Monthly plan premium $71 You must continue to pay your Medicare Part B premium. Deductible Maximum out-of-pocket amount responsibility (does not include prescription drugs) This plan does not have a deductible for medical services. There is a $350 deductible for prescription drugs. Drugs in Tier 1 and Tier 2 are excluded from the deductible. There is a $50 deductible for supplemental comprehensive dental benefits. $6,700 annually The most you pay for copays, coinsurance and other costs for medical services for the year. 2

After your total drug costs (including what our plan has paid and what you have paid) reach $3,820, you will pay no more than 37% coinsurance for generic drugs or 25% coinsurance for brand name drugs, for any drug tier during the coverage gap. After your yearly out-of-pocket drug costs (including drugs purchased through your retail pharmacy and through mail order) reach $5,100, you pay the greater of: 5% coinsurance, or $3.40 copayment for generics (including brand drugs treated as generic) and a $8.50 copayment for all other drugs. Medical Benefits (Part C) Inpatient hospital coverage $300 copayment each day for days 1 to 6 and each day for days 7 to 90 each day for days 91 to 150 (lifetime reserve days) Our plans cover up to 90 days for an inpatient hospital stay. Our plans also cover 60 lifetime reserve days. These are extra days that we cover. If your hospital stay is longer than 90 days, you can use these extra days. Once you have used up these extra 60 days, your inpatient hospital coverage will be limited to 90 days. All elective inpatient admissions require prior authorization. All other admissions will be reviewed for medical necessity and authorization. Urgently needed services $65 copayment each visit Diagnostic services/labs/imaging Diagnostic tests and procedures Lab services Advanced radiology services (such as MRI, PET, CT and nuclear medicine) Outpatient diagnostic imaging tests (such as X-rays, ultrasound and mammography) Therapeutic radiology (such as radiation treatment for cancer) $250 copayment $30 copayment 20% coinsurance PCP or Specialist copayment also applies if service is provided during an office visit. Medical Benefits (Part C) Comprehensive dental services The plan pays $500 a year toward supplemental comprehensive dental services (after a $50 deductible). Prior authorization may be required. Vision care Exam to diagnose and treat diseases and conditions of the eye Eyewear after cataract surgery Routine eye exam Supplemental eyeglasses or contact lenses $50 copayment Limited to 1 visit every year $260 limit toward eyeglasses or contact lenses every 2 years Mental health services Inpatient visit $225 copayment each day for days 1 to 7 and each day for days 8 to 90 each day for days 91 to 150 (lifetime reserve days) Our plans cover up to 90 days for an inpatient mental health hospital stay (190-day lifetime psychiatric hospital limit applies). Our plans also cover 60 lifetime reserve days. These are extra days that we cover. If your hospital stay is longer than 90 days, you can use these extra days. Medical Benefits (Part C) Fitness program Health advice line The plan pays for annual membership at participating fitness centers or a Home Fitness Kit. The plan covers access to a health advice line for help with non-emergency medical problems or questions. Home health care Podiatry services Medicare-covered services include: Diagnosis and the medical or surgical treatment of injuries and diseases of the feet (such as hammer toe or heel spurs). Foot care for members with certain medical conditions affecting the lower limbs. $50 copayment Referral is required. Pulmonary rehabilitation services $30 copayment 8

Visit www.hppmedicare.com or call 1-866-901-8000 to view a copy of the EOC. o Review the Provider & Pharmacy Directory (or ask your doctor) to make sure the doctors you see now are in the network. If they are not listed, it means you will likely have to select a new doctor. o Review the Provider & Pharmacy Directory to make sure the pharmacy you use for any prescription medicine is in the network. If the pharmacy is not listed, you will likely have to select a new pharmacy for your prescriptions. Understanding Important Rules o In addition to your monthly plan premium, you must continue to pay your Medicare Part B premium. This premium is normally taken out of your Social Security check each month. o Benefits, premiums and/or copayments/coinsurance may change on January 1, 2019. o Except in emergency or urgent situations, we do not cover services by out-of-network providers (doctors who are not listed in the Provider & Pharmacy Directory).
