How to Select Your Open Enrollment Benefits for 2014 Overview: The following Step-Action table will walk you through the process of selecting your benefit plans for 2014. Open Enrollment for 2014 benefits starts October 14, 2013, and ends on Monday, November 18, 2013 Step Action 1 Click the Oracle Icon on your computer desk top. 2 To login to the Oracle System: 1. Type your Given name (no nicknames) in the User Name box. Type a period between your first and last name. Example: james.smith not jimmy.smith. 2. Password: Type Welcome then add the last four digits of your Social Security number. Be sure to make the W a capital letter. (example: Welcome9876) 3. Click the button 2
3 You will be prompted to change your password. 1. Type your current password (i.e. Welcome9876) using the last four digits of your Social Security Number. 2. Type your New Password. (It must be at least 8 characters long, contain at least 1 number and no repeating characters) 3. Re-enter your New Password. 4. Click button. 4 From the Oracle Applications Home Page, click the link under the Main Menu. 3
5 Click the link. 6 1. Read the Employee Benefits Medical Disclaimer. Note: If you select Decline, you will not be able to elect your benefits. 2. Click Printable Page should you desire to have a copy of the disclaimer for your records. 3. Click to continue your benefits selections. 4. Click button to go to the next page. Tip: Next is also the Save the information of this page button throughout the sections. 4
7 Dependents and Beneficiaries are carried over from the 2013 Open Enrollment however; they haven t been assigned to your 2014 choices yet. 1. Click the pencil to update information. 8 To add another person, click button. 9 Type in the name and relationship information, then click the Tip: * Indicates required field. button. 5
10 1. Click box if the person you added has the same address as yours. 2. Finish adding addresses for the new beneficiaries or dependents as well as the Miscellaneous Information. 3. Click button to save your information. 11 Tip: To assist you in making your elections, you were previously provided with your 2013 Employee Benefit Report with your personal elections. It may be helpful to have that document on hand to complete the election process. Please refer to your Open Enrollment Booklet for coverage details. 12 The following categories will be your plan options. These include five elective benefits. Additional elective benefits will be featured at the end of this document. To begin electing your benefits, click the button. 13 The top menu bar will guide you through the enrollment process: As you move through the process, the yellow ball will move to the right. 14 The choices listed below may vary based on family member information. Please add any dependents or beneficiaries you want to cover or designate that have not been added in the system. You will need to enter the social security number and birthdate for each. Tip: If the dependents/beneficiaries did not have a social security number in the current system, they will not appear in the list. 15 Select the plan of your choice by placing your cursor in the box and clicking. A checkmark will appear in the box. If you are waiving the Medical Plan, be sure to select the Waive Medical Coverage box. The 2014 cost for your election will be listed in the Pay Period Before Tax Deduction column to help you make your decisions. 7
16 Select your Dental Plan or be sure to select Waive Dental Coverage. 17 Dental election in the PDP plan will not require you to pre-select your dentist. When electing the DHMO plan (Safeguard Met 3768, Group #149992), you will be asked to select a dental Primary Care Provider for each plan member. You ll need to go to the MetLife website at www.metlife.com and select a Provider and note their Provider ID. 1. Select the Dentist for each member of the family. 2. Click to go to the next page. 8
18 This is an elective vision plan, available through Advantica separate from the basic vision coverage available under the UnitedHealthcare health plan. Select one option or waive the coverage. 19 Select Medical Flexible Spending Account (FSA) Tip: The federal law limiting annual contributions in the Medical FSA to $2,500 per year does not affect the Dependent Care FSA limit of $5,000. 20 Select Dependent Care Flexible Spending Account (FSA) Tip: The federal law limiting annual contributions in the Dependent Care FSA is $5,000. 21 Your Life and Accidental Death and Dismemberment insurance is provided at no cost to you. The coverage amount is equal to your annual salary. 9
22 Select a Long Term Disability option. Tip: All full-time employees automatically receive the City-paid base plan that replaces 30% of income up to a maximum of $7,000 per month. You may increase your coverage by one level during Open Enrollment. Your 2013 Employee Benefit Report reflects your current 2013 coverage (you may increase it to the next level). 23 Select a Deferred Compensation option, enter the monthly amount you would like to elect, then scroll down and click the button to find out the cost per pay period. 24 Tip: Designation of beneficiaries to receive unused accruals and unpaid earned wages. 25 26 Verify your Dependent Selections 10
27 Determine the percentage for your Group Life and Last Paycheck beneficiaries. Tip: Total percentages for the plan must equal 100%. Click the refigure the percentages. button to 28 Update your Primary Care Provider if you need to change your DHMO dentist. Click to go to the next screen. Tip: Notice your guide at the top of the page indicates there is one more step to go! 11
29 1. Review your choices. 2. You will not receive a confirmation of your 2014 elections from the City of Tampa. 3. If you want a Confirmation Statement, you can save or print your confirmation statement by clicking the button. 30 Click when you are finished! 31 During the Open Enrollment period, October 14, 2013 to November 18, 2013, you can change or update your benefits. If you make any changes, be sure to print a new Confirmation Statement for your records! Click to do so. You will be back at the beginning of the enrollment process. 32 You have additional Elective Benefits. You may read about them in your Open Enrollment Employee Benefits Booklet.
