05/09/18 for School Year 2018-19 Email and Phone Our communications are via email. It is very important for you to provide us with a current email address(es) and that you check this email account. One primary contact phone number is also necessary. Please read each email and DO NOT REPLY to a group email. Voice messages will be returned between the hours of 9:00-1:00. Enrollment When you enroll, you reserve your child s spot in that class and commit to the class for the Full School Year Fall and Spring semesters No Refund Policy We have a NO refund policy. Credits must be used by the same account in the following session or they are lost. Please note the Recital section concerning Recital and Costume Fees. The only exception to the refund policy is for injuries or a medical condition that prohibits the student from dance for more than 4 months. Injuries/illnesses greater than 4 weeks to may receive tuition credit with a doctor note. Tuition and Payments At registration you are enrolling for the full school year 2018/19. Tuition for the first semester is due at registration, along with registration fees, recital fees and costume fees. Second semester is tuition only and is billed January 1 via email and due January 15 th. Your invoice will have a link to pay online via your bank account through a secure website with Intuit Quickbooks. There is a 10% discount for more than one family member enrolled. SCBD reserves the right to drop a student for non-payment. All Invoices are emailed and due within 15 days. After this, a $20 late fee will be assessed and the invoice will be emailed again. Invoices more than 30 days past due will receive an additional late fee. Fees - The following fees will be charged. These charges are before tax. 1. Late payment fee - $20.00 2. Insufficient funds checks and returned payment fees - $25.00 3. Drop fee - $10.00 Installment Option You may choose to pay via installments for a $25.00 per semester fee. Recital Recital and costume fees are due at registration. A student is not required to participate in the recital if he/she does not wish to. You will automatically be charged recital fees unless you specifically tell us your child will not be participating. Recital and costume fees are not refundable and will not be credited. The student must perform in the recital to receive their costume. * For students not participating in recital, we expect to stay in class and learn the choreography. This will benefit the student and is respectful to all. Your children are your responsibility As the parent, you are responsible for your child. Teachers are in classes teaching and cannot watch your children. You must be there on time to pick up your child from class. Students must be instructed to stay inside if they must wait on you. If we must wait after hours for you to pick up your child, you will be charged the hourly private rate. Dropping a Class(es) To drop a class, email us at scbdwdm@gmail.com with the subject DROP. Your drop will be verified with a reply from us. You will be charged for and must pay until a drop is received. If the class is paid in full, there will be No Refund, but you may have partial credit. Drops received after week 9 will not receive credit. * There are no credits for Summer Classes. Attendance We are a ballet school and place great emphasis on attendance. Many classes have a waiting list. If attendance is not maintained, SCBD has to the right to drop a student from the class without refunds or credits. During the spring semester, our teachers also have the right to drop a student from the recital dance for lack of attendance. Please notify us via email, scbdwdm@gmail.com, if your child is ill and we will note that on their attendance sheet. Behavior Problems A student may be asked to leave a class for poor behavior. Students may be dropped without refund or credit for behavior. Poor behavior may be in or out of the classroom. Weather Cancellations In bad weather it is extremely important to check your email and the website. Classes cancelled due to weather will not be rescheduled. Private Lessons and Choreography All Private Lessons and/or Choreography from a SCBD staff member or student must be arranged and billed through the school. Please contact us at scbdwdm@gmail.com. Rates are $68.00 per hour for privates and solo choreography fees start are $425.00. Choreography fees include 4 hours of choreography. 2

For example, if a student takes a one hour class each week, tuition would be $265.20 per semester. All money is due at registration which includes 1st Semester Tuition, Registration fee, Recital fee, and Additional costumes. You are automatically enrolled in 2nd Semester and tuition only is billed January 1. You may choose installments for an additional $25.00 per semester. If choosing installments: Registration fee, Recital fee, and Additional costumes are due at registration. Tuition and installment fee is divided into 4 payments each semester.

Pre-Ballet/Tap - Pink leotard, pink tights, pink ballet shoes, black tap shoes, hair tied back out of face. Level 1 Ballet/Tap - Black leotard, pink tights, pink ballet shoes, black tap shoes, hair tied back in ponytail. Level 1 Hip Hop - Girls wear any color leotard and hot shorts or comfortable pants and jazz shoes. Boys any shirt, shorts or pants easy to move in and hip hop or jazz shoes. Hair tied back out of face. Level 2 Ballet/Jazz/Tap - Black leotard, pink tights, pink ballet shoes, black tap shoes, hair tied back in pony tail. Black jazz shoes are optional for jazz. Level 2 Hip Hop/Funk Tap - Girls wear any color leotard and hot shorts or comfortable pants and hip hop shoes or jazz shoes and black tap shoes. Boys wear any shirt, shorts or pants easy to move in and hip hop shoes and black tap shoes. Level 3 and Level 4 Ballet - Girls wear black leotard, pink tights and pink ballet shoes. Tights must be worn over feet. No leg warmers. Hair needs to be in a bun. Boys wear white shirt, black shorts or tights and black ballet shoes. Boys may need black tights for recital. Level 5 Ballet/Pointe - Girls wear royal blue leotard, pink tights, and pink ballet shoes/pointe shoes. No leg warmers. Hair needs to be pulled back nicely in a bun. No dangly earrings, bracelets or necklaces. No hot shorts in ballet. Tights must be worn over feet. Dancers may not wear JUNK in class. Boys wear white shirt, black tights and black ballet shoes. A black leotard will be required for recital. Level 6, 7 & 8 Ballet/Pointe - Girls wear navy blue leotard, pink tights, and pink ballet shoes/pointe shoes. Boys wear a black or white fitted shirt, black tights and black ballet shoes. A black leotard will be required for recital. Dancers should also have in their wardrobe a black wrap skirt. Jazz - Any color leotard, tights or hot shorts and black jazz shoes. Lyrical - Any color leotard, tights or hot shorts and flesh colored ballet shoes. Tap/Funk Tap - Any color leotard, tights, hot shorts or pants and black tap shoes. Levels 5/6, 7/8 and 9 TAP need T strapped high heeled black tap shoes. Funk Tap will need black tap shoes that look like jazz shoes. Hip Hop - Any color leotard, hot shorts or sweat pants. Levels 1-4 need black jazz shoes for hip hop and levels 5-9 may wear black hip hop shoes or clean tennis shoes. Contemporary - Any color leotard and tights or hot shorts. Ask the teacher what kind of shoes to wear. Sometimes barefoot, ballet shoes, pointe shoes.
