2017 Annual Enrollment Process and Requirements Do I need to make new benefit elections for 2017? 2017 will be a Passive Annual Enrollment What is a Passive Enrollment? Your current 2016 elections, except for Flexible Spending Accounts (FSA), will automatically roll over as your 2017 elections, except. If you want to contribute to the Health Care FSA, Limited Purpose FSA or Dependent Care FSA in 2017 you must make this election during annual enrollment If I make a change to my current elections or elect new coverage during annual enrollment, when is the change effective? o Changes made during annual enrollment will be effective on January 1, 2017 4

New Medical ID cards You will receive a new medical ID card o if you are enrolled in medical coverage administered by BCBS or UnitedHealthcare in 2017 o regardless of whether or not you elect a new medical plan option for 2017 o should arrive at home address prior to January 1, 2017 o o Make sure your home address shown in ADP Vantage is correct If not, log into Vantage to change it or call the HR/Payroll Shared Services Center to update it o Be sure to show your new medical ID card to all providers you see on or after January 1, 2017 Express Scripts ID cards are not changing, therefore should keep your current card to use in 2017 9

Important Reminders Life Events and Dependent Verification Changing Benefit Elections Due to a Life Event Qualified life event = marriage, divorce, birth/adoption of a child, or gain/loss of other coverage Must notify Veolia Benefits Center within 31 days of the event in order to change your current elections You do not immediately need a newborn s Social Security Number or birth certificate to add the child to coverage, however you must call within 31 days of the child s birth Letter requesting you submit documentation supporting the life event will be mailed to your home Must fax, mail or upload documentation by the stated deadline Failure to provide this information by the stated deadline will result in the change being reversed Verification of Dependents Following the addition of any newly-added or newly-eligible dependent(s), including a dependent(s) added during annual enrollment, you will receive a letter requesting you submit documentation supporting in order to verify the dependent s eligibility Must fax, mail or upload documentation by the stated deadline Failure to provide this information by the stated deadline will result in loss of coverage for the new dependent(s) 14
Important Reminders New Hire/Newly Eligible Benefit Elections and Flexible Spending Accounts New Hires/Newly Eligible Employees Regular (not temporary) employees scheduled to work 30+ hours per week are eligible for benefits the first of the month following date of hire You must enroll in benefits or actively waive coverage within 45 calendar days of your hire date or the date you become eligible for benefits If you do not elect or actively waive coverage, you will be enrolled in default coverage and the associated cost will be deducted from your pay Default coverage includes HSA Silver medical coverage (employee only), basic life and AD&D insurance, core long-term disability, short-term disability and EAP Temporary, non-union employees scheduled to work 30+ hours per week are eligible for medical coverage only Eligible for coverage the first of the month following 60 days You must elect or actively waive medical coverage beginning 15 days prior to your coverage effective date and ending 30 days after your coverage effective date Default coverage (HSA Silver, employee only) will be assigned if you do not elect or waive by the deadline Flexible Spending Accounts (FSA) o o o A new election is required each year to participate in the Health Care, Limited Purpose and/or Dependent Care FSA. Current elections will not carry over into 2017 An FSA debit card will be issued to participants, if you are enrolling for the first time. FSA amounts elected for 2017 will be added to current HealthHub FSA debit cards If you are currently participating in an FSA, make sure you incur expenses by December 31, 2016 to use the rest of your 2016 funds. The deadline to submit 2016 expenses is March 31, 2017 15
Important Reminders The HSA Medical Plans and Health Savings Accounts How the HSA Medical Plans Work The HSA Gold or HSA Silver medical plans may be right for you if you: Rarely have enough medical expenses during the year to meet the annual deductible Want an account you can use specifically to budget for health expenses that apply to your deductible and other out-of-pocket health, dental and vision care costs Want to build interest-bearing savings to help with future health expenses (for example, in retirement) Keep in mind that in the HSA medical plans: You are required to pay 100% of your covered expenses, including prescriptions, until you meet the individual or family deductible before the Plan pays a benefit Health Savings Account (HSA) You are allowed to contribute pre-tax dollars up to the IRS limits to an HSA You can elect, change or cancel your HSA contributions at any time If you are enrolled in the HSA Gold Medical Plan, Veolia makes a contribution each pay period to your HSA up to an annual maximum of $750 (individual coverage) or $1,500 (other coverage tiers) For more information, watch the HSA video on YourVeoliaBenefits.com 16
