Plan Features Provider choice: Members have direct access to their choice of providers. Coinsurance levels are lowest for In- Network providers. If a member chooses an Out-of-Network provider, the member may be required to pay costs above the Out-of-Network allowed amount. Office visits and professional services performed in a provider's office, such as injections or office surgery, are not subject to the deductible for In-Network providers. In addition, the first $400 of outpatient radiology and laboratory services per calendar year are not subject to deductible.
