Don't send cash. Write the following on your check or money order: - "28 Form 14" - Your name and address - Your daytime phone number - Your Social Security Number On the right side of the check or money order write the dollar amount of the payment, like this: $5156.. Don't staple or otherwise attach the payment to the return. Instead, just place it loose in the envelope with the return.
